Licensing supervisor, Michele Remington, was present during today’s visit. Upon arrival, we were greeted by the provider Maimah Ellis. You assisted me with the visit. I completed a general walk through of the children's space, kitchen area, outdoor space and bathroom. At the time of the visit, there were seven (7) children present. Five preschool and 2 school aged children. The children were observed playing with materials, playing in centers, and interacting with the provider. LICENSE STATUS Currently this Family Childcare Home (FCCH) operates with a five-star license issued 2/13/19. INSPECTIONS I reviewed required inspections including fire drills, emergency drills, and outdoor inspections. All inspections were found to be in compliance. DESIGNATED CHILDREN’S SPACE The childcare space is located in a converted garage with double doors. The children’s space has a variety of manipulatives, pretend play toys, books, and art material available. The bathroom is located to the right just past the childcare space and is accessible to children who are potty trained. A changing table is located right outside the childcare space. The children use mats for rest time. MONITORING During this visit, a full assessment of Child Care Requirements was conducted. A checklist was used to note the requirements monitored today. All spaces were monitored for supervision, capacity, children files materials, equipment and required postings. The checklist will be provided at the end of the visit. ANNUAL IN-SERVICE TRAINING I reviewed annual in-service trainings requirements. Your annual calendar year is January 1-December 31. One violation was documented: Violation Number Comment Rule 930 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years from the date of issuance). A household member, the provider's husband, expired February 2025. G.S. 110-90.2 & .2703(n) & (o) Please mail or email your letter of compliance by July 16, 2025. The letter must address each violation and describe how the violations were corrected. Any violations documented during visits to your program may have an impact on the total compliance history score and cause your score to drop below the mandated level. **Prior to today’s visit, the compliance history score for the center was 100%. Any violations cited during today's visit may affect this percentage. TECHNICAL ASSISTANCE and CONSULTATION >>Your husband’s qualification letter expired in February due to medical reasons. We discussed the procedure for obtaining a medical wavier. >>We also talked about the new QRIS system. We will share the new system as soon as we get trained and we can decide which system would be best. AGENCY UPDATES WATER TESTING You have completed the lead paint/water and asbestos testing. I reviewed RTI website summary, and it appears that you still need asbestos. You stated that you will check because you are pretty sure that you turned everything in.
