I completed a walk-through of the facility. At the time of the visit, there were 70 children present. The children were observed playing in center, enjoying free play activities, playing with housekeeping material, manipulating Play-Doh, decorating a mitten worksheet, playing pretend hokey and pretending to ice skate on white paper. The infants were observed crawling, playing with toys and enjoying tummy time. LICENSE STATUS Currently this center operates with a four (4) star license issued 9/1/2020. INSPECTIONS I reviewed all required inspections including fire drills, emergency drills, fire inspection, sanitation, and outdoor inspections. MONITORING During today’s visit, a partial assessment was conducted, monitoring all space occupied by the children. All spaces were monitored for materials, equipment and required postings. I reviewed seven new staff file and discussed required trainings. The following violations were documented; however they were corrected during today’s visit so the letter of compliance is not required. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. Five classrooms did not have current lesson plan posted. GS 110-91(12); .0508(a) 847 Parent's medication authorization did not include required information. One child's EPI-pen was missing the permission form. 10A NCAC 09 .0803(4)(6-9) TECHNICAL ASSISTANCE/CONSULTATION: **We discussed at length your plans for your curriculum. You stated that you are in the process of switching to Creative Curriculum’s full version. This has been a struggle in the past and you are working with the staff to get them trained on the new curriculum. Five classrooms did not have the current lesson plan posted. Mrs. Taft was able to access, print, and the current lesson plans before the end of my visit. **One child did not have permission to administer an EPI-Pen. Mrs. Vess was able to contact the child’s parents and they sent over the signed permission form. >>Please fix the chipped paint in the ones-D105 classroom. RATED LICENSE Hold harmless has been extended until the new QRIS (STAR rating) rules are implemented (SB 425). Providers in Cohort 1 are not required to continue their with a rated license assessment unless you want to, and providers in Cohort 2 do not need to start their preparation year assessment unless you want to. Prep year scales and assessment year scales are optional and up to your discretion at this time. Please continue to work on preparing and ensuring staff education is current in WORKS. A full assessment of child care requirements was conducted. Your program currently operates with a Four-star license. The facility was monitored for approved curriculum. Today’s visit was conducted with you both, Valerie Vess and Peggy Taft, Co-Directors. You visited each child care area with me. I monitored classrooms in use for compliance with staff/child ratios, supervision, capacity, group size and staff interaction/discipline. Children throughout the facility were playing in activity areas, completing group art activities, listening to a book being read, transitioning outside and toileting, and eating lunch. Staff assisted children with routines, transitions, conflict resolution, and meeting children’s needs. Younger children were resting, having floor play time, and eating. Staff held children, assisted them with eating, and play. The facility was monitored for health and safety requirements both indoors and outdoors. Program requirements, along with equipment and furnishings were monitored. All inspections and drills were current. A sampling of children’s and staff’s records including all new staff information, was monitored and information documented on the children’s records form and staff and training worksheet. A checklist was also used to monitor compliance. The last annual compliance visit was conducted 7/12/2023. The owner/operator is listed as current/active with the NC Secretary of State. Violation Number Comment Rule 1893 Distance learning was used to complete First Aid, CPR and/or playground safety training. One staff member completed CPR and First Aid training online. 10A NCAC 09 .1102(c-e); 10A NCAC 09 .1105(3) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Being able to respond responsibly is crucial and can be the difference between life and death. The staff member came with CPR and FA training. Today you stated you did not realize the staff member could not complete CPR and First Aid online. You contacted the CPR and FA trainer to schedule a training. You also talked about getting additional staff enrolled in the training since they may be due within the next six months. The EPR plan has been updated but is in Draft form. You will need to republish the plan to make it active. Lead Free Classrooms - It appears you have started the registration for the lead in paint and asbestos check. Be sure to follow the steps on the cleanwaterforuskids.org site to complete the process within the required timeframe. OUTDOOR SPACE: Today I measured a new outdoor space for infants. The space calculations, and diagram will be sent to you. A full assessment of child care requirements was monitored. Your program currently operates with a Four-star license. Your compliance history is 93 percent. Today’s visit was conducted with you, Valerie Vess, Center Co-Director. You visited each child care area with me. I monitored all classrooms in use for compliance with staff/child ratios, supervision, capacity, group size and staff interaction/discipline. Children were participating in water day, group activities, free play, resting, eating lunch, transitioning, and playing outside. Staff assisted children with materials and other needs. The last annual compliance visit was conducted 7/13/22. Information regarding compliance was documented on a checklist. The facility was monitored for health and safety requirements both indoors and outdoors. The program requirements, along with equipment and furnishings were also observed. A sampling of children and staff’s records and all new staff files were monitored. Information was documented on the staff training worksheet and children’s records form. The Owner is listed as active/current with the NC Secretary of State as of 7/12/2023. Violation Number Comment Rule 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. A staff member wrapped a child in their blanket, placed the child on their stomach while placing her hand on the child's head and rubbing their back in an effort to get the child to go to sleep. The child was crying loudly because of the staff member's actions. G.S. 110-91(10) All violations must be corrected immediately. A corrective action letter explaining how each violation was corrected, must be sent to me no later than July 26, 2023. Documentation observed today and information received, documented a named staff member wrapped a child snuggly in their blanket, placed the child on their stomach with the staff member’s hand on the child’s head while rubbing the child’s back as a method to get the child to go to sleep. It was documented in your records and reported that the child was crying loudly and wanted to get up from the position. You documented another concerned staff member reported the incident. You and your Co-Director held a meeting with both staff members to discuss the incident and other concerns. You explained the named staff member acknowledged another specified concern that was also discussed with future consequences. You and your Co-Director have observed in the classroom, discussed with the named staff member how to handle frustrations, and ask for support in the classroom. You also explained, the named staff member will be required to complete a specific requirement within the coming days and to report back to you. You also stated you will identify specific trainings the named staff member will be required to complete regarding, classroom management, discipline, and staff/child interactions. You stated you will develop a plan to monitor the staff member’s progress. Today you stated you have observed the staff member has used the suggestions you have given and you have seen a positive change. Today I observed the staff member call for support while in the classroom. TECHNICAL ASSISTANCE: Nurture/Care – G.S. 110-91(10) requires that children be attended to in a nurturing and appropriate manner and with keeping with their developmental needs. You may contact Child Care Services Association or Wake County Smart Start to see what programs they have available that may assist the staff member. They may also have training opportunities for the staff member to participate in such as thee NCB3 Birth to Three Quality Initiative, Infant Toddler Teacher Orientation that offers 14 credit hours through CCSA.
