Facility ID: 92004037 Consultant: KIMBERLY WIMBERLY Operation Type: Center Case Number: Visit Date: 6/4/2025 Number Present: 63 Completed Date: 6/4/2025 Age: From 0 To 5 Total Minutes: 155 Time In: 10:10 AM Time Out: 12:45 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today's unannounced visit was to monitor child care requirements during a routine unannounced visit. Today’s visit was conducted with Wanda Dixon, Co-Administrator. I reviewed the NC Secretary of State’s website and the owner of the facility, Little Believer’s Academy, Inc. is listed as current-active as of 6-4-25. Program requirements The following items were observe posted: first aid poster, license, no smoking sign, summary of the law and emergency care plan. The indoor environment was monitored for safety and no items of non compliance were observed. Daily arrival and departure times of the children are documented on clipboards in each classroom. All space used by the children were adequately heated and cooled. An incident log is stored in the Director’s office. Fire inspection and sanitation inspections were current and on file. The last documented outdoor playground inspection was 6-2-25. Fire drills and EPR drills were current and on file. The last fire drill was conducted on 5-9-25 and the last EPR drill was conducted on 3-28-25. Remember that a new EPR drill must be conducted by 6-30-25 in order to remain in compliance. Staff Records: I monitored two new staff files who were hired since the last annual visit during my visit today. I observed that one staff member was missing orientation, proof of review of EPR plan, proof of review of EMC plan and signature receipt of operational and personnel polices. A second staff member was missing an application and proof of completion of orientation required within the first two weeks of first day of employment. Criminal background check qualification letters were observed current and on file for all staff. Staff/Child Ratios and Supervision: During today's visit I observed the children playing in centers, playing outside and some were having circle time. The Infants were being changed and having tummy time. Supervision was observed being maintained. Staff/child ratios were observed in compliance. Medications and Hazardous Products: All hazardous products were either locked up or placed out of reach of the children. Six violations of child care requirements regarding staff records were observed out of compliance. A corrective action letter stating how each violation was corrected is due to me no later than June 18, 2025. The letter can be mailed or e-mailed to me at the bottom of this report. Violation Number Comment Rule 1030 Application for employment and date of birth was not on file for all staff. One staff member did not have an application on file during my visit. .0302(d)(1)(A) 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. One staff member hired on 4-16-25 did not have documentation that 16 hours of orientation had been completed. .1101(a) 1067 Each new employee did not complete, within the first two weeks of employment, six clock hours of training in required topic areas. Two staff did not have documentation on file of the six hours of required orientation training. .1101(a)(b) 1230 Personnel policies were not discussed with each staff at employment and a copy was not available to all staff. One staff member did not have documentation on file that personnel policies were discussed. 10A NCAC 09 .0514(e) Technical Assistance: *Keeping staff records organized is key to maintaining a well-run child care center. With a structured system in place, Administrators can easily track all paperwork required by DCDEE ensuring that every staff member is up to date and ready to provide quality care. Whether using digital tools or physical files, a well-maintained record system saves time, reduces errors and supports a strong professional team. I encourage you to keep tweaking your process for records and develop a system that works for your individual program and watch your efficiency soar!
