This facility currently operates with a One (1) Star License that was issued on 1-28-16. The last annual compliance visit was conducted on 8-22-24. Today’s visit was conducted with you, Melinda Sanders, owner/provider. Upon arrival to the residence, I was greeted by you. You explained that there were no children present due to summer vacations. The residence was monitored to ensure compliance with safe indoor environment requirements. The required postings, along with appropriate equipment and furnishings were observed. Emergency drills, outdoor inspections, the activity plan/schedule, daily attendance, and all three (3) children’s files were also observed. Documentation of program records, as well as the operator verification information was observed and has been documented on the attached form. You stated that currently you are not transporting children but would start back transportation once school starts. Your vehicle was monitored for transportation requirements. You added that currently there are no children with medication to administer. During today’s visit, an assessment of applicable child care requirements was conducted. The following violations of child care requirements were cited and must be corrected immediately. Technical assistance on how to maintain compliance was provided today. Violation Number Comment Rule 1853 The operator did not conduct a monthly fire drill. Documentation was not available that a fire drill was conducted in the month of May 2025. .1719(a)(15) & .1721( e)(2) 1875 The EPR Plan did not include the location of the Ready to Go File and/or required information. A Ready to Go File was not available during today's visit. .1714(d)(10) The above violations must be corrected immediately and a letter must be sent to me no later than 8-20-25 stating how the items were corrected and a plan to maintain compliance with the requirements. The following were discussed with you during today’s visit: • Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. • Technical Assistance was provided with the following suggestions that may be beneficial: Monthly Fire Drills: Conducting monthly fire drills help reduce the potential for panic and confusion during an actual fire, as you and the children are familiar with the procedures. By practicing various scenarios, both you and children become better prepared to respond effectively during an emergency. These drills confirm the reliability of the emergency plan and identify areas for improvement, which are essential for maintaining a safe environment. Overall, monthly fire drills are a vital component of emergency preparedness in daycare settings. It is strongly suggested to mark your calendar monthly and conduct your fire drill and playground inspections on the same day. It is important to immediately document the fire drill on the required emergency drill log in order to ensure you do not forget to log that you conducted the fire drill and playground inspections as required. Ready to Go File: The Ready to Go File requirement is crucial because it acts as a readily accessible collection of critical information needed to quickly respond to an emergency situation, allowing you to efficiently evacuate children, contact parents, and address specific needs during a crisis, ensuring the safety and well-being of all individuals in the facility. You need to compile a Ready to Go File notebook with all of the required paperwork. A Ready to Go File checklist was provided today for you to ensure required paperwork is added to your notebook/folder. At least monthly, your Ready to Go File should be monitored to ensure that it is up to date. The file should be updated when new children enroll or disenroll, and/or when new medication is provided for a child or when the medication is no longer needed, and/or when a child has an allergy, the allergy list needs to be modified. It is suggested to mark your calendar or set an alarm to remind you at least monthly to update your EPR Plan/Ready to Go File. • Today you explained that you had updated your Emergency Preparedness and Response Plan (EPR) but could not access your Emergency Preparedness and Response Plan on the Risk Management website. I tried to assist you and we contacted the NCID Help Desk. The NCID Help Desk suggested that you contact the NC Health and Safety Resource Center for assistance. Please update me when you have accessed your EPR Plan. Remember to print the updated EPR Plan and place it in your Ready to Go File. A suggestion was also made to save the PDF version of the EPR Plan to your desk top or on your computer to ensure access. • You have completed the ABCMS Moodle training now ASAP connect/link you and your husband. FCCH owners can now access the Automated Background Check Management System (ABCMS), the DCDEE’s criminal background check system. This access will allow providers to: • See the real-time background check status of staff members. • Run a printable report of the staff roster to assist with compliance visits. • See new background check applicants and add to staff roster. Instructions were emailed to you today on how to connect/link staff in the ABCMS portal. This facility currently operates with a One (1) Star License issued on 1-28-16. The last annual compliance visit was conducted on 8-22-24. Today’s visit was conducted with you, Melinda Sanders, owner/operator. Upon arrival to the residence, I was greeted by you. I did a walk-through of the home with you, visiting the room where child care occurs and the kitchen that is occupied by children. Compliance with capacity, supervision, discipline, attendance, program requirements, storage of hazardous substances, CPR & First Aid, permit restrictions, the required postings, and your staff record documentation was monitored as a partial assessment of applicable child care requirements. Medications were observed on the premises. During the visit, the children were observed on the front porch where they were playing with age appropriate toys. A partial assessment of child care requirements was monitored during today’s visit. The following violation of child care requirements was cited and must be corrected immediately. Violation Number Comment Rule 903 When medication is administered, authorization for the operator to administer the specific medication according to the parent's or health care professional's instructions was not on file and available. There was no written permission to administer Parent's Choice Diaper Ointment and Aquaphor for two (2) children. 10A NCAC 09 .1720(c)(1&2) & .1721(a)(4) The following were discussed with you during today’s visit: • Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any medication in the residence must be labeled with the child’s name, be accompanied by a current permission slip, filled out completely with the parent/guardian’s signature and date. Set a reminder to review/update medications. You must remember the “five rights” of medication use: the right child, the right medication, the right time, the right dose, and the right route—all of which are generally regarded as a standard for safe medication practices. You must know what medication the parent/guardian authorized to be administered and how the parent/guardian wants medication given. If documentation is required after the medication is administered, then documentation with the required information must be completed and kept the documentation on file for six months. Having clear, detailed procedures for obtaining administered medication protects children from mistakes that can affect their health. It was suggested to reiterate to parents that medication must accompany a permission slip, and it was suggested that you complete training on administering medications. This accordion folder contains other folders with the required paperwork needed for licensing. You explained that this black accordion folder is placed in the vehicle when children are transported. If there is an emergency, you want your red folder with the children’s emergency information easily visible. You can keep the accordion folder with the other information at the residence. As well, an even better solution would be to just make a copy of each child’s application and attach their photos to the application which contains their emergency information. Add your emergency information also in case you are unconscious. This paperwork can be kept in a folder that is visible in the front seat of your vehicle and can be labeled, “Emergency Information.” • A suggestion was made to save the most recent child care requirements in Chapter 9 effective November 1, 2024, to your desktop computer for easy access. As well as, bookmark the Division of Child Development and Early Education on your computer for access to valuable resources like, the criminal background check link, forms and documents, what’s new, etc. • You explained that you were having difficulty logging into Moodle to complete the ABCMS training. Call the NCID Help Desk at 800-722-3946 to assist you with logging in to Moodle. • Please connect/link you and your husband in the criminal record check portal asap. Family child care home providers can now access the Automated Background Check Management System (ABCMS), the DCDEE’s criminal background check system. This facility currently operates with a One (1) Star License that was issued on 1-28-16. The last annual compliance visit was conducted on 9-12-23. Today’s visit was conducted with you, Melinda Sanders, owner/provider. Upon arrival to the residence, I was greeted by you. The children were observed eating at the table. Once the children finished eating, they engaged in toileting and handwashing routines. In the room used for child care (beside the kitchen), the children were observed having free play with a variety of toys. The children also went outside in the fenced backyard for gross motor play. The residence was monitored to ensure compliance with safe indoor environment requirements and the fenced back yard where the children engage in gross motor play was observed to ensure compliance with safe outdoor environment requirements. The required postings, along with appropriate equipment and furnishings were observed. Emergency drills, outdoor inspections, the activity plan/schedule, daily attendance, and two children’s files were also observed and in compliance. Documentation of program records, as well as the operator verification information was observed and has been documented on the attached form. Violation Number Comment Rule 726 Children were sleeping or napping in a room with a closed door between the child and the supervising staff. The door (with a large plexiglass opening) leading from the kitchen to the room used for child care was closed, as a toddler was observed sleeping in the room, in a pack-n-play. .1711(a)(4)(D) The following were discussed with you today: • Supervision was discussed in detail, along with examples of inadequate supervision. The importance of adequately supervising children at all times, even when children are sleeping by leaving door open was discussed. Adequate supervision shall mean that staff interact with the children while moving about the indoor or outdoor areas and are able to always hear and see the children, except when emergencies necessitate that direct supervision is impossible for brief periods of time. • We discussed that play is an important part of a child’s development. Playing outside in the fresh air can be fun and adventurous. With careful planning and monitoring, play environments can be safe for children. There have been numerous reports of snakes on the playground in outdoor environments. Remember child care rule 10A NCAC 09 .0604 reads that once a day, prior to initial use, the indoor and outdoor premises shall be checked for debris, vandalism, and broken equipment, the temperature of outdoor equipment, in addition look for other hazards and/or snakes. Handouts with information regarding burn safety and protecting children from thermal burns were provided today. • A lot of time was spent with you looking for your required paperwork. A suggestion was made to purchase a 3-inch 3-ring binder and protective sleeves. Use the protective sleeves to store all of the required information that must be made available to your child care consultant, like criminal background check letters, fire drills, outdoor inspections, incident log, children’s records, training certificates, etc. When your consultant comes to visit, you can hand them your binder instead of looking through piles of papers and folders to find information/documentation. • A suggestion was also made to save the most recent child care requirements in Chapter 9 effective January 2024, to your desktop computer for easy access. As well as, bookmark the Division of Child Development and Early Education on your computer for access to valuable resources like, the criminal background check link, forms and documents, what’s new, etc. • Your children’s files were discussed to suggest that required paperwork could be grouped together for ease of review due to there being a lot of papers in each child’s file. • Your refrigerator thermometers observed in both refrigerators need to be replaced. The red line observed was broken and an accurate reading is not possible. • Please connect/link yourself and your husband in the criminal record check portal asap.
