You, Krystan Remaley, Center Director accompanied me on a walk-through of the center. Allegation regarding the cleanliness of the facility – Upon entering the child care facility, I observed the entrance area and hallways were freshly mopped. A wet floor caution sign was in the middle of the floor. The floors in the bathroom were freshly mopped and were drying. I observed a staff member in the classroom for school-aged children wiping down the tops of shelves. In the room for children aged one, staff assisted children who were finishing eating lunch, with cleaning up the eating area and handwashing. In another classroom, a vacuum cleaner was recently used. Items throughout the classrooms were stored orderly on shelves, in containers, or other groupings. Based on my observation, this allegation could not be confirmed. This allegation is unsubstantiated. Allegation regarding drinking water available for children – Today I observed water available for children. The classroom for school-aged children had water bottles outside the classroom and the water container was being refilled. Other classrooms had large containers with water available for children. Water containers were stored on top of shelves or a table. Water for younger children is made available. Based on my observation, this allegation could not be confirmed. This allegation is unsubstantiated. Allegation regarding activity plan activities – Today the classroom for children aged two years old, posted activity plan was dated July 21st to July 25th. The staff member in the classroom for school-aged children stated she did not have the activity plan. I observed all other classrooms with current activity plans. There were children art work from past and current activity plans posted in the hallways throughout the child care facility. Artwork related to animals including green painted plates for snake making, fish activity, friendship mural, campfire activity, and bee artwork were displayed for children aged one. They also had books to support the current activity plan related to animals. Campfire art activity was still displayed from last week’s activity plan. There were pictures of younger children and infants participating in recent activities. Their displayed artwork was related to this week and past week’s activity plan and included ice cream painting, giraffe made from hand print, bugs made from footprints, and painted paper towel rolls to make a snake. Although evidence that activity plans were implemented in classrooms with current activity plans, there were two classrooms that did not have current activity plans available. Based on my observation and staff report, this allegation is substantiated. Staff/child ratio, supervision, group size and capacity complied. Children throughout the facility were finishing eating lunch and preparing for rest time. Staff assisted children with transitions and routine tasks. One infant who was falling asleep in the bouncy was being removed to be placed in a crib. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. In space two, the posted activity plan was dated July 21 to July 25. An activity plan was not available for children in space four. A full assessment of child care requirements was conducted. Your program currently operates with a Four-star license. The facility was monitored for approved curriculum. Today’s visit was conducted with you, Krystan Remaley, New Center Director and Ms. Hurma Gedeon, Human Resource Director. You visited each indoor and outdoor child care space with me. Staff/child ratios, supervision, capacity, group size, and staff interaction/discipline complied. Children throughout the facility were transitioning to rest time, finishing eating lunch, resting on their cots, and completing routine tasks at the time of my walk through. Infants were having floor play time, sleeping in the crib, playing with toys, learning to sit up, moving freely in the indoor space, and interacting with staff and each other. Staff assisted the children with transitions, routines, clean-up, and toileting. Staff talked with children and infants about their activities, helped children when needed, and met their developmental needs. Throughout the facility, children displayed artwork, teacher display, and materials were related to the current and recent lesson plans. The facility was monitored for health and safety requirements indoors. Program requirements, along with equipment and furnishings, were monitored. All inspections and drills were current. A sampling of children’s and existing staff records, and all new staff records were monitored and information documented on the children’s records form and the staff and training worksheet. One violation related to children’s records was documented. A checklist was also used to monitor compliance. The last annual compliance visit was conducted on 4/23/2024. The facility does not provide transportation. Violation Number Comment Rule 1311 Emergency medical care information was not on file in the center on the child's first day of attendance and/or was not updated as changes occurred or at least annually for each child. HOW TO LINK QUALIFYING LETTERS IN THE ABCMS SYSTEM: Please link you and your staff member(s) asap. Center directors can now access the Automated Background Check Management System (ABCMS), the DCDEE’s criminal background check system. This access will allow providers to: • See the real-time background check status of staff members. • Run a printable report of the staff roster to assist with compliance visits. • See new background check applicants and add to staff roster. Access to the ABCMS requires the successful completion of a Moodle course consisting of a short video followed by a test. The course is titled ABCMS Child Care Provider Portal Training - under the Early Childhood Professional Development Tab, then under the Criminal Background (CBC) tab. The last annual compliance visit was conducted on 4/23/24. You, Krystan Remaley, a Center Administrator, assisted me today. Your program currently operates with a Four-Star license. You accompanied me as I walked through the facility today. I observed in specific classrooms for an extended time. Throughout the child care I observed children’s art work related to current and past activity plans hanging in the classrooms, displayed on classroom walls, and on walls in the hallway. In one classroom, children made flags from another country as part of the activity plan. The facility had multicultural displays, displays about family, and door displays that reflected recent activities. Staff talked with me about the activities and how the children responded to the activities. Children throughout the facility were resting or having quiet time on their cots. Staff sat with children to assist them with rest time, walked around the room while providing supervision, and talked assisted children with quiet activities. In the room for infants, children were having floor play time, drinking from a bottle, and being fed. Some children were resting and when they woke, were removed from their crib, and placed on the floor for floor time. Violations related to documentation of monitoring sleeping infants and bottle feeding were cited. Supervision, group size, permit capacity, staff/child ratios and space capacity complied. Four new staff’s files were monitored. Other staff’s qualifying letter and special training complied. One violation related to first aid certifications was cited. Children’s records were not monitored. Program records complied. Fire and sanitation inspections were current. Emergency medical care plan was accessible. After further review of the staff certification, I observed the CPR did include the certification for the required ages of children served. The violation was not cited. TECHNICAL ASSISTANCE: Sleep Charts – Monitoring sleeping infants sleep position at least every fifteen minutes is critical in ensuring the safety of infants while they are sleeping. Today we discussed that staff stated they had just put down a child to rest in their crib and they may have documented the date wrong. The last date for that infant showing they were monitored for sleep position was listed as 11/18, which may have been the wrong date. Two other infants who were napping did not have documentation of being monitored today and documentation was last completed for them on 11/13. We discussed when documentation should occur. Infant Feeding – Holding an infant while being bottle fed when they are unable to feed themselves or learning to feed themselves helps ensure the child does not choke and offers adequate supervision if they need to be readjusted while feeding. It also offers a bonding experience between the infant and the caregiver. Today you explained you recently talked with staff about propping bottles for infants who were learning to hold their bottles. You also explained you talked with staff about that being a choking hazard and you thought the issue was resolved. Today I observed one infant propped on a pillow on the floor while trying to hold a bottle. You stated you will talk with the staff again. First Aid – Today we observed the first aid certification was for adult first aid and not pediatric first aid. The staff will need to complete pediatric first aid and receive an updated first aid certification card. Clean Classrooms for Carolina Kids: I reviewed the information for your facility and the summary notes you have completed the asbestos screening portion. Information indicates in-progress for the lead in water registration. It shows results for lead in water testing from 2023. You may need to reach out to find out if they require additional information from you. It also indicates the lead in paint has no results yet. CONSULTATION: ABCMS (new staff) - In G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility, within five business days of employment. The process of notifying the Division has changed and is now captured in Automated Background Check Management System (ABCMS). This change has been in effect since February 2024. The last annual compliance visit was conducted on 4/23/24. You, Krystan Remaley, a Center Administrator, assisted me today. Your program currently operates with a Four-Star license. You accompanied me as I walked through the facility today. I observed in specific classrooms for an extended time. Throughout the child care I observed children’s art work related to current and past activity plans hanging in the classrooms, displayed on classroom walls, and on walls in the hallway. In one classroom, children made flags from another country as part of the activity plan. The facility had multicultural displays, displays about family, and door displays that reflected recent activities. Staff talked with me about the activities and how the children responded to the activities. Children throughout the facility were resting or having quiet time on their cots. Staff sat with children to assist them with rest time, walked around the room while providing supervision, and talked assisted children with quiet activities. In the room for infants, children were having floor play time, drinking from a bottle, and being fed. Some children were resting and when they woke, were removed from their crib, and placed on the floor for floor time. Violations related to documentation of monitoring sleeping infants and bottle feeding were cited. Supervision, group size, permit capacity, staff/child ratios and space capacity complied. Four new staff’s files were monitored. Other staff’s qualifying letter and special training complied. One violation related to first aid certifications was cited. Children’s records were not monitored. Program records complied. Fire and sanitation inspections were current. Emergency medical care plan was accessible. ADDITIONAL REQUIREMENTS: General health requirements complied. Medications were stored properly. The facility continues to meet requirements for adequate and approved space. The NC Secretary of State website was reviewed and the Owner of this facility is listed as current- active. Violation Number Comment Rule 531 Bottles were propped. A full assessment of child care requirements was conducted. Your program currently operates with a Five-star license. The facility was monitored for approved curriculum. Today’s visit was conducted with you, Alice Danzie, Center Director. You visited each child care area with me. I monitored classrooms in use for compliance with staff/child ratios, supervision, capacity, group size and staff interaction/discipline. Infants were having floor play time, napping, manipulating various materials, and preparing to be fed. Children throughout the facility were transitioning from playing in activity areas, to preparing for lunch and rest time. I observed diapering procedures for one child. Staff assisted with activities and met the developmental needs of children. The facility was monitored for health and safety requirements. A checklist was also used to monitor compliance. Program requirements, along with equipment and furnishings complied. All inspections and drills were current. A sampling of children’s and staff’s records was monitored and information documented on the children’s records form and staff and training worksheet. Violations related to both children and staff records were documented. The last annual compliance visit was conducted 5/19/2023. You do not provide transportation. Today, the owner/operator is listed as current/active with the NC Secretary of State. All staff are required to complete the training within 90 days of hire. The training may be accessed via the DCDEE website at www.ncchildcare.nc.gov. Recognizing and Responding to Suspicions of Child Maltreatment is also available on Prevent Child Abuse NC website, www.preventchildabusenc.org, free of charge, and counts toward Health and Safety Training. This training is part of the Health & Safety Training and should be completed again within five years. You explained you did not realize it needed to be completed every five years. Medical Action Plans - Growing up healthy is every child's right and every parent's dream for their children. Significant advances in improving the overall health and development of children with chronic illnesses have been made. Having a medical action plan in place when caring for children with health issues is vital in ensuring their health and safety. These health issues could include allergies, asthma, or other chronic conditions that require specialized health services. The maintenance of these plans and the ability to implement such plans for the treatment of reactions are essential. It is important to remember that a medical action plan must be attached to the child’s application for enrollment, completed by the child's parent or a health care professional and must be updated on an annual basis. Permission to administer emergency medications must be updated every 6 months by the parent. You immediately contacted the parent to have them obtain the action plan. Another parent was contacted to complete the medication authorization.
