Your last annual compliance visit was conducted on September 23rd, 2025. A checklist was used to note the requirements I monitored today. The center currently operates with a 5-star license, operating at 30sq. ft. inside, & enhanced ratios and space. This facility was issued with its NAEYC accreditation license on August 14th, 2025. During today’s visit we visited DCDEE’s website, and I asked that you print and post the pathway 3 accreditation staff/child worksheet posted online to recognize accreditation. My visit was conducted with Ajeenah Foureau, center director and center staff. Some of the classrooms were observed participating in teacher directed & free play activities today both inside and outside time. I observed serving and eating lunch (meal was in compliance with USDA guidelines for children in childcare), preparing for outdoor play, participating in outdoor play, and the making of binoculars with paint, toilet paper rolls and feathers to participate in bird watching tomorrow. I observed the cooking academy and the teacher worked with the children to make watermelon aqua fresca- I heard her discussing the ingredients with the children and heard a blender buzzing. During the walkthrough to obtain enrollment and conduct my observations I monitored required posted documents (license, EMC plan, sanitation inspection, first aid chart), interactions between staff and children, discipline, supervision, group sizes, space capacity, program requirements, equipment, diaper changing routines and documented infant sleep charts) furnishings. This facility does not provide transportation to enrolled children. VERIFICATION OF RECORDS: *Last fire inspection= 10/25/2025 *Sanitation inspection= 3/24/2026 with 0 demerits and a superior rating * Fire drill- 2/10/2026 * Shelter in place- 3/16/2026 * Playground inspection- 3/9/2026 * Water testing- 7/14/2023- due by 7/14/2026 * Lead Paint- 6/21/2024 * Asbestos- 7/24/2024 *One violation was cited during today’s visit Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). A staff members CBC letter expired 3/11/2026 and requires renewal. G.S. 110-90.2(b) & .2703(n)&(o) Technical Assistance: A violation of childcare requirements related to criminal record qualification renewal was cited during today’s visit. All staff are required to complete their criminal record qualification renewal prior to expiration. Your last annual compliance visit was conducted on October 30th, 2024. A checklist was used to note the requirements I monitored today. The center currently operates with a 5-star license, operating at 30sq. ft. inside, & enhanced ratios and space. My visit was conducted with Tiana Padin, assistant director and Alicia Cothran, program specialist and center staff. Some of the classrooms were observed participating in teacher directed & free play activities today both inside and outside time. During the walkthrough to obtain enrollment and conduct my observations I monitored required posted documents (license, EMC plan, sanitation inspection, first aid chart), interactions between staff and children, discipline, supervision, group sizes, space capacity, program requirements, equipment, diaper changing routines and documented infant sleep charts) furnishings, medications requirements. This facility does not provide transportation to enrolled children. VERIFICATION OF RECORDS: *Last fire inspection= 10/30/2024 *Sanitation inspection= 2/10/2025 with 10 demerits and a superior rating * EPR plan last updated= 3/2025 * Fire drill- 5/30/2025 @ 10:44am and took 2 minutes * Shelter in place- 3/5/2025 @10:15 am and took 2 minutes * Playground inspection- 5/27/2025 * Water testing- 7/14/2023 * Lead Paint- 6/21/2024 * Asbestos- 7/24/2024 *One violation was cited during today’s visit Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. The turf surfacing on the soccer field as well as the PS2 playground have areas that are tripping hazards as well as insufficient in depth based on the turf missing and the foam layer being picked at by the children or wear and tear because it is exposed to weather elements and requires replacement or repair. 10A NCAC 09 .0601(a) Technical Assistance: The number one cause of injuries on playgrounds are falls to the surface. Maintaining the correct depth of protective surfacing material is crucial for safety. The outdoor learning environment offers a sense of freedom for children. Children can play freely with peers, expand their imagination beyond restraints of indoor activities, release energy and explore their sense of touch, smell, taste and their sense of motion. Since children are exploring it is very important to provide a safe environment. Climbing equipment on the playgrounds used for toddlers and preschool-aged children contained mulch surfacing that measured between 4 - 5 inches at the exit of the slide rather than the required 6 inches. On the playground used for school-agers is a metal climbing structure with a critical height of over 8 feet. Therefore, the mulch underneath and around the structure must be at least 9 inches in depth. The mulch measured between 1 - 2 inches underneath and around the structure. A violation of childcare requirements was cited during today’s visit and requires a letter of corrective action by June 20th, 2025. The facilities last annual compliance visit was November 15th, 2023. The purpose of today’s follow-up was to review staff files, your signature on the bottom of staff and training worksheets serves as verification that the information provided is accurate and complete. My visit was conducted with administrator Ajeenah Foureau. *The following violation of childcare requirements was observed today. Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. A staff person whose start date was January 2023 did not have a medical completed. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. A staff person whose start date was January 2023 did not have a TB test completed. .0701(a) *****TECHNICAL ASSISTANCE/REMINDERS: ******* *A violation of staff files was cited today for a staff person who’s physical and TB were not completed during hire in January 2023. The administrator and I talked about strategies to make sure this does not happen again. Due to a violation of childcare requirements being cited during today's visit a letter of corrective action is required and due by December 16th, 2023 explaining how the violation was corrected and how you will kepp in compliance going forward.
