A full assessment of child care requirements was conducted. Your program currently operates with a Five-star license. The facility was monitored for approved curriculum. Today’s visit was conducted with you, Pam Ray, Center Director. Michele Remington, Licensing Supervisor, was also present. You visited each child care area with me. I monitored classrooms in use for compliance with staff/child ratios, supervision, capacity, group size and staff interaction/discipline. Children throughout the facility were playing in activity areas, playing outside, completing ink art activities, playing with playdough, and participating in teacher led activities. Infants were having outdoor time on the porch area, having floor play time, and napping. Children were served lunch, completed routines, and prepared for rest time. I observed proper handwashing, and toileting practices. Staff assisted children with materials, facilitated play and group time, and routines. The facility was monitored for health and safety requirements both indoors and outdoors. Six violations regarding health and safety were documented. Program requirements, along with equipment and furnishings were monitored. One violation regarding program requirements was documented. All inspections and drills were current. A sampling of children’s and existing staff’s records, and all new staff records was monitored and information documented on the children’s records form and the staff and training worksheet. Two violations regarding staff records were documented. A checklist was also used to monitor compliance. The last annual compliance visit was conducted 9/26/2023. Prior to today’s visit, the facility’s compliance history was 97 percent. The owner/operator is listed as current/active with the NC Secretary of State. The facility does not transport children. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Two wall outlets not in use were uncovered in the infant/toddler indoor play area. 10A NCAC 09 .0604(c) 843 A drug or medicine was administered after its expiration date. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff member's CPR certification expired on 8/2024 and has not been renewed. .1102(d) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. On or before 9/25/2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. You explained children changed classrooms last week as part of the new school year. Any medication and authorizations were moved to the child’s new classroom. All medication and authorizations were not reviewed for expiration. You explained the staff takes this seriously. You also explained your program works with a Child Care Health Consultant. Today you immediately pulled medications and authorizations that were expired. We discussed using electronic reminders, creating spreadsheets, and creating a center wide renewal plan for authorizations. Items with Heating Elements – Children are curious and can touch items that are heated and burn themselves. All items with heating elements, even when unplugged, pose a burn hazard. A toaster oven was on the counter in a space that is not closed off and adjoins to the adjacent classroom. This space is used for storage, food related space, or where the refrigerator is kept. Today we discussed placing a gate to make the space inaccessible to children throughout the facility. Wood Splintering – A natural learning environment offers great benefits to children and requires ongoing maintenance. The wood used throughout the spaces was splintering in what appeared to be a shedding manner. You explained your facility works with an individual who maintains your outdoor learning environment. You will contact the individual to determine how to repair the splintering. Foam Blocks – Children under three can choke on foam that is easily torn. Children can bite foam toys and tear off small pieces that can be swallowed. Today foam blocks were immediately removed from being accessible to children. You explained you would find additional blocks to replace the ones that were removed. CPR and First Aid certifications - Completing CPR and First Aid training through an approved organization ensures staff receive the proper training that allows them to respond to children using the most up to date information in case of an emergency. Being able to respond responsibly is crucial and can be the difference between life and death. The staff member provided CPR and FA training information for completing the training online. Today you stated you did not realize the staff member completed CPR and First Aid online. You also explained the facility regularly provides CPR and First Aid classes for new staff. You have a training on 9/13/2024 that you will have the staff member attend in person. CONSULTATION: Hold harmless - Hold harmless has been extended until the new QRIS is implemented (SB 425). You already completed the rated license reassessment on 6/18/2024. The violations were reviewed with you at the end of my visit. The computer-generated visit summary was sent to you on 9/12/2024.
