Due to Mi-fi connection issues. During today’s visit, the assistant director Tikara assisted me. During the visit, I discussed the information with Ms. Tikara. Based on information obtained, the following was determined. On 6/4/2026, a young toddler DOB: 4/1/2026 was left in the classroom while the teachers and other children were outside. The child was alone in the classroom for eighteen (18) minutes. The child was not injured and still enrolled. It was discovered that the staff were not doing the required face-to-name sheets. The child’s family was informed of the incident. Prior to the visit, Mrs. Tikara reported the incident to me via phone, and I advised that I would follow up with the DCDEE intake reporting line to ensure it was received by our agency. The caregivers and director were suspended then terminated due to the policies/expectations the facility has in place to ensure children transition place to place. Limited monitoring of child care requirements occurred during today’s visit. The following violation was cited. Violation Number Comment Rule 303 Children were not adequately supervised at all times.On June 4, 2026 a child was left alone in the classroom for 18 minutes while the class transitioned to outside. .1801(a)(1-5) Please mail or email your letter of compliance by June 30, 2026. The letter must address the violation and describe how the violation was corrected. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. RESPONSE In response to this incident, the district manager, talked to every teacher about the importance of the face-to-name and reinforced the policy and why it is in place. Since the center does not have a director, the other admin team is planning to have a staff meeting with another director from a sister school. The staff meeting will focus on the face-to-name transition sheets. I asked Tikara to send me the agenda and sign in sheet with the violation correction letter. TECHNICAL ASSISTANCE Children must be supervised at all times. When transitioning to different places within the center, staff must ensure that they have all their assigned children by counting the children several times, using a head count sheet, face-to name sheet or the attendance sheet. Elizabeth Lester, Child Care Consultant, accompanied me during the visit. Tikara Hemmingway, Assistant Director, accompanied us during a walk-through of the facility. During the visit, we discussed the allegations with Michelle Mull, Director, and additional staff members. The following violations were observed and/or confirmed during today’s visit. Violation Number Comment Rule 611 All beds,cots, or mats with individual linen were not provided for each child. Four children were observed napping on cots without linens. 15A NCAC 18A .2821(c) 871 Center staff did not comply with the safe sleep policy. During the visit, "I can roll" was not posted on the cribs for infants. One infant was sleeping in their crib wearing a bib. The bib was removed during the visit. 10A NCAC 09 .0606(a) 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. A staff member did not pick a three-year-old child up appropriately when attempting to remove the child from the classroom, and the child sustained marks on their under arm. On at least two occasions, two staff members engaged in verbal confrontations in the presence of children. G.S. 110-91(10) Violations must be corrected immediately. Upon arrival, I was greeted by the director Michelle Mull, and she assisted me with the visit. I completed a walk-through of the facility. At the time of the visit, there were 92 children present. The children were observed preparing for and eating lunch and completing personal care routines. The infants were observed enjoying tummy time and sleeping in their assigned crib. LICENSE STATUS Currently this center operates with a three- star rate license issued January 16, 2020. This center is in cohort 2. The preparation year started July 1, 2024 and the assessment year will start July 1, 2025. INSPECTIONS I reviewed all required inspections including fire drills, emergency drills, sanitation, fire and outdoor inspections. Inspections were found to be in compliance. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children. A selection staff files were reviewed. A selection of children files was reviewed. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. A checklist was used to note the requirements I monitored today. The checklist will be provided at the conclusion of the visit. The following violation was observed or documented. Violation Number Comment Rule 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. One child did not have a health assessment on file. Any violations documented during visits to your program may have an impact on the total compliance history score and cause your score to drop below the mandated level. **Prior to today’s visit, the compliance history score for the center was 100%. Any violations cited during today's visit may affect this percentage. TECHNICAL ASSISTANCE **One child’s file is missing their medical report. You immediately sent the parents a message requesting the medical report. **I reviewed your latest fire inspection. It is due by 7/5/24. You said that you have left a message with the Fire Marshal. Please send me a copy of your fire inspection form once it is completed. **Please be sure all staff has their updated education in WORKS. We also discussed prep year assessment.
