This facility currently operates with a Five (5) Star License issued on 8-16-23. The last annual compliance visit was conducted on 7-24-24. Today’s visit was conducted with you, Keisha Miller, owner/operator. Upon arrival to the facility, I was greeted by you. A walk-through of the downstairs area of the residence was conducted visiting the one (1) room used for childcare. The backyard was monitored where the children engage in gross motor play to ensure compliance with safe outdoor environment requirements. The required postings, along with appropriate equipment and furnishings were observed. The EPR Plan, emergency drills, outdoor inspections, the activity plan, daily attendance, and two children’s files were also observed and in compliance. Documentation of program records, as well as the operator verification information was observed and has been documented on the attached form. You stated that your SUV is used to transport children and the vehicle was monitored for compliance with transportation requirements. No medications were observed, as you stated that at this time no children have medication to be administered. The children awoke from naptime, completed toileting and handwashing routines before having snack at the table. Afterwards you read the book “Mr. Seahorse” as seahorses are the theme of the week. The children made seahorses out of cardstock that were painted with glue, then the children put oatmeal on the cardstock and added google eyes. A caring and nurturing environment was observed as you interacted with the children and attended to their needs. During today’s visit, an assessment of applicable child care requirements was conducted. The following violations of child care requirements were cited and must be corrected immediately. Violation Number Comment Rule 930 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years from the date of issuance). The child care provider's qualification letter expired on 3-5-26. G.S. 110-90.2 & .2703(n) & (o) 1104 Operator or other transportation provider did not comply with all applicable state and federal laws and regulations concerning vehicles and the transportation of passengers. The registration/license plate of the vehicle used to transport children expired on 6-30-26. G.S.110-91 & .1723(2) 1846 An individual over the age of 15, who resides in a family child care home or a center located in a residence on a permanent or temporary basis, or those who had their 16th birthday after the initial licensing of a family child care home; did not complete and submit required forms within five business days. A household member turned 16 years old on 8-3-25 and does not have a qualification letter on file. Upon turning 16 years old the household member must complete the criminal background check process. It was suggested to print the roster from the ABCMS Portal that contains the expiration dates of qualifying letters for you and your household members. You can mark your calendar and set alarms for reminders to have criminal background checks completed a couple of months prior to their expiration dates. Vehicle Registration: Maintaining valid vehicle registration is crucial because it is a legal requirement for all vehicles transporting children, ensuring the safety of the children, protecting you from liability, and complying with state and federal laws. It verifies that the vehicle is properly owned and insured, which is essential for safety during transportation. It was recommended to set a reminder in your phone and on your calendar to renew vehicle registration one month to 3 weeks prior to the expiration date to ensure the registration does not expire. • Your lead in water results were completed on 3-28-2025 and your asbestos and lead in paint from Clean Classrooms for Carolina Kids were completed on 3-21-25. • ASAP please link you and your household members to the ABCMS portal. We reviewed the different Pathway options and the requirements within each of the three Pathways. Resource and planning documents were shared with you. We completed the QRIS transition guide/conversation form, and a copy of this form was left with you today. This form indicates the chosen Pathway the facility would like to pursue, area(s) where support is needed, and the anticipated date when you will apply for a rated license. • We discussed both QRIS Pathway 1 and Pathway 2. After reviewing the options, you decided your facility will choose QRIS Pathway 2 – Classroom and Instructional Quality Pathway. We reviewed in detail the requirements for a Five Star License for Pathway 2 - Classroom and Instructional Quality. The application process, clickable links, forms, and resources for obtaining a Five-Star License for QRIS Pathway 2 Classroom and Instructional Quality were discussed today. • Please update your education in the WORKS system by submitting your official transcript to reflect your Bachelor’s degree. • Effective communication is needed until all steps of the transition to the new QRIS Rated License process have been completed and a new license is issued. • It is strongly recommended that you complete the NC Foundations for Early Learning and Development training. The purpose of the North Carolina Foundations for Early Learning and Development is to establish a framework that supports the healthy development and school readiness of children from birth to age 5. Be prepared to support those around you by getting certified in Mental Health First Aid. Mental Health First Aid teaches the skills needed to recognize and respond to signs and symptoms of mental health and substance use challenges, as well as how to provide someone with initial support until they are connected with appropriate professional help. In the free course, you learn risk factors and warning signs for mental health and addiction concerns, strategies to help someone in both crisis and non-crisis situations, and where to turn for help.
