A staff member greeted me upon arrival. Shawonne Pickett and I completed a walk-through of the facility. At the time of visit, there were a total of 20 children present. The children were observed and playing with materials in centers and completing routine care. LICENSE STATUS Currently this center operates with four star license issued August 13, 2019. This center is in cohort 1 (two). The preparation year started July 1, 2023 and the assessment year will start July 1, 2024. I reviewed the NC Secretary of State's website and observed that the owner is listed as current/active. INSPECTIONS I reviewed all required inspections including fire drills, emergency drills, sanitation, fire and outdoor inspections. Inspections were found to be in compliance. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. I reviewed all staff and a selection of children files. All spaces were monitored for supervision, staff/child ratios, materials, equipment, pet health assessment and required postings. This center has goats and chickens on site but in a separate area away from the playgrounds. A checklist was used to note the requirements I monitored today. I left a copy of the checklist at the end of the visit. ANNUAL IN-SERVICE TRAININGS This center’s annual in-service calendar dates are March 1st-February 29th. The following violation was observed and documented: Violation Number Comment Rule 847 Parent's medication authorization did not include required information. **One child’s permission to administer medication was missing and one was incomplete. 10A NCAC 09 .0803(4)(6-9) Please mail or email your letter of compliance by March 26, 2024. The letter must address each violations and describe how the violations were corrected. Any violations documented during visits to your program may have an impact on the total compliance history score and cause your score to drop below the mandated level. **Prior to today’s visit, the compliance history score for the center was 97%. Any violations cited during today's visit may affect this percentage. TECHNICAL ASSISTANCE **One child’s permission to administer medication was missing and one was incomplete. We discussed permission to administer medication. Please have the parents complete the permission form. **Benadryl and Tylenol have to be locked up even if it is part of the medical action plan. REMINDERS CLEAN WATER/WATER TESTING Rule, 10A NCAC 41C .1001 through .1007 and 15A NCAC 18A .2816 require all child care facilities (homes and centers) to test for lead in drinking, water, identify lead-based paint hazard, and mitigate or restrict access to identified hazards. Please check your lead water testing date. Per sanitation this must be completed every three (3) years.
