Upon arrival, my visit was with April Carlson, Assistant Director. My visit was finalized with Ashely Smith, Director, once she arrived toward the end of my visit. The facility currently operates with a 5-star license issued August 2024. I conducted a partial assessment monitoring for safety issues, supervision, capacity, discipline, program records, posted documents, permit restrictions, medications, allergy lists, safe sleep charts, feeding schedules, and I spot checked staff files for completion of BSAC, CPR/1st Aid and SIDS. One new employee (TK) has been hired since my last visit; during my visit, she contacted the facility; her employment was terminated. The children were observed playing outside and interacting with staff during circle time. ***Verification of Inspections: *** *The last fire inspection was on 6-23-25. Email me your 2026 fire inspection within 5 days of completion. *The Last sanitation inspection was on 11-24-25. Forward your 2026 inspection to me immediately upon completion. *The last outdoor inspection was completed on 12-15-25 by A. Carlson, she completed playground safety training on 10-9-17. *The EPR plan was updated on 3-17-25. To maintain compliance with NC laws and rules regarding childcare operation, the following violations must be corrected immediately. Please email me a signed and dated corrective action letter with your ID # and facility name BY 1-21-26, to michelle.ezzell@dhhs.nc.gov. Address each violation/item number separately explaining how the violation is now corrected. Violation Number Comment Rule 805 Fire drills were not practiced monthly and/or the drill record was incomplete. The last fire drill recorded on the log was on October 31, 2025. No fire drills were recorded for November and December. .0604(t); .0302(d)(5) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In space 5, one unused outlet was not covered. 10A NCAC 09 .0604(c) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last recorded Shelter-in-place or lockdown drill was April 2025. .0604(u);.0302(d)(8) *We discussed the importance of keeping electrical outlets covered when not in use. You may want to purchase outlet covers that are attached to each other, so the cover hangs beside the outlet when not in use; and the employee will have accessibility to the cover vs. trying to locate an outlet cover. *We discussed when fire drills, shelter-in-place & lockdown drills are conducted have the fire drill record log sheet in your hand as a reminder to immediate log them. Your last annual compliance was on 6-17-24. A checklist was used to note the requirements I monitored. I monitored posted documents, safe sleep charts, allergy lists, etc. The center currently operates with a 5-star license, operating at 30sq. ft. inside, & ratios at .2818 (b). All classrooms were monitored for compliance. Space 7 was not in use and space 8 is used for storage. My visit was conducted with you, Ms. Ashley Smith, Director. Ms. Smith has updated the ABCMS for her staff. I observed the children playing outside, playing in centers and preparing for lunch. The infants were being fed, playing on the floor with providers and one was taking a nap. VERIFICATION OF RECORDS: *Last fire inspection was on 7-10-24. Remember to email me your 2025 fire inspection within 5 days of the inspection. *Sanitation inspection=5-8-25 (email me all your inspections to me after 11-6-23) *The last outdoor inspection was on 5-30-25 by April C. April completed playground safety training on 10-9-17. *EPR training was completed by Ashely Smith on 10-15-22 & the EPR plan was updated on 3-17-25. *The lead water test will expire 2-12-27. *Current Curriculum-Creative Curriculum & Learn Everyday by Kaplan. *Current insurance & registration was on 3 vehicles=2005 GMC Savannah Express, 2008 Ford E350 & 2017 Dodge Grand Caravan. The Dodge Caravan was not at the facility during my visit. *I reviewed the NC Secretary of State’s website and observed the owner of the facility (Pam’s School of Raleigh, Inc.) listed as current. The following violations were observed and corrected during the visit. No corrective action letter is due. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In space six, five unused outlets located on a power strip were not covered. 10A NCAC 09 .0604(c) 1115 First aid kit and/or fire extinguisher was not located in vehicle; kit and/or extinguisher was not mounted or secured if in passenger area. The fire extinguishers located in both vans were not mounted or secure. One was on the front seat of the van. 10A NCAC 09 .1003(c) *****TECHNICAL ASSISTANCE/REMINDERS: ******* *Electrical outlets not in use and accessible to children must be covered with socket protectors. This includes outlets on power strips. *The fire extinguishers located in the vehicles should be mounted or secured. This will prevent the extinguisher from moving around if the driver makes a sudden stop or makes a sharp turn. *We discussed different ways to capture the required signatures for children’s records. *Remember to keep the storage unit on the playground locked when it’s not in use. ****Rated License Reminders***** *Staff’s education information should be current & on file with WORKS/Workforce. Several classrooms did not have an updated cot/mat chart. 15A NCAC 18A .2821(b) & (c) 847 Parent's medication authorization did not include required information. One permission to administer medication form did not have an amount for diaper cream and one form was missing the parents signature. 10A NCAC 09 .0803(4)(6-9) 1311 Emergency medical care information was not on file in the center on the child's first day of attendance and/or was not updated as changes occurred or at least annually for each child. The emergency information form for two out of three children was not updated. .0802(c) *****TECHNICAL ASSISTANCE/REMINDERS: ******* *Children’s emergency information must be updated as changes occur and at least annually. We discussed different ways to capture this information. *All permission to administer medication forms must be signed by the child’s parent/guardian. When indicating how much diaper cream needs to be administered, its suggested to put “dime size, quarter size, etc.” *Cot lists should be current in each classroom. *We discussed the fence on the playground should touch the ground; landscaping timbers may help with the erosion issue. *Remember to continue to update your Emergency Preparedness and Response (EPR) Plan every year and print out the cover page showing the current date. Also, print the pages you updated. Failure to log onto the website may result in losing your entire EPR plan.
