The facility’s last annual compliance visit was on April 23rd, 2025. A checklist was used to note the requirements I monitored today. The center currently operates with a 5-star license (NAC accredited 4/23/2025)- operating at 30sq. ft. inside, & enhanced ratios and space. My visit was conducted with LaShawn Scriber, Director and center staff. The children were observed participating in indoor and outdoor play, free play, center play, story/circle time, tummy time and napping in the infant room, personal care, setting up and participating in lunch. (Lunch followed USDA guidelines for children in childcare, The enrolled four and five year old children along with the school-age children present were observed listing to the three little pigs story and following that the teacher hung paper to look like the material of each pigs home and children said, “I will huff and I will puff and I will blow your house down.” While standing in front of each. It appeared based on their participation that the children enjoyed participating in an activity having to do with the book they just listened to. During the walkthrough to obtain enrollment and conduct my observations. I monitored the required posted documents, interactions between staff and children, discipline, supervision, group sizes, space capacity, equipment, furnishings, and medication requirements (one epi-pen and paperwork monitored). VERIFICATION OF RECORDS: *Last fire inspection= 1/9/2026 and approved for daytime care only *Sanitation inspection=12/4/2025- 16 demerits and an approved rating *EPR plan updated – 4/8/2025 and again today 3/24/2026 due to a conversation related to updating the plan and the fact that it says “draft”. We worked step by step and corrected the errors and published the plan. *Facility is approved to provide transportation and currently transports school-age children to and from school for before and after care. * Registration expires 6/30/2026 insurance expires 12/1/2026 * Clean water for Carolina Kids requirement Water- 11/15/2024 due by 11/15/2027 Lead paint- 7/22/2024 Asbestos- 12/11/2024- testing completed and report received 1/15/2025- no asbestos found. * The owner of the facility KinderCare Learning Center LLC is current/active in the NC Secretary of state website. *Compliance history prior to 3/24/2026= 99% The following violations were cited today Violation Number Comment Rule 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. Review of fourteen children's files found five files did not contain either the doctor portion, the parent portion of the document complete or a medical was missing from an enrolled child's file. GS110-91(1) 1322 A written statement from each child's parent giving standing permission which may be valid for up to twelve months for participation in off premise activities that occur on a regular basis was not available. review of fourteen children's files found one child's file did not contain the form giving permission to play outside the fenced in area. (this form includes permission to be outside the fence in case of a fire drill or other emergencies). .1005(b)(4) *****TECHNICAL ASSISTANCE/REMINDERS: ******* • In order to keep organized and keep track of paperwork in the children’s files the file to avoid violations, it can be split in two with the KinderCare checklist and paperwork attached to the left side of the file folder and the DCDEE checklist and paperwork attached to the right side of the folder and the paperwork in order based on the checklist. Have a spread sheet reminder on your desktop to remind you following receipt of a file what if anything was missing and when it is due to be submitted so you no longer have missing information from files. • Although staff files were reviewed today, a copy of your staff and training worksheet was not received. Please send me a copy of your worksheets once they are complete. • A conversation was had regarding the city of Raleigh/DCDEE child and adult fire inspection report #17 related to maximum 20% of decorative materials covering walls- nothing suspended from the ceiling. Violations of childcare requirements were cited during today’s visit. A letter of corrective action is required within 14 business days April 7th, 2026. The facilities’ last annual compliance visit was on April 23rd, 2025, and an AC follow-up on May 8th, 2025. A checklist was used to note the requirements I monitored today. The center currently operates with a 5-star license, operating at 30sq. ft. inside, & enhanced ratios and space. An NAC accreditation certificate was issued April 23rd, 2025. My visit was conducted with you, LaShawn Scriber Director, and center staff. The children were observed participating in picture day that was set up in the school-age classroom, circle time in the preschool classrooms and napping, feeding and diaper changing in the infant classroom. During the walkthrough to obtain enrollment and conduct my observations. I monitored required posted documents, interactions between staff and children, discipline, supervision, group sizes, space capacity, equipment, furnishings, program records and transportation requirements. The center currently operates with a 5-star license (NAC accredited)- error made and will be corrected. License currently states NAEYC not NAC, operating at 30sq. ft. inside, & enhanced ratios and space. My visit was conducted with LaShawn Scriber, Director and center staff. The children were observed participating in indoor and outdoor play, free play, center play, story/circle time, tummy time and feeding in the infant room, personal care, setting up and participating in lunch. (Lunch followed USDA guidelines for children in childcare- pizza, spinach salad with ranch, mixed fruit and milk), school-age children were observed cutting out paper money and identifying coins and bills. During the walkthrough to obtain enrollment and conduct my observations. I monitored the required posted documents, interactions between staff and children, discipline, supervision, group sizes, space capacity, equipment, furnishings, and medication requirements (one epi-pen and paperwork monitored). A soccer shots coach was present working with a group of children during today’s visit. Based on the randomly selected children's files it was found that there were missing medical statements, parent acknowledgement of laws and policies along with incomplete applications. If parents are bringing in food and decorations, please make sure no balloons are included as rule 10A NCAC 0604(q) prohibits balloons. • Outdoor Area: The outdoor learning environment offers a sense of freedom for children. Since children are exploring and taking risks it is very important to provide a safe environment. Proper maintenance is a key factor when trying to ensure a safe play environment for children. The environment includes the surfacing where grass, sand, mats, or other surfacing needs to be free of tripping hazards as observed in the toddler playground area where toddlers are new walkers and are unbalanced. The gully caused by water run off is a hazard as it is beside the walkway and stepping off they can fall. The mats that are cushioned were power washed and are lifting and have caused a tripping hazard. During the walkthrough to obtain enrollment and conduct my observations. I monitored for required posted documents, interactions between staff and children, discipline, supervision, group sizes, space capacity, equipment, furnishings, and medication requirements (on epi-pen and paperwork monitored). VERIFICATION OF RECORDS: *Last fire inspection= 12/4/2023and approved for daytime care only *Sanitation inspection=5/22/2023 *EPR plan updated - 9/28/2023 *Facility is approved to provide transportation and currently transports 14 school-age children to and from school for before and after care. * Registration expires 7/30/2024 insurance expires 12/1/2024 * Water, lead paint and asbestos testing are in process. The last water testing was 11/2021. * The owner of the facility KinderCare Learning Center LLC is current/active in the NC Secretary of state website. *Compliance history prior to 5/15/2024 95% Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. One of the toddler playgrounds had two areas of tripping hazards. One was from water run off that created a hole just off the sidewalk and the second was the rubberized surfacing that was power washed and lifting creating a tripping hazard. 10A NCAC 09 .0601(a) 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. It does not specify anything other than “maintained separately from the staff member’s individual personnel file. The sealed file folder labeled confidential in the same hanging folder as the binder is fine because they are filed separately. There is no specifics about where in the rule other than maintained separate. 10A NCAC 09 .0701 HEALTH STANDARDS FOR CHILDCARE PROVIDERS, SUBSTITUTE PROVIDERS, VOLUNTEERS, AND UNCOMPENSATED PROVIDERS Each staff member did not have the required medical report, proof of tuberculosis test or screening, and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. • A conversation was had today due to this week being teacher appreciation. If parents are bringing in food and decorations, please make sure no balloons are included as rule 10A NCAC 0604(q) prohibits balloons. • Outdoor Area: The outdoor learning environment offers a sense of freedom for children. Since children are exploring and taking risks it is very important to provide a safe environment. Proper maintenance is a key factor when trying to ensure a safe play environment for children. The environment includes the surfacing where grass, sand, mats, or other surfacing needs to be free of tripping hazards as observed in the toddler playground area where toddlers are new walkers and are unbalanced. The gully caused by water run off is a hazard as it is beside the walkway and stepping off they can fall. The mats that are cushioned were power washed and are lifting and have caused a tripping hazard. They need to either be glued down or replaced so the surfacing is flat. • Safe sleep position checks: 10A NCAC 09 .0606 SAFE SLEEP PRACTICES (a) Each center licensed to care for infants aged 12 months or younger shall develop, adopt, and comply with a written safe sleep policy that: (7) specifies how caregivers shall document compliance with visually checking on sleeping infants aged 12 months or younger. * Review of sleep charts found missing weeks. No matter who is working in the classroom full time, part time, floater, substitute should know who is in attendance and where sleep charts are kept and told upon arrival to the classroom who is napping (if anyone) and shown the last time they were checked and reminded to document the napping period for the child every 15 minutes. You reported that fifteen children were two years old and four children were three years old. Two staff members were present along with a high school student. The facility meets enhanced staff/child ratios. Based on enhanced staff/child ratios the maximum group size for children aged two years old is eighteen. One child aged three was shifted to another classroom. One violation regarding group size was documented. Violation Number Comment Rule 1756 Enhanced staff/child ratios and group sizes were not met. At that time, you changed staff’s hours so that they would arrive earlier to make sure staff/child ratios in the two morning homeroom classrooms complied. You also explained that in 2023, staff were instructed on how to shift children to other classrooms when children are dropped off and it may cause classrooms to be out of compliance with staff/child ratios. Staff stated they have walkie-talkies they use to call for assistance if a child is dropped off and the staff need to shift a child out of their classroom to maintain staff/child ratios. We discussed that you would have a staff meeting to discuss steps staff should take to ensure the classrooms always comply with staff/child ratios/group size. You also stated you will talk with parents about staff/child ratios when children are dropped off. You may also need to revisit your current staffing plan to ensure staff/child ratios/group size are always maintained. You asked if a classroom was out of staff/child ratio for forty seconds to two minutes does this count as being out of ratios. I explained it does count. I stated there should not be a time a classroom is out of staff/child ratio.
