I was greeted by the director Ciera Baker, and she assisted me with the visit. There was a total of 71 children present. The children were observed playing outside, reading books, transitioning from the playground, and completing personal care routines. LICENSE STATUS Currently this center operates with a five star license issued 03/01/2023. INSPECTIONS I reviewed all required inspections including fire drills, emergency drills, sanitation, and outdoor inspections. Inspections were found to be in compliance. MONITORING During today’s visit, a partial assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. I reviewed five new staff file and discussed required trainings. ANNUAL TRAINING The annual training year for this facility is July 1st-June 30th The following violations were documented: Violation Number Comment Rule 603 All floors and floor coverings were not constructed of nonabsorbent material and/or were not kept clean and in good repair. In space 104 (bees) there were two rugs that were not clean and had stains. 15A NCAC 18A .2824(a)&(b) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In space 124 & 106, there were outlets without covers. 10A NCAC 09 .0604(c) Please mail or email your letter of compliance by May 19, 2026. The letter must address the violation and describe how the violations were corrected. Any violations documented during visits to your program may have an impact on the total compliance history score and cause your score to drop below the mandated level. **Prior to today’s visit, the compliance history score for the center was 97%. Any violations cited during today's visit may affect this percentage. TECHNICAL ASSISTANCE **All outlets must be covered when not in use. In two classrooms there were outlets exposed to and not blocked by furniture. **Carpeting used as a floor covering must be kept clean and in good repair. In the Bee’s (104) classroom there were two area rugs that had stains and not clean. QRIS RATED LICENSE We discussed the QRIS- Rated License standards. You are looking into Pathway 2- classroom and instructional quality. We went over each standard in detail: enhanced ratios, education standards, family and community engagement standards, CQI plan, approved curriculum and formative assessments, assessments shared with families, and coaching/mentoring. We discussed the staff information sheet. You are still getting WFID numbers and WORKS letter, so you need some time to complete it. I will check back in on June 19th. Your program currently operates with a Five-star license. Today’s visit was conducted with you, Jan Baker, Center Director and Charles Skeeter, Director of Special Education. You visited each child care area with me. I monitored all classrooms in use for compliance with staff/child ratios, supervision, capacity, group size and staff interaction/discipline. A group of children were on a field trip. Other children were playing outside. The facility was licensed on 8/29/22. Information regarding compliance was documented on a checklist that was left with you today. The facility was monitored for health and safety requirements. The program requirements, along with equipment and furnishings were also observed. A sampling of children’s records and staff files was also monitored. Information was documented on the staff and training worksheet and children’s records form. One violation regarding children’s records was documented and technical assistance was provided. The facility does not provide transportation. The owner of this facility is listed as current/active with the NC Secretary of State. Violation Number Comment Rule 1320 Children's records that include an application for enrollment, medical and immunization records, and permission to seek emergency medical care was not on file for each child. A medical record was not on file for one child. GS 110-91(1);.0302(d)(2); .0304(g) The violation documented today must be corrected immediately.
