This facility currently operates with a Notice of Compliance issued on 5-23-23. The last annual compliance visit was conducted on 4 -9-25. Today’s visit was conducted with you, Joyce Gupton, center director. Upon arrival to the facility, I was greeted by you. A walk-through of the facility monitoring the eight (8) classrooms where children were in care was completed. Compliance with staff/child ratios, supervision, discipline, program requirements, storage of hazardous substances, CPR & First Aid, permit restrictions and various staff records documentation was monitored as a partial assessment of applicable child care requirements. A sampling of medications and permission to administer medications was observed. The children were observed on the playground running about, at the table with the caregiver counting items, in activity centers, completing toileting and diaper routines, and completing handwashing routines. A caring and nurturing environment was observed as the caregivers met the children’s needs and interacted with the children. During today’s visit, a partial assessment of applicable child care requirements was conducted. The following violation was cited and must be corrected immediately. Technical assistance on how to maintain compliance was provided. Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. The seat of the gray child-sized chair observed in space #B2S4 was sunk in, as the bottom of the chair was broken. G.S. 110-91(6); .0601(b) The following were discussed with you during today’s visit: • Technical assistance on how to maintain compliance with toys and equipment being in good repair. A well-maintained classroom environment supports the safety and well-being of children, allowing them to learn and play in a secure setting. Broken furniture can pose hazards such as sharp edges, tripping hazards, or choking hazards, which can be dangerous for children. Proper maintenance and repair of furniture helps reduce waste and promote sustainability by extending the life of toys and materials. Repairing furniture can save money in the long run by preventing the need for new purchases and maintaining existing assets. By addressing these issues and monitoring toys and equipment on a daily basis, classrooms will be a safer and more supportive environment for children. • Quality Rating and Improvement System (QRIS), is North Carolina’s new star rating system for early childhood education facilities. QRIS is a systemic approach to assess, improve, and communicate the level of quality in child care. It encourages programs to engage in continuous quality improvement and families may use the star rating to make informed decisions when choosing a child care facility. Today it was explained that you can review the new QRIS requirements, which includes three pathways to choose for the rated license process. Even though you are a one star center, you can take time to review this process to see if it is feasible for your facility. This facility currently operates with a Notice of Compliance that was issued on 5-23-23. The last sanitation inspection was completed on 5-17-24 with an “Superior” classification. The last approved fire inspection was conducted on 5-22-24. The NC Secretary of State website was reviewed today and this facility’s corporation’s name, North Haven Ministries, Inc. was listed as “Current-Active.” Today’s visit was conducted with you, Joyce Gupton, center director. I monitored the eight (8) classrooms in use for compliance with staff/child ratios, supervision, and staff interaction/discipline. The program was also monitored for health and safety requirements indoors using the annual compliance monitoring checklist. The children were observed running about, riding trikes, on the sew-saw, on riding toys, and interacting on the stationary play structure on the playground. The children completed daily activities with adequate supervision and engagement from the caregivers. The children were also observed at the table with manipulatives, in circle time where the caregiver was reading a book to the children, engaged in activity centers, and the children were observed completing toileting and handwashing routines. The program requirements, along with equipment and furnishings were observed. Compliance with staff records was documented by you. New staff records, along with children’s records were reviewed today. Medication observed on the premises were also monitored. This facility does not provide transportation. The following violations of child care requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance was provided. In building 2, space #3 permission to administer CeraVe ointment was dated 11-7-23 and in building #1, space #1 permission to administer Boudreaux Butt Paste was dated 2-5-2024. 10A NCAC 09 .0803(1)(a & b) 1053 On-going training received was not within the 9 topic areas listed in GS 110-91(11) and/or the health and safety training topic areas in rule .1102. Recognizing and Responding to Suspicions of Child Maltreatment training was not updated by one staff member as required, the training was last completed on 12-27-2017. GS 110-91(11); 10A NCAC 09.1102(b)(1-11) 1320 Children's records that include an application for enrollment, medical and immunization records, and permission to seek emergency medical care was not on file for each child. One child's file did not contain a medical report/physical examination. GS 110-91(1);.0302(d)(2); .0304(g) The above violations must be corrected immediately and a letter must be sent to me no later than 4-23-25 stating how the items were corrected and a plan to maintain compliance with the requirements. The following were discussed with you during today’s visit: Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Technical Assistance was provided with the following suggestions that may be beneficial: Medication Authorization: Any medication on the premises must be labeled with the child’s name or a prescription label, be accompanied by a permission slip, filled out completely with the parent/guardian’s signature and date. Set a reminder to review/update medications in each classroom. You must remember the “five rights” of medication use: the right child, the right medication, the right time, the right dose, and the right route—all of which are generally regarded as a standard for safe medication practices. You and the caregivers must know what medication the parent/guardian authorized to be administered and how the parent/guardian wants medication given. If documentation is required after the medication is administered, then documentation with the required information must be completed and kept on file for six months. Having clear, detailed procedures for obtaining administered medication protects children from mistakes that can affect their health. It was suggested to reiterate in your monthly newsletter to parents that medication must accompany a signed permission slip and it was suggested all staff complete training on administering medications. Child’s Medical Assessment: Pediatric physical exams from a physician are important because they help ensure a child's growth and development are on track. They give you and your child’s pediatrician the chance to track your child’s progress and prevent health issues through early detection. These routine check-ups can identify, prevent, and treat acute and chronic diseases, including allergies, asthma, and infections. Regular well-child visits help keep children healthy and safe. The pediatric physical examination which includes discussing vaccines, age-appropriate screenings, and addressing concerns should be documented and on file prior to enrollment or within thirty (30) days of enrollment. It is strongly suggested to have the parent to submit the physical exam on the DCDEE Child’s Medical Report form that contains the pediatrician’s signature prior to enrollment. Child Maltreatment Training: Completing the required Recognizing and Responding to Suspicions of Child Maltreatment training is important because it equips childcare providers with the knowledge to identify potential signs of abuse or neglect, understand their legal responsibility to report suspected cases, and ultimately creates a safe and nurturing environment for children in their care by proactively preventing maltreatment. It is suggested to have caregivers take this training during the first two (2) weeks of employment to comply with the orientation requirements regarding maltreatment. If a caregiver is hired with a Recognizing and Responding to Suspicions of Child Maltreatment training certificate the date of the training must have been completed within the past five (5) years. Recognizing and Responding to Suspicions of Child Maltreatment training can be accessed on the Positive Childhood Alliance NC website, https://preventchildabusenc.org/resource-hub/recognizing-responding-to-child-maltreatment/, it is free of charge and counts toward Health and Safety Training requirements to be completed every five (5) years. • Please complete the asbestos and lead in water testing via the Clean Classrooms for Carolina Kids website asap. There are results on the website for your lead in paint testing but not for lead in water and asbestos. • Please connect/link you and your staff asap. Center directors/owners can now access the Automated Background Check Management System (ABCMS), the DCDEE’s criminal background check system. This access will allow providers to: • See the real-time background check status of staff members. • Run a printable report of the staff roster to assist with compliance visits. • See new background check applicants and add to staff roster. Access to the ABCMS requires the successful completion of a Moodle course consisting of a short video followed by a test. The course is titled ABCMS Child Care Provider Portal Training - under the Early Childhood Professional Development Tab, then under the Criminal Background (CBC) tab. Ensure staff know their NCID Username and Password. It is strongly suggested to have the parent to submit the physical exam on the DCDEE Child’s Medical Report form that contains the pediatrician’s signature prior to enrollment. The last approved fire inspection was conducted on 5-22-24. The NC Secretary of State website was reviewed today and this facility’s corporation’s name, North Haven Ministries, Inc. was listed as “Current-Active.” Today’s visit was conducted with you, Joyce Gupton, center director. I monitored the eight (8) classrooms in use for compliance with staff/child ratios, supervision, and staff interaction/discipline. The program was also monitored for health and safety requirements indoors using the annual compliance monitoring checklist. The children were observed running about, riding trikes, on the sew-saw, on riding toys, and interacting on the stationary play structure on the playground. The children completed daily activities with adequate supervision and engagement from the caregivers. The children were also observed at the table with manipulatives, in circle time where the caregiver was reading a book to the children, engaged in activity centers, and the children were observed completing toileting and handwashing routines. The program requirements, along with equipment and furnishings were observed. Compliance with staff records was documented by you. This facility currently operates with a Notice of Compliance issued on 5-23-23. The last annual compliance visit was conducted on 5 -21-24. Today’s visit was conducted with you, Joyce Gupton, center director. Upon arrival to the facility, I was greeted by you. A walk-through of the facility monitoring the eight (8) classrooms where children were in care was completed. Compliance with staff/child ratios, supervision, discipline, program requirements, storage of hazardous substances, CPR & First Aid, permit restrictions and various staff records documentation was monitored as a partial assessment of applicable child care requirements. A sampling of medications and permission to administer medications was observed. The children were observed on the playground, in activity centers, completing handwashing and toileting routines, at the table eating lunch, and resting on mats/cots with linens. A caring and nurturing environment was observed as the caregivers met the children’s needs and interacted with the children. During today’s visit, a partial assessment of applicable child care requirements was conducted. The following violation was cited and must be corrected immediately. Children are able to play freely with peers, expand their imagination beyond restraints of indoor activities, release energy and explore their sense of touch, smell, taste and their sense of motion. Since children are exploring it is very important to provide a safe environment, free of debris. A designated staff member should verify daily that the area is safe for children to use.
