The center currently operates with a 4-star license, operating at 25 sq. ft. inside, & ratios at .0713 (a). My visit was conducted with you, Felicia Turner, Lead Teacher/Assistant Director. The children were observed playing in centers and playing outside. You stated you do not provide transportation, and no infants are currently enrolled, and no medications are at the center. *****VERIFICATION OF RECORDS: *The last fire inspection was on 10-25-24. Remember to send your 2025 fire inspection within 5 days of completion. *Sanitation inspection=1-21-25 (Always scan your inspections to me). *The last outdoor playground inspection was on 8-11-25. Felicia completed playground safety training on 7-28-21. *The last fire drill was on 8-8-25. *The Lead Water test is due by 7-12-27. *The EPR Plan was updated on 8-25-25. Felicia completed EPR training on 8-23-16 and by Jo on 9-15-15. *I reviewed the NC Secretary of State’s website and observed the owner of the facility (Jo’s daycare, Inc.) listed as current-active. *The center uses Creative Curriculum, an approved curriculum. Technical Assistance was offered on how to correct the violations cited and how to maintain compliance. To maintain compliance with NC Childcare laws/rules regarding childcare operation, the following violations must be corrected immediately. Email a corrective action letter by 9-8-25 to michelle.ezzell@dhhs.nc.gov. Violation Number Comment Rule 303 Children were not adequately supervised at all times. A three-year old child ran out of the center after his/her mother and was unsupervised by a staff member. The neighbor saw the child and was with him/her when the owner arrived, within minutes. .1801(a)(1-5) 1311 Emergency medical care information was not on file in the center on the child's first day of attendance and/or was not updated as changes occurred or at least annually for each child. One child enrolled February 2024 did not have the emergency medical care information updated at least annually. .0802(c) 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. Two children enrolled in February 2024 and March 2025, did not have a medical exam or health assessment on file within 30 days. GS110-91(1) *****TECHNICAL ASSISTANCE/REMINDERS: ******* *Children’s Records: We discussed using the children’s file checklist to avoid missing any deadlines when children’s records are due or need to be updated. *Supervision was discussed in detail, along with examples of inadequate supervision. Children should be adequately supervised at all times. Adequate supervision shall mean that staff interact with the children while moving about indoor or outdoor areas and are able to always hear and see the children, except when emergencies necessitate that direct supervision is impossible for brief periods of time. We discussed trainings offered on supervision. ***ABCMS-The ABCMS portal must be completed immediately. In May 2025, I offered a director’s meeting and Serena Luke (serena.luke@dhhs.nc.gov), with the CBC department reviewed step-by-step instructions with the Directors. *Linking Criminal Background Checks – Staff need to log into the Criminal Background Check Portal on the DCDEE website and link to the facility license/ID number. It is important to do this so staff at DCDEE can contact the facility if a random criminal history check uncovers a staff member with a criminal charge or conviction working in a childcare facility. It is also a way for staff at the CBC Unit to keep you updated about the status of out-of-state checks if a new employee has a Provisional Qualification. Instructions on how to link to the facility were discussed with you and emails and newsletters were sent to you. *In the future, new staff need to be linked within five days of hire or transferred to your center. Review Criminal History Record Check Requirements for Child Care Centers in the NC Child Care Requirements 10A NCAC 09 .2703. Education is based on what’s on file in WORKS/WORKFORCE’s database. *PROGRAM STANDARDS: For trainings on environment rating scales go to www.ncrlap@uncg.edu ***********FYI-Self-Report Made by Felicia ********************** *IN SUMMARY: On Tuesday, August 19, 2025, Felicia, Administrative Assistant made a self-report notifying me that a three-year-old child ran out of the center after his mother and was unsupervised by a staff member within minutes. The neighbor saw the child approaching her before Ms. SeBerry was approaching her too. The center is a converted house about 1,500 square ft. with three classrooms. It’s enclosed by a chain link fence, the fence encloses the parking lot and the playground, located in the back. The playground has a separate fence in area. When entering the facility through the back door, there’s a foyer area and another doorway. The Director’s desk is to the left and the kitchen is on the right. It’s an open area with two classrooms and a bathroom. Space two (2) is behind the Director’s office and beside the bathroom. On 8-19-25, the child arrived at the center at 8:18, it was his 1st day at the center. His mother dropped him off in space two (2) which was the only open classroom. A total of four, three-year-old children were present. Felicia was in the process of serving breakfast and asked the child if he wanted breakfast. The child responded “yes”. Felicia turned to pour him a cup of milk, the child began looking around and ran to the back door toward his mother. Felicia called out to him several times as she was gathered the three, three-year-old children. She and the children ran to the empty classroom (space 1) located in front of the facility and opened the front door, still calling out to him; Felicia could see him heading to the sidewalk. Felicia stated she thought the child was going home, since he lives across the street from the daycare. He was looking to the right, in the direction of his mother’s car. At 8:25, Felicia immediately called 911 as he was getting out of her sight and gave them the child’s description and described what he was wearing. She hung up on them and called the child’s mother, and she said she was on her way back. Felicia called Ms. Jo which was less than a half-a block away, approaching the daycare. Ms. Jo saw a neighbor she knows with the child. The child was not upset and was not hurt. Ms. Jo returned the child safely to the center. Felicia stated this all happened so fast. Felicia stated during this time, no vehicles passed in front of the center during this time and explained the elderly neighbors usually sit on their porches or walk in the neighborhood. Felicia and Jo stated the neighbors were interviewed and they stated the child had run out of his yard before. Back at the center, the child’s mother, father, grandfather and police arrived and stated the child had run out of his yard and they were worried he may run from the daycare. The mother asked if the child “could come back to the center tomorrow”? The child was taken home for the rest of the day. The child returned to the center the next day. Jo and Felicia stated before the child started, the mother was competing the child’s enrollment forms, she joking make a comment “I don’t know” he may “run-away”. Jo and Felicia stated they were not sure what she meant. After this incident Jo and Felicia talked to the mother and informed her that this is something they should have been aware of so they could put extra procedures in place. Since the incident, the center has the following procedures in place. I observed some of these changes during my visit: *Turned the chime up on the back exterior door. The door automatically locks from the outside, but not the inside. *The interior door leading from the foyer to the entrance now stays closed. *The door to space one (1) stays closed since this classroom has a direct exit to the front of the center. *Staff have discussed supervision issues and different ways to prevent this from happening again. *Administrative staff will ask parents of newly enrolled children specific questions about the child they need to know to keep the child safe. *Staff hold the child’s hand as they transition around the center. *When the child enters the classroom, he’s been given a “special job” to do to keep him occupied, while staff are watching him closely. *The gate around the entire center will be closed after 9am and will be opened after 3:30. Between 9:00 and 3:30 pm parents and visitors will have to manually open the gate. My visit was conducted with you, Felicia, Assistant Director/Lead Teacher and Ms. SeBerry, Director was present and working in a classroom. The facility currently operates with a 4-star license that’s in the hold harmless stage. Your last annual compliance visit was on 10-23-24. I conducted a partial assessment monitoring safety issues, supervision, capacity, discipline, posted documents, permit restrictions and program records. I monitored all staff’s files for completion of CPR/1st Aid, SIDS, Health & Safety Trainings & CBC’s. Felicia stated no incident reports have been needed since my last visit and no medications are at the center. No new staff have been hired since 10-23-24. Space 1, the classroom for infants is currently closed since no infants are enrolled. The children were observed playing outside and preparing for lunch. During my visit, A. Donnell, Behavioral Therapist, was observed working one-on-one with a 2- or 3-year-old child. Ms. Donnell’s criminal background letter was on file and in compliance. ***Verification of Records & Training*** *All records/files were organized. *The last fire inspection was on 10-25-24. Send me your 2025 inspection by 10-25-25. *The last sanitation inspection was on 1-21-2025. *The last playground inspection was on 4-9-25 by Felicia. If I do not receive your letter within the established time, a return visit may be made. Additional violations may be documented if the violation has not been corrected/addressed. Staplers, stapler removers, several pairs of adult size scissors and BIC correction fluid (liquid white out) were located on the top of children’s cubbies in space 2 measuring 3 ft. high and on a child size shelf in space 3. In space two (2) a staff’s handbag was in an unlocked closet, sitting on top of cots with the shoulder strap, hanging within a child’s reach. The cots were propped/leaning on the side, measuring 4 feet 5 inches tall. In space 2, two-gallon size Ziploc bags used to hold toys were within children’s reach. 10A NCAC 09 .0601(a) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). There was one (1) employee, J. Fennell, that was present and working in the kitchen that did not have a current CBC qualifying letter on file with DCDEE; her qualifying letter expired on 2-28-25. A CBC letter must be on file by 5-7-25 or Ms. Fennell cannot be on the premises starting 5-8-25; until a current CBC letter is on file at the center. G.S. 110-90.2(b) & (d) & .2703(e) *Technical assistance was given regarding the following violations: *CRIMINAL BACKGROUND: There was one (1) employee, Joyce Fennell, that was present and working in the kitchen that did not have a current CBC qualifying letter on file with DCDEE; her qualifying letter expired on 2-28-25. A CBC letter must be on file by 5-7-25 or Ms. Fennell cannot be on the premises starting 5-8-25; until a current CBC letter is on file at the center. *SAFETY: teacher supplies were located on the top of children’s cubbies in space 2 measuring 3 ft. high and on a child size shelf in space 3, within a child’s reach. We discussed items that may harm children and/or have “keep out of reach of children” on the label must be out of reach, locked and/or kept at least 5 ft. high from the floor height. *In space two (2) a staff’s handbag was in an unlocked closet, sitting on top of cots with the shoulder strap, hanging within a child’s reach. The cots were propped/leaning on the side, measuring 4 feet 5 inches tall. *In space 2, children ages 2 and 3 years old were present. Two-gallon size Ziploc bags were used to hold toys and were within a child’s reach. Felicia, relocated these safety items-correcting the violation. We discussed installing a shelf at least 5 ft. high in both classrooms so staff will have somewhere to keep their supplies and personal items. We discussed installing a shelf at least 5 ft. high in both classrooms so staff will have somewhere to keep their supplies and personal items.
