Elizabeth Lester, licensing consultant, was also present during today’s visit. Elizabeth will be the assigned consultant for this facility once the rated license has been issued. The temporary license is in effect until July 1, 2026. The facility is approved for 28 children aged 0-6 years old on first shift only. No cooking is allowed in the building. During the first monitoring visit 2/19/2026, nine (9) violations were cited. Seven (7) of the nine violations were corrected in a timely manner. Pending violations include: Staff orientation (x3) and medicals for children. (x 3) The director stated that one of the children that did not have a medical on file no longer attends the facility. Two other children still do not have a medical on file. One other child’s file was reviewed today and that child did not have a medical on file either. A total of 3 children did not have a medical on file. The director has emailed the family more than once and they plan to bring the completed medical form tomorrow 4/10/26. Orientation: Visit: https://www.swcdcinc.org/ Scroll down to see the two part orientation training that you can complete with your staff. I received the staff and training worksheets that were requested during the 2/19/2026 visit on 4/5/26. This was over a month after the visit was completed. Several emails were sent to the provider with sample corrective action letters, request for the staff and training worksheets and to request an update on the status of the pending violations. After reviewing the staff and training worksheets, I noticed that there was information missing. I reviewed all staff files again during today’s visit to ensure all required information was on file. I added the missing information on the forms after reviewing the staff and training worksheet. Two staff members did not have a date documented for the medical. Four staff members did not have a date for the emergency information for staff. A date was not listed for first aid training for all 4 staff members. This information was updated during the visit. Monica was the only staff member that had Playground Safety training so she should be completing the monthly outdoor inspections. One staff member who was working in the classroom for infants did not have a date entered for ITS-SIDS training. The staff member that had completed an ITS-SIDS training for one hour completed the 2-hour training as required. The other teacher in the classroom for infants should have completed ITS-SIDS by March 1, 2026. (violation cited)1831 When we arrived this morning, Z. Franco was outside with infants and A. Alvarez was outside with the preschool age children. Since Z. Franco works with infants consistently she must have ITS-SIDS training. Safe sleep position checks were not documented since 2/19/2026. The sleeping position for infants under one year old, must be documented every 15 minutes. A notebook with safe sleep position check documentation was located on the shelf in space 2 but it was not current. The teacher stated that she checks the children but did not document the information. The checks were organized by the child's name. It is very important to document this information and to confirm that infants are safe while they sleep. If anything happens while an infant is sleeping and you don't have proof that sleep position checks were completed, you may have legal issues. Two soft books were removed from a crib today. Nothing should be in a crib while infants are sleeping. The infant in the crib was not sleeping while the books were in the crib so a violation was not cited. Ana needs to update the Recognizing and Responding to Child Maltreatment training. This should have been completed within 90 days of licensure. (by April 1, 2026) It was more than a year old when she was employed. (violation cited) 1897 https://positivechildhoodalliancenc.org/online-trainings/ The prevention of Shaken baby syndrome policy should be dated January 1, 2026 for all staff since that was the date all staff started working at the center. If you entered the date the DCDEE Moodle training was completed, that date should be removed. The operational policies and the parent participation plan should have been reviewed with staff during orientation. (#27 on the staff and training worksheets)You stated you would add the dates once the orientation forms are completed and show the full 16 hours with all applicable topics covered. I confirmed that the temporary license, NC Summary of the Law, safe arrival and departure procedures, emergency telephone numbers, and the emergency medical care plan were posted. Children were observed playing outside, washing hands, using the restroom, having diapers changed, eating lunch and napping. Teachers were observed providing supervision, interacting with the children and soothing children that needed a little extra attention. Both classrooms had a current daily schedule and a current activity plan posted. I reviewed four staff files today. Keep an electronic copy of the staff and training worksheets current so they are always ready to print or share with a consultant if needed. Fire drills: Completed as required. The time to evacuate the building should be 2 minutes or less. The times documented so far were 3-6 minutes. Practice to shorten the time it takes to evacuate the building. Evacuation routes were posted. Outdoor inspections: Current Incident reports: Current Attendance: Current on a clip board for each classroom. ABCMS: I will email instructions for you to set up the account. This should be completed before the expiration date of the temporary license. Lead water testing should be updated before October 2026. It is currently under the previous facility name. Once you create an account and enter the address, you should be able to update the facility information. All staff completed the training except one that had completed it more than a year ago before being hired. .1102(g) The violations cited today must be corrected immediately. Email me a corrective action letter that addresses the violation, how the violation was corrected, the date the violation was corrected and how you plan to prevent repeat violations in the future. I should receive the letter within 2 weeks of today’s visit. No extension will be approved so they need to be corrected in a timely manner. Two teachers were present with the children in space two. I verified required postings. The temporary license, NC Summary of the law, safe arrival and departure procedures, emergency telephone numbers, and first aid reference chart were posted. I monitored two classrooms for staff/child ratios, group sizes, capacity, supervision, discipline, interactions, materials, activities, postings, and health and safety. Space 2-Classroom for infants/toddlers (children under one year old and one year old children only): If infants under one year old are present, children that are two years old may not be present. Only infants and one year old children can be grouped together. Children that are two years old must be cared for in space 1. If no infants are present, the two year old children can be grouped with the one-year-old children. During arrival and departure times, (the first and last hour of the day) all ages can be grouped together as long as the ratio for the youngest child in the group is maintained and the group size for the youngest child in the group is maintained. If infants are present, the group size is 10. If one year old children are present and no infants are present, the group size is 12. Children were observed exploring Montessori materials and having circle time with the teacher. Teachers were observed interacting with the children as they explored materials. The younger children had circle time singing songs with the teachers. Younger children had diaper changes and naps during the visit. Younger children in space 2 were eating snack in low chairs when I arrived. The children had their hands washed and the teachers washed their hands. Safe sleep checks were completed for 4 infants under age one. The curtains were open during the visit. The rooms were bright and open. Feeding schedules were posted for children under 15 months old. A crib chart was posted. Bottles were labeled and dated. I confirmed that 5 diaper creams, one bottle of liquid Tylenol and a saline nasal spray had appropriate permission from parents. None of the children had emergency medications for chronic illnesses. A current daily schedule was posted along with an activity plan that was current. Be sure to date the activity plans when you post them. Staff/child ratios were posted in the classroom. Space 1-Classroom for preschool age children ages 2-6 years old: Staff/child ratios for the youngest child in the group must be maintained from open to close every day. I reviewed five (5) children’s records today. I completed a children’s records checklist to show files reviewed. Children were missing information. Violations were cited based on the information that was missing. Review the checklist to make corrections. I reviewed four staff files today. A staff and training worksheet was not completed during the visit. Complete the staff and training worksheets and email them to me once completed. Violations were cited regarding orientation, and ITS-SIDS training. I will email training resources for you to use for ITS-SIDS trainings. One of the teachers in space 2 had a one-hour safe sleep training but it was not the two hour required ITS-SIDS training that is required within 60 days of employment. A fire drill and an outdoor inspection were completed as required. A fire drill and outdoor inspection must be completed this month before the end of the month to remain in compliance. Attendance records were current as of today. Arrival and departure times were documented for each child. Only one incident has occurred since the temporary license was issued. One child did not have the information on file. .0608(b)(1-6) Technical assistance: Orientation: Staff qualifications and training requirements are essential for adults who interact with and teach children. These requirements protect children in childcare facilities by ensuring these facilities provide a physically safe and healthy environment where the developmental needs of children are being met and where children are cared for by qualified staff. Each staff person shall receive 16 hours of orientation within the first 6 weeks of employment. Six of the hours are required to be completed within the first two weeks of employment. When documenting orientation, please ensure that all required information is entered onto the form. You can conduct orientation training as a group with new staff instead of one on one. Ensure you and applicable staff members reflect the date, hours, and signature on the orientation training form. We referenced adding any additional information to the bottom of the orientation form that is reviewed with the employees. Medical Exams - A medical exam is required to be on file within 30 days of enrollment and must have been completed within one year of the date of enrollment. This ensures you have all of the information you need to provide appropriate care for each child and how to proceed with medical attention if the need arises. Children’s files: Record keeping is one of the most time-consuming tasks associated with administering a childcare program. It is an important part of licensure. Review children’s files often to ensure the program is maintaining accurate licensing records. Organization and completeness of records is important to ensure easy access to information in emergency situations. We discussed making sure that you develop a record keeping system for children’s records that promotes organization and efficiency. Administrative staff must allow time to review all necessary documentation prior to children’s start date to confirm all required documentation is complete before the child is left at the childcare facility. As an administrator, it is your responsibility to review all employment documentation and verify that required documents are received within the specified time frame. Using a checklist to check and date receipt of documents will help you track employment documents. A children’s file checklist is available on the DCDEE website under the Provider tab and provider documents links. The Emergency Preparedness and Response training is required within one year of licensure. It is best practice to complete the training as soon as possible. You can contact Lisa Terry for EPR training 919-795-3179. Once the training has been completed, the EPR plan must be created within 4 months of training completion. The plan must be created on the NC Risk Management portal website and the plan must be updated annually or it will archive. A temporary license was issued effective January 1, 2026, through July 1, 2026, for 28 children aged 0-6 on first shift only. Upon arriving, I was greeted by the teachers in space two. Two teachers were present with the children in space two. I verified required postings. The temporary license, NC Summary of the law, safe arrival and departure procedures, emergency telephone numbers, and first aid reference chart were posted. I monitored two classrooms for staff/child ratios, group sizes, capacity, supervision, discipline, interactions, materials, activities, postings, and health and safety. Space 2-Classroom for infants/toddlers (children under one year old and one year old children only): If infants under one year old are present, children that are two years old may not be present. Only infants and one year old children can be grouped together. Children that are two years old must be cared for in space 1. If no infants are present, the two year old children can be grouped with the one-year-old children. During arrival and departure times, (the first and last hour of the day) all ages can be grouped together as long as the ratio for the youngest child in the group is maintained and the group size for the youngest child in the group is maintained. If infants are present, the group size is 10. If one year old children are present and no infants are present, the group size is 12. Children were observed exploring Montessori materials and having circle time with the teacher. Teachers were observed interacting with the children as they explored materials. The younger children had circle time singing songs with the teachers. Younger children had diaper changes and naps during the visit. Younger children in space 2 were eating snack in low chairs when I arrived. The children had their hands washed and the teachers washed their hands. Safe sleep checks were completed for 4 infants under age one. The curtains were open during the visit. The rooms were bright and open. Feeding schedules were posted for children under 15 months old. A crib chart was posted. Bottles were labeled and dated. I confirmed that 5 diaper creams, one bottle of liquid Tylenol and a saline nasal spray had appropriate permission from parents. None of the children had emergency medications for chronic illnesses. A current daily schedule was posted along with an activity plan that was current. Be sure to date the activity plans when you post them. Staff/child ratios were posted in the classroom. Space 1-Classroom for preschool age children ages 2-6 years old: Staff/child ratios for the youngest child in the group must be maintained from open to close every day. I reviewed five (5) children’s records today. I completed a children’s records checklist to show files reviewed. Children were missing information. Violations were cited based on the information that was missing. Review the checklist to make corrections. I reviewed four staff files today. A staff and training worksheet was not completed during the visit. Complete the staff and training worksheets and email them to me once completed. Violations were cited regarding orientation, and ITS-SIDS training. I will email training resources for you to use for ITS-SIDS trainings. One of the teachers in space 2 had a one-hour safe sleep training but it was not the two hour required ITS-SIDS training that is required within 60 days of employment. A fire drill and an outdoor inspection were completed as required. A fire drill and outdoor inspection must be completed this month before the end of the month to remain in compliance. Attendance records were current as of today. Arrival and departure times were documented for each child. Only one incident has occurred since the temporary license was issued. Each staff person shall receive 16 hours of orientation within the first 6 weeks of employment. Six of the hours are required to be completed within the first two weeks of employment. When documenting orientation, please ensure that all required information is entered onto the form. You can conduct orientation training as a group with new staff instead of one on one. Ensure you and applicable staff members reflect the date, hours, and signature on the orientation training form. We referenced adding any additional information to the bottom of the orientation form that is reviewed with the employees. Medical Exams - A medical exam is required to be on file within 30 days of enrollment and must have been completed within one year of the date of enrollment. Children’s files: Record keeping is one of the most time-consuming tasks associated with administering a childcare program. It is an important part of licensure. Review children’s files often to ensure the program is maintaining accurate licensing records. Organization and completeness of records is important to ensure easy access to information in emergency situations. We discussed making sure that you develop a record keeping system for children’s records that promotes organization and efficiency. Administrative staff must allow time to review all necessary documentation prior to children’s start date to confirm all required documentation is complete before the child is left at the childcare facility. As an administrator, it is your responsibility to review all employment documentation and verify that required documents are received within the specified time frame. Using a checklist to check and date receipt of documents will help you track employment documents. A children’s file checklist is available on the DCDEE website under the Provider tab and provider documents links. The Emergency Preparedness and Response training is required within one year of licensure. It is best practice to complete the training as soon as possible.
