The facility received a GS-110 Notice of Compliance issued January 2000. The church facility operates at 25 sq. ft. inside and rations in rule .0713 (a) with a capacity of 49 for children ages 2-5 years old. My visit was conducted with you, Peggy Tomlinson, Director & Lead Teacher. The corporate contact is Bishop Marchall Brunson. Both CBC’s are in compliance. Four (4) preschool children were observed getting up from a nap and eating a snack. You stated you do not transport the children, and you did not have any medications at the facility. I observed specialized trainings, CBC’s, posted documents, supervision, child/staff ratios, discipline, safety issues, etc. ******Proof of Compliance/Reminders & Technical Assistance: ****** *The last fire drill, SIP/lockdown drill was on 9-15-25. *The last Sanitation inspection was on 10-31-24. Since your inspection is due by 10-31-25, I encouraged you to contact Naterra McQueen, environmental health specialist at 919-796-7988 to request an inspection. Remember to scan or forward all inspections to me. *The last water lead test was on 10-30-24 & will be due again on 10-30-27. *The EPR plan was updated on 9-24-25. Peggy completed EPR training on 9-22-18. *I reviewed the NC Secretary of State’s website and observed that the owner of this facility, Morning Star World Harvest Ministries, Inc. was listed as current-active. The following violations were observed and must be corrected immediately; email a corrective action letter with a copy of the CPR/1st Aid Cards and your 2025 fire inspection by 10-21-25 to Michelle.Ezzell@dhhs.nc.gov Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The center's Fire Inspection expired on August 16, 2025. 10A NCAC 09 .0304(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. During today’s visit, you showed me an email request you submitted this morning on 10-7-25 to Deputy Joe Rice requesting a fire inspection. A new inspection must be completed and sent to me 10-21-25. Remember all fire inspections must be sent to me within 5 days of completion. *ABCMS LINKING CRIMINAL BACKGROUND CHECKS-Staff need to log into the Criminal Background Check Portal on the DCDEE website and link to the facility license number. It is important to do this so staff at DCDEE can contact the center if a random criminal history check uncovers a staff member with a criminal charge or conviction working in a childcare facility. It is also a way to for staff at the CBC Unit to keep you updated about the status of out-of-state checks if a new employee has a Provisional Qualification. Instructions on how to link to the facility was provided today and detailed instructions emailed to you on 10-11-24 and 2-4-25 and emails have been sent to you by DCDEE. In the future, new staff need to link within five days of hire or transfer to your center. Review Criminal History Record Check Requirements for Child Care Centers in the NC Child Care Requirements 10A NCAC 09 .2703. *A step-by-step handout was sent to you by email, during my visit on 2-12-25 & today. Also, A Director’s meeting was offered in the Spring of 2025 going over this requirement. *CPR/1st AID: You stated you have an approved CPR/1st Aid trainer. CPR/1st Aid training must be face-to-face only, no on-line or blended courses. It must be renewed every two years before the expiration date on the certification card. For safety purposes, at least one person with current CPR/First Aid must be on-site at all times. ******************REMINDERS ************************* *Pastor Bishops CBC will expire 2-11-26. CBC’s can be completed up to 6 months prior to the expiration date. *CRIMINAL BACKGROUND CHECKS (CBC) are completed to protect the children from abuse and reduce liability risks. The CBC process must be completed before a person can begin work; therefore, a Qualification Letter must be on file prior to employment. Any employee in a licensed childcare facility who is ever left alone with a child or group of children for any length of time must have a background check completed through the Division of Child Development and Early Education (DCDEE). A CBC must be completed every five years. An expiration date is listed on each Qualification Letter. Submissions can be made six months before the expiration date. Operators will need to be proactive and make staffing decisions and arrangements in advance to avoid non-compliance with this and other DCDEE staffing requirements. Having applicants submit a copy of their Qualification Letter along with their resume and application can facilitate a shorter selection and hiring process. Criminal background checks must be completed online. To complete online criminal background check applications, go to www.ncchildcare.nc.gov/general/dhhscrc.asp. The $26.50 payment is a part of the online criminal background check application process. Fingerprints must be completed through LIVESCAN at your local county sheriff’s office. If you have any questions regarding the criminal background check process, dial 1-800-859-0829 and ask for the criminal background unit. *****OUTDOOR PLAY STRUCTURE: ***** *During my last visit we discussed removing the large play structure on the playground since adding mulch has been an issue. Upon arrival, I noticed the play structure had been removed and one (1) small metal airplane was on the playground. We discussed when children play outside, remember to provide toys such as bats, balls, hula-hoops, trucks, sand, sand toys, baby dolls, books, etc. We discussed adding a shade structure for shade. It can be an umbrella or a permanent shade structure. ******ADDING AGE ONE (1) TO FIVE (5) vs. TWO (2) to FIVE (5) on the Notice of Compliance**** *The facility has four rooms that are licensed for childcare. Space one (1) is used for a conference room, space two (2) is used for childcare, space three (3) is the eating area/lunchroom, and space four (4) will be used for children in diapers/toddlers. *Space one (1) and four (4) has a direct exit and children under 2 ½ years old must be cared for in rooms with a direct exit. This is indicated as a restriction on your posted Notice of Compliance, located in the hallway at the entrance of the facility. ***********************SPACE FOUR (4) **************************************** *During today’s visit, you gave me a report from Naterra McQueen, Environmental Health Consultant dated 3-25-25, approving space four (4) for children ages one (1) and up. This room is in compliance and can now be used by children (once you receive your new notice of compliance in the mail). *I observed a changing table, two (2) sinks, handwashing posters, 1st aid documents, large mats, staff/ratios posted, etc. and a variety of age-appropriate toys (variety of blocks, caterpillar climbing structure, variety of musical toys, and trucks/cars) have been added for toddlers. *This space has a direct exit leading to the parking lot and an exit leading to the hallway. *If locks are required, appropriate locks are lock & key, combination lock, keypad, or magnetic lock. The inspection from Mr. McQueen indicated in the comment section that the classroom is approved for six (6) children. Ms. Peggy Tomlinson indicated she told Ms. McQueen that her goal was to have one (1) caregiver and six (6) toddlers. This is why Ms. McQueen documented this on the inspection. I tried contacting Ms. McQueen, and she has not contacted me back. Space measurements at 25 sq. ft allows ten (10) total children in this space. *Reminders: Plastic bags cannot be accessible to children 3 and under, infant/toddler feeding schedules must be posted (forms are on DCDEE’s website under Provider Documents), sippy cups/bottles must be dated and labeled with the child’s name, each child must have a cubby. I suggested adding another shelf for teacher storage and adding a table/shelf with a dorm size refrigerator to keep water/juice, etc. The refrigerator must have a thermometer with a temperature of at least 45 degrees or colder. *****GROUPING CHILDREN*** We discussed the center must maintain staff/child ratios in rule .0713 and children 3 years of age cannot be grouped with one-year old children unless it’s at the beginning or end of the day (during drop off and pick up) for a short amount of time. Refer to rule .0713 and your posted staff/child ratio sheet for additional information. If you have at least one (1), one year old child present, your ratios will be (1:6) one (1) person to six (6) children, no matter what the ages of the remaining children. Currently your ratios are 1:10. We discussed this on 2/12/24 and by email on 8-28-24. Space one (1) is used for a conference room, space two (2) is used for childcare, space three (3) is the eating area/lunchroom, and space four (4) will be used for children in diapers/toddlers. *Space one (1) and four (4) has a direct exit and children under 2 ½ years old must be cared for in rooms with a direct exit. This is indicated as a restriction on your posted Notice of Compliance, located in the hallway at the entrance of the facility. ***********************SPACE FOUR (4) **************************************** *During today’s visit, you gave me a report from Naterra McQueen, Environmental Health Consultant dated 3-25-25, approving space four (4) for children ages one (1) and up. This room is in compliance and can now be used by children (once you receive your new notice of compliance in the mail). *I observed a changing table, two (2) sinks, handwashing posters, 1st aid documents, large mats, staff/ratios posted, etc. and a variety of age-appropriate toys (variety of blocks, caterpillar climbing structure, variety of musical toys, and trucks/cars) have been added for toddlers. *This space has a direct exit leading to the parking lot and an exit leading to the hallway. *If locks are required, appropriate locks are lock & key, combination lock, keypad, or magnetic lock. The inspection from Mr. McQueen indicated in the comment section that the classroom is approved for six (6) children. Ms. Peggy Tomlinson indicated she told Ms. McQueen that her goal was to have one (1) caregiver and six (6) toddlers. This is why Ms. McQueen documented this on the inspection. I tried contacting Ms. McQueen, and she has not contacted me back. Space measurements at 25 sq. ft allows ten (10) total children in this space. *Reminders: Plastic bags cannot be accessible to children 3 and under, infant/toddler feeding schedules must be posted (forms are on DCDEE’s website under Provider Documents), sippy cups/bottles must be dated and labeled with the child’s name, each child must have a cubby. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The facilities fire inspection expired April 27, 2023. 10A NCAC 09 .0304(a) 1311 Emergency medical care information was not on file in the center on the child's first day of attendance and/or was not updated as changes occurred or at least annually for each child. Three children's applications which contain his/her emergency information has not been updated since March and April 2023. .0802(c) 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. One child did not have the bottom part of his/her medical form completed by a doctor or health care professional within 30 days. GS110-91(1) 1322 A written statement from each child's parent giving standing permission which may be valid for up to twelve months for participation in off premise activities that occur on a regular basis was not available. Three out of five children did not have an updated permission to play outside the fenced in area document on file. .1005(b)(4) 1899 Health and safety training topics were not included as part of on-going training within five years of completing the previous health and safety training topics. The Providers Health and Safety trainings expired April 2024. .1103(b) **************Reminders/We Discussed****************** *Children’s emergency information located on the bottom of their application must be updated when changes occur and at least annually. *Children’s permission to play outside the fenced in area must be updated annually. *Children’s medical record must be completed within 30 days of the child’s enrollment. The bottom part of the form must be completed by a health professional. *Your Health and Safety trainings expired April 2024. Technical assistance was given during your last visit. Scan proof of your certificate(s) to me on or by 8-20-24. *Fire inspections must be completed annually and submitted to your consultant within 5 days of completion. *I suggest creating a reminder for you on your computer or desk calendar annually so inspections, training’s, etc. will not be overlooked. *Both of your CPR/1st Aid training will expire in 2025. *The water test for lead was completed on 6-30-21 & expired 6-30-24. This must be completed immediately through Water for Carolina Kids. *Remember to continue to update your EPR annually and print out the cover page with the date for verification.
