Your last annual visit was on 8-21-24. The FCCH operates with a 4-STAR license. A checklist was used to note the requirements I monitored. My visit was conducted with you, Evelyn Dunn, owner/operator. Shawnice Dunn, your daughter, was present during the visit. Upon arrival, I was greeted by you. The children were observed eating lunch, washing their hands and preparing for a nap. The primary care area is a room on the right down the hallway. Age-appropriate toys and posted documents were observed in this room. The children take naps and eat lunch in the family room at the front entrance of the home. The children have access to the bathroom in the hallway and the kitchen. You stated due to issues with the fence in the backyard, the children play in the front yard. There are two pets in the home. I monitored five (5) children’s files, all program and operator files. I monitored posted documents, equipment, furnishings, capacity, supervision, discipline and safety requirements. You are not approved for transportation; these requirements were not monitored. You stated currently there are no children with medication to administer. **VERIFICATION OF RECORDS: *EPR Training completed on=9-22-16 and your EPR plan was updated on 5-28-24. *Your last outdoor safety inspection was on 2-17-25. *Technical Assistance was offered on how to correct the violation cited and how to maintain compliance. To maintain compliance with NC laws and rules regarding childcare operation, the following violations must be corrected immediately. Email a copy of Shawnice’s qualifying letter BY 3-18-25 to michelle.ezzell@dhhs.nc.gov If a qualifying letter is not on file within 15 days, then on the 16th day, she cannot be present in the home. Violation Number Comment Rule 1846 An individual over the age of 15, who resides in a family child care home or a center located in a residence on a permanent or temporary basis, or those who had their 16th birthday after the initial licensing of a family child care home; did not complete and submit required forms within five business days. The Provider's adult daughter (SD) moved into the home and did not have a criminal record qualifying letter. A qualifying letter must be on file within 15 days and submitted to me. If not, she (SD) cannot be on the premises on the 16th day. G.S. 110-90.2 & .2703(q) *****TECHNICAL ASSISTANCE/REMINDERS: ***** *Anyone that moves into your home that’s 16 years and older must have a current CB qualifying letter on file with DCDEE. A copy of the letter must be on file in your home. *We discussed proof of your asbestos and lead water test must be on file in your home and completed every three years. Documentation must be on file for lead paint, water test and asbestos hazards in your facility. Go to www.cleanwaterforus kids.org/en/Carolina/ to enroll and/or complete the survey. Proof of these tests must be on file in your home and a lead water test must be completed every three years. For questions call 1-888-997-9290. This was documented in your visit summary dated 3-28-24 and 8-21-24. *Remember to continue to update your Emergency Preparedness and Response (EPR) Plan every year and print out the cover page showing the current date. Also, print the pages you made changes to. Failure to log onto the website may result in losing your entire EPR plan. To contact Early Years, call 919-403-6950 or go to https:www.earlyyearsnc.org ******** DCDEE NEWS & UPDATES********* *FCCH rules and updates are on DCDEE’s website at www.ncchildcare.nc.gov. *The only approved locks are combination locks, lock and key, electronic or magnetic locks. Rule .1719 (a) (2) *ABCMS-Criminal Background Portal UPDATE: As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. Numerous emails and newsletters have been sent to you in 2024-2025. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your FCCH roster to ensure current staff & household members are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members/household members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the Family Child Care Home within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted.
