Name of Operation: GREEN ELEMENTARY Facility ID: 92003887 Consultant: KIMBERLY GIROUARD Operation Type: Center Case Number: Visit Date: 10/29/2025 Number Present: 12 Completed Date: 10/29/2025 Age: From 4 To 5 Total Minutes: 166 Time In: 09:44 AM Time Out: 12:30 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor compliance during your annual compliance visit. Upon my arrival to the one Pre-K classroom, I introduced myself to the Pre-K staff and was informed the lead teacher had an emergency this morning and had recently had to leave for the day. When I entered the Pre-K classroom, I was greeted by Ms. M. Smith and Ms. B. Harrison. Ms. Smith informed me she was assisting in the classroom due to the lead teacher having to unexpectedly leave for the day. Ms. Smith is an Instructional Assistant at Green Elementary and stepped into the class to assist so Ms. Mock could leave. Principal of Green Elementary, Ms. Blake, entered the room right after I arrived and I informed her that the two Assistant Teachers were in ratio and Ms. Smith did not need to remain in the classroom. I also reminded the Principal to always confirm any staff that assist in the licensed Pre-K class have the correct criminal background check in place before doing so. When I arrived today there were 12 children ages 4 and 5 years old who were finishing their breakfast. The staff were moving about the tables assisting the children as needed. After breakfast the children washed their hands and went over to the carpet for circle time. It was a rainy day, so Ms. Elkins and Ms. Harrison led a wonderful interactive circle time that all the children participated in and they played a dancing game with the children that they all enjoyed. They led a great discussion about animals and encouraged all the children to participate. With the unexpected absence of the lead teacher and it being a rainy day today, Ms. Elkins and Ms. Harrison did a fabulous job keeping the normal schedule for the children and keeping the children engaged and on task with the planned activities. The classroom atmosphere was age appropriate, positive and engaging. All spaces used by children were monitored. The classroom was organized and nicely equipped with age-appropriate materials. After circle time, the children participated in center activities. I could not find attendance records in the classroom today. I was informed by the classroom teachers that the front office staff take attendance as the children arrive. I made copies of the required attendance forms and left 10 blank attendance sheets and explained to the teachers that attendance must be recorded every day the children are in care and to include the time they arrive to the classroom and the time they leave for the day. While monitoring the refrigerator that the children’s lunches are stored in, I observed there to be a lot of lunch boxes and the thermostat to be registering at 50 degrees opposed to the required 45 degrees or less. I suggested possibly getting another small size refrigerator to store the lunches in which may help with maintaining the required temperature. I was informed later that normally the lunches are taken out of the lunch boxes and placed in large plastic bags which helps in keeping the temperature at the required 45 degrees. CURRENT LICENSE STATUS: This program operates with a Five (5) Star Licensure that was issued June 10, 2023, earning 7 points in program standard, 3 points in staff education Component, and 1 quality point through enhanced space and enhanced ratios. INSPECTIONS: Sanitation and fire inspections were found to be current. Fire and Emergency drills are being conducted as required as well as outdoor playground inspections. The last annual compliance visit was conducted on 11/18/2024. All applicable childcare requirements were assessed during today’s visit. The facility does not provide transportation. Therefore, this requirement was not monitored. A checklist was used to note the requirements monitored today. PROGRAM RECORDS: Staff files and on-going training requirements were assessed. The teachers are working on completing the annual training hours and are on target to complete them within the required timeframe. Two children’s records were monitored today and found to be in compliance. Two violations were cited during today’s visit and must be corrected immediately.
