Name of Operation: LITTLE HANDS N FEET CHILD CARE CENTER Facility ID: 92002288 Consultant: ELIZABETH LESTER Operation Type: Center Case Number: 0226-262L Visit Date: 3/10/2026 Number Present: 30 Completed Date: 3/10/2026 Age: From 3 To 5 Total Minutes: 105 Time In: 10:30 AM Time Out: 12:15 PM Time In: Time Out: List to Use: Center Type Of Visit: Self Report Announced/Unannounced: Unannounced The purpose of today’s visit was to obtain information related to a self-report received by the Division on 2/25/2026. During today’s visit, the director assisted me with the visit. During the visit, I discussed the information with the director. Based on information obtained, the following was determined. On 2/24/26 a child was left unattended for approximately five (5) minutes while the other children went to the restroom. The restroom is outside of the classroom. Prior to the visit, the director reported the incident to me via email, and I advised her to follow up with the DCDEE intake reporting line to ensure it was received by our agency. The director stated the parents were informed of the incident. They wrote up on incident report and spoke to the child’s mother when she came to pick up the child. The caregiver was interviewed and she stated that she was transitioning to the bathroom and accidently left a child sitting on their cot as they went to the bathroom right outside the room. The director reviewed the video and the child was safe and reading a book on their cot. Limited monitoring of child care requirements occurred during today’s visit. The following violation was confirmed during today’s visit: Violation Number Comment Rule 303 Children were not adequately supervised at all times. On 2/24/2026, a three year old child was left unattended today for five minutes while the classroom went to the bathroom which is located outside of the classroom. .1801(a)(1-5) You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited during today's visit may negatively impact your compliance score. RESPONSE In response to this incident, the director held a staff meeting and met with each staff member. She shared and reviewed the existing supervision and head count policies. She added to the policies and shared that with the staff members. The staff signed that they reviewed and understand the updated policies and procedures. The director has the staff call up front before leaving the classroom to go to the bathroom, so there are always two (2) teachers while the children transition to other areas in the building and the playground. TECHNICAL ASSISTANCE All staff must count the children every time they transition from place to another. Please continue to use and the shard count sheets.
