The center currently operates with a 5- star license, operating at 30sq. ft. inside, & enhanced ratios and space issued on April 24th, 2024. My visit was conducted with you, director Diaa El Aridi and center staff. The facility acquired their NAEYC accreditation on September 8th, 2025. We discussed the QRIS pathway #3 and I collected the facilities application for rated license, the NAEYC accreditation congratulatory cover letter and accreditation certificate and informed you that I would be completing the necessary paperwork for your accreditation rated license to be processed and mailed to you following today’s visit. I monitored the required posted documents (permit/restrictions, EMC, posters, interactions between staff and children, discipline, supervision, group sizes, space capacity, program requirements, equipment, furnishing requirements, and program records. Transportation was not monitored all pickup and drop off is done by the parents of the enrolled children. VERIFICATION OF RECORDS: *Last fire inspection= 3/20/2025 and approved for daytime care only *Sanitation inspection= 12/30/2025- 12 demerits and a superior rating *Compliance history as of today’s visit 97% *Fire drill- 2/18/2026- make sure to complete one before the end of the month per requirements. *Shelter in place- due to the threat of tornadoes a practice drill was conducted on 3/16/2026 *Playground inspection- 3/15/2026 *EPR plan updated last updated 7/2/2025 *ABCMS criminal record qualification portal not complete and a violation of childcare requirements was cited. *One items of non-compliance was observed today and requires a letter of corrective action. Violation Number Comment Rule 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The childcare provider did not complete the ABCMS criminal record provider portal and did not link staff. G.S. 110-90.2 & .2703(r) technical assistance: A violation of childcare requirements in the area of criminal records specifically in the creation requirement of the ABCMS provider portal and connecting all staff to the portal. At the conclusion of today’s visit to make sure this was successfully completed in a timely manner I provided you with a power point presentation via email with step by steps to complete both portals as well as provided you with a name and contact in the criminal record unit in case you had questions during completion. A violation of childcare requirements was cited during today’s visit and requires a letter of corrective action by September 30th, 2026. Please document your facility name, ID #, item # of the violation, how you corrected the violation and how you will keep in compliance going forward. Upon arrival, I shared the allegations regarding uncovered outlets with you. Jayla Peterkins, assistant director, assisted me during today’s visit. Staff/child ratio, group size, supervision, use of licensed space, space capacity, license restrictions, and the indoor environment were monitored during today’s visit. Based on information provided in the report, I did a walkthrough of the facility and checked all outlets in the hallways and classrooms to ensure they were all covered if not in use. During today’s visit, all outlets that were not in use were covered in the classroom for one year old children. I observed one uncovered outlet in the hallway as well as two uncovered outlets in a preschool classroom, this was corrected during today’s visit. In some classrooms outlet covers were clear and it was hard to tell they were covered. The teacher in the classroom for preschool age children stated that the children remove the outlet covers. If teachers are moving about the indoor environment, interacting and engaging with the children, the children won’t have an opportunity to remove the outlet covers. Based on my observation of covered outlets in the classroom for one year old children, the allegation was unsubstantiated. The following violation was observed. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. One outlet in the main hallway and two outlets in a classroom for preschool age children were observed. 10A NCAC 09 .0604(c) Childcare programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation documented must be corrected immediately. By March 16, 2026, I must receive a compliance letter that describes accurately and in detail how and when the violation was corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. Consider adding a note near the outlets to serve as a reminder to re-cover the outlets. Review this with teachers during orientation as part of the observation of daily routines. Hold a staff meeting to review the different ways you all can work together to maintain compliance with childcare requirements regarding outlet covers.
