The center currently operates with a 5 STAR rated license. The center is approved for 123 children ages 2-8. My visit was conducted with Rebecca Smith, Administrator. During today’s visit an assessment of Child Care Requirements was conducted, inspecting all space used by children. During today’s visit I verified on the NC Secretary of State website that the owner of the facility, Frankie Lemmon School & Development Center, Inc. is listed as current-active as of 10-15-25. Upon arrival to the program today, the children were observed participating in morning activities. Daily arrival and departure times are documented on an electronic system at the front of the center. I monitored all space used by the children during my visit. Staff were observed engaged with the children and interacting in a warm and nurturing manner. Three staff are typically in each classroom to offer an enhanced learning experience and to assist with coverage of the classrooms. Staff and training worksheets were reviewed during my visit today. All new staff files were reviewed for compliance. We did discuss that there were some staff who serve in front desk, cook, floater and part time positions that are not listed on the worksheets. Please remember that all staff who are in attendance in the center are required to be listed on the Staff and Training worksheets. I reviewed the staff roster in the ABCMS portal and observed that there was one staff member at the facility that had an expired CBC qualification letter. We discussed that this staff member is retiring on 10-31-25 but a renewal background check would need to be completed within 2 weeks from today’s visit and if one cannot be obtained, the staff member will not be allowed to return back to work until one has been obtained. We discussed that the staff roster in ABCMS has some employees who are no longer employed with the facility and will need to be removed from the roster. There are also two staff who have current CBC qualification letters but they have not been updated in the ABCMS portal for the facility. Please have the staff go into their accounts and link their new qualification letters to the facility. A spot check was conducted on four children’s files and all were observed to be in compliance. I inspected the facilities posted information such as the license, EMC plan, emergency telephone numbers, staff/ratio sheets, allergy lists, etc. Fire drills, EPR drills and outdoor playground inspections were monitored and observed to be in compliance. The last fire drill was conducted on 9-25-25 and the last EPR drill was conducted on 7-25. Don’t forget to complete another fire and EPR drill by 10-31-25! The last outdoor playground inspection was conducted 9-25-25. The most recent fire inspection was conducted on 10-13-25. The most recent Sanitation inspection was conducted on 4-21-25. The center is not approved for transportation therefore those requirements were not monitored. *****Violations: One violation regarding criminal background records was observed out of compliance and must be corrected no later than 10-29-25. Please send a correction letter stating how the violation was corrected to me at the address at the bottom of this report. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). One staff member working at the center had a CBC qualification letter that expired on 9-15-25. G.S. 110-90.2(b) & .2703(n)&(o) Rated license information was also reviewed during the visit today. The facility submitted rated license paperwork on 10-2-25 and has applied for a 5 STAR rated license through Pathway #3 and the details are below. *Frankie Lemmon School & Development Center achieved NAEYC accreditation on 7-9-25. The accreditation expires 9-1-30. *Staff/Child Ratios – The facility operates using Accreditation Staff/Child Ratios as below: 2 years old – 1:6 – Max. group size 12 3 years old – 1:10 – Max. group size 20 4 years old – 1:10 – Max. group size 20 5-6 years old – 1:12 – Max. group size 24 6 + years old – 1:15 – Max. group size 30 It appears the facility is qualifies for a 5 STAR rated license. Paperwork will be submitted to DCDEE to be processed and a new license will be issued once the violation from today’s visit is corrected. Please feel free to call me with questions.
