A full assessment of child care requirements was conducted. Your program currently operates with a Five-star license. Today’s visit was conducted with you, Emily Stokes, Center Director. Currently there is only one group of children. Children were playing with materials, moving freely about the indoor space, having diapering done, and interacting with each other. Staff assisted children with developmental needs. I observed proper diapering and handwashing procedures. I monitored the classroom in use for compliance with staff/child ratios, supervision, capacity, group size and staff interaction/discipline. The facility was monitored for health and safety requirements both indoors and outdoors. Program requirements, along with equipment and furnishings complied. All inspections and drills were monitored. A sampling of children’s and staff’s records was monitored and one violation regarding staff’s record was documented. A checklist was also used to monitor compliance. The last annual compliance visit was conducted 2/20/2024. You do not provide transportation. The owner/operator is listed as current/active with the NC Secretary of State. Violation Number Comment Rule 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. One staff member's documentation of new staff orientation did not indicate they completed 16 hours of orientation. .1101(a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before 2/4/2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 Today I received a letter requesting your facility transition to a mixed-age group staff/child ratio. We discussed based on your license, your staff/child ratio mixed age group will be as follows: Ages 0 – 36 months 1/4 staff-child ratio with a maximum group size of 8 children. You explained you currently only have 8 children enrolled. The ratios are effective today and must be maintained at all times unless you submit another letter stating you no longer want to meet the ratios. Today I observed small pinecones on the playground. You may want to rake the area or have maintenance rake the area. The handwritten visit summary indicated a violation regarding training would be cited. However, after the form was written, we observed documentation of the staff member's training. This violation was not cited. Updates/Resources: Make sure you are using the most recent child care requirements dated November 2024. You may access the newest version on DCDEE website. Hold harmless: Hold harmless has been extended until the new QRIS is implemented (SB 425). This means that you do not need to complete the rated license process unless you voluntarily request to.
