A full assessment of child care requirements was conducted. My visit today was conducted with you, Sheila Fowler, Owner. There were six children present. Children were eating breakfast, playing in activity areas, completing routines independently, and preparing to go outside. You assisted children who required additional support and helped children with interactions. The facility was monitored for health and safety requirements. One violation related to health was cited. Equipment and furnishings were also observed. Operator, children, and program records were monitored. Violations related to children’s program records were cited. A checklist was used to monitor compliance. Transportation requirements were monitored, and one violation was cited. The last annual compliance visit was conducted on 9/12/2024. Organization and completeness of records is important to ensure easy access to information in emergency situations. You explained you will have the parents complete the missing items. Transportation - Authorization is important to transport children in child care to ensure their safety and providing parents with confidence that their children are in safe hands. Without proper authorization and oversight, there is a high risk of injury, liability, and potential harm to children during transportation. You may review 10A NCAC 09 .1723 TRANSPORTATION REQUIREMENTS. Parents may provide a written statement giving standing permission which may be valid for up to 12 months for participation in off premise activities that occur on a regular basis. Each time the children are taken off premises, you must take identifying information about each child including the child's name, photograph, emergency contact information, and a copy of the emergency medical care information form. We discussed the difference between routine transportation and field trips. You may find transportation authorization forms on the DCDEE provider documents portal. LEAD IN WATER, LEAD IN PAINT, AND ASBESTOS: Today I incorrectly documented a violation related to lead in water. You have completed the lead in water testing with Wake County Environmental health. Make sure the testing is completed for each water faucet your children drink from, and you cook from. Also, I observed you started the enrollment for the lead in paint and asbestos. You may need to reach out to Clean Classrooms RTI to see if you need to complete any additional steps. HOW TO LINK QUALIFYING LETTERS IN THE ABCMS SYSTEM: Today we discussed you need to link you to your facility in the ABCMS. I reviewed the portal and observed you have not completed this process. You explained you did not think you needed to do anything after having a conversation with Support Staff. This is a child care requirement that was effective in 2024. Access to the ABCMS requires the successful completion of a Moodle course consisting of a short video followed by a test. The course is titled ABCMS Child Care Provider Portal Training - under the Early Childhood Professional Development Tab, then under the Criminal Background (CBC) tab. My visit was conducted with you, Shelia Fowler, Owner. This facility currently operates with a Five-Star Child-Care License. The last annual compliance visit was conducted on 9/12/2024. Two children were present playing in the child care space. Children interacted with each other, played with chalk and moved freely around the child care space. You talked with children about their interactions, activities, and helped children write on a chalk board with chalk. You also talked with children about the activity completed related to the upcoming holiday. Supervision, capacity, and permit restrictions complied. The license was posted. Storage of hazardous products complied. General safety requirements, and general health met compliance. Program records, fire safety, and sanitation were monitored, and four violations were cited. Yours and one other household members’ qualifying letters are current. Violation Number Comment Rule 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. The operator did not have a record of monthly check for hazards outdoors for January, February, and March of 2025. 10A NCAC .1721(e)(5)(A-F) 905 Record of monthly fire drills, giving the date, time of day and length of time to evacuate, and operator's signature, were not available. This ensures each child is accounted for during the day and helps maintain compliance with attendance requirements. Emergency Drills - Children learn how to respond in emergencies when drills are practiced often. Fire drills are required to be conducted monthly. Practice fire drills at different times of the day so the children are always ready. Document dates that you conduct fire drills each month and have someone available for backup in case the person that usually conducts the fire drills isn’t present. An emergency drill log is available on the DCDEE website. Fire drills teach children what to do in case of a fire. If fire drills are not practiced monthly, it will affect the children’s safety if there is a fire. Outdoor Inspections- The outdoor learning environment offers a sense of freedom for children. Children are able to play freely with peers, expand their imagination beyond restraints of indoor activities, release energy and explore their sense of touch, smell, taste and their sense of motion. Since children are exploring it is very important to provide a safe environment, free of debris. Each month, the outdoor play areas should be inspected and documented on the form provided by the Division of Child Development and Early Education. CONSULTATION/REMINDERS: Lead Free Classrooms: Based on information in the Clean Classrooms for Carolina Kids site, it appears your water testing is in progress. A full assessment of child care requirements was conducted. My visit today was conducted with you, Shelia Fowler, Operator, arrived later during the visit. You explained your older granddaughter was present visiting with you but does not reside in the home. Your program currently operates with a Four-star license. The facility was monitored for health and safety requirements both indoors and outdoors. Equipment and furnishings were also observed. Operator and children’s records were monitored. One violation regarding children records was documented. Program records were monitored. A checklist was used to monitor compliance. Today I monitored one passenger van used for transportation. You explained you have not been transporting children but want to continue the approval. The last annual compliance visit was conducted on 10/5/2023. Two children were present. One child was resting while another child was moving about the space and eventually transitioned to rest time. You provided supervision, completed sleep chart, and assisted one child with transitioning to rest time. The following violation was documented: Violation Number Comment Rule 1805 Copy of the safe sleep policy was not given and explained to the parent/guardian of each infant on or before the first day the infant attended the home and a signed statement from each parent/guardian acknowledging the receipt and explanation of the policy was not on file. Signed statements by the parent/guardian acknowledging the receipt and explanation of the safe sleep policy was not on file for two children. 10A NCAC 09 .1724(c) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before 9/26/2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. This facility currently operates with a Four (4) Star License that was issued on 10-6-22. The last annual compliance visit was conducted on 10-6-22. Today’s visit was conducted with you, Shelia Fowler, owner/operator. The space used for children, (the lower level which can only be accessed from the side exterior door of the home) was monitored for compliance with staff/child ratios, supervision, and staff interaction/discipline. The children were served muffins and milk and afterwards interacted in dramatic play before you transported them to school. Your program was also monitored for compliance with implementing an approved curriculum as required for all Four and Five Star licensed facilities. The residence was monitored for health and safety requirements both indoors and outdoors. The program requirements, along with equipment and furnishings were also observed. The Certificate of Compliance for the Community Playthings crib was observed, however is not being used at the time. Compliance with your staff records, along with children’s records were reviewed today. This facility is approved for transportation. Your 2002 Ecoline Van was observed and meets transportation requirements. At this time, no children are being administered medication and no medication was observed. During today’s visit, an assessment of applicable child care requirements was conducted. The following violations of child care requirements were cited and must be corrected immediately. Technical assistance was provided to maintain compliance. Violation Number Comment Rule 702 Potentially hazardous items, including but not limited to, power tools, nails, chemicals, propane stoves, lawn mowers, and gasoline or kerosene, whether or not intended for use by children were not stored in locked areas, removed from the premises, or otherwise inaccessible to children. Located in the van was a bottle of Microban Cleaner, an aerosol can of Microban Sanitizing Spray, and Neosporin ointment (in the first aid kit). In the room where child care occurs, there was an aerosol can of Clear Acrylic Sealer in a basket. .1719 (a) (1) 810 Refrigerator was not maintained at a temperature of 45 degrees Fahrenheit or below and/or was not monitored by a refrigerator thermometer. A refrigerator thermometer was not located in the refrigerator to monitor the temperature. 10A NCAC 09 .1725(a)(10) 911 Copy of each child's immunization record, including the operator's own preschool child(ren), was not on file. The two children enrolled did not have immunization records on file. GS 110-91(1); .1721(a)(2) The above violations must be corrected immediately and a corrective action letter must be sent to me no later than 10-19-23 stating how the violations were corrected and a plan to maintain compliance with the requirements. You stated today that you had already paid your registration fee online and are awaiting the registration paperwork to come through in the mail. • Please retest your well water since your test results indicate that when the Total Coliform is present it is recommended that the well water be re-tested to verify that the result is accurate. • Please look over the children’s file paperwork when the parent returns it to ensure that parents have completed all of the required paperwork and signed the paperwork where needed. • There are a lot of fruit flies in the kitchen and child care area, please take action to get rid of the fruit flies. • Clean out your binder with the outdoor inspections, you only have to keep the past 12 months and you have outdoor inspections from 2011. • Look out for your invoice for the annual licensing fee that will be mailed soon. Please pay the licensing fee upon receipt of the invoice. • The NC Child Care Health and Safety Resource Center is a valuable resource where you can find skills-based training and technical assistance from your Child Care Health Consultant on specific medications or health conditions, as well as the new poster on Stand-Up Changing Procedures. Sign up for their quarterly newsletter and find updated resources at www.healthychildcare.unc.edu or 800-367-2229, choose option 1, then 2. • It was suggested to keep your incident report forms and incident log in a folder/binder that is easily accessible. The incident reports for all incidents should be maintained in each child’s file. • The Healthy Social Behaviors (HSB) Project supports teachers to promote healthy social-emotional development and reduce the expulsion rate among young children in licensed child care centers across North Carolina. Specialists are available with early childhood education backgrounds who are passionate about empowering teachers to develop learning environments and teaching practices that promote prosocial skills in young children. A flyer with the challenging behaviors hotline information was provided today. • You mentioned that you have not received the past couple of months of Stabilization Grant funds.
