Name of Operation: EDUCATIONAL JOURNEYS Facility ID: 92003369 Consultant: ANTIONETTE WILLIAMS Operation Type: Center Case Number: Visit Date: 9/26/2024 Number Present: 54 Completed Date: 9/26/2024 Age: From 1 To 4 Total Minutes: 95 Time In: 12:20 PM Time Out: 01:55 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor compliance with child care requirements for a routine unannounced assessment. The last annual compliance visit was conducted on 4/16/24. You, April Keen, Center Director, assisted me today. Your program currently operates with a One-Star license. You accompanied me as I walked through the facility today. The licensed portion of the program starts at 12 noon daily. Children throughout the facility were eating lunch, cleaning up after themselves, completing toileting activities, preparing for rest time, napping, and having quiet activities. One group of children listened to a book being read while others napped. Staff assisted children with transitions, toileting, cleanup, and rest time when needed. Staff also talked with children about interactions. Supervision, group size, permit capacity, staff/child ratios and space capacity complied. Two new staff’s files were monitored. Other staff’s qualifying letter and special training complied. Staff’s CPR and first aid certifications were current. Children’s records were not monitored. However, one violation related to one child’s action plan was cited. Program records complied. Fire and sanitation inspections were current. Emergency medical care plan was accessible. ADDITIONAL REQUIREMENTS: General health requirements complied. Medications were stored properly. The facility continues to meet requirements for adequate and approved space. The NC Secretary of State website was reviewed the Owner was listed as current- active. Violation Number Comment Rule 1835 The medical action plan was not updated on an annual basis or when changes to the plan were made by the child's parent or health care professional. The medical action plan for one child expired on 7/17/2024. .0801(b) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before 10/10/2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 TECHNICAL ASSISTANCE: Medical Action Plans - Having a medical action plan in place when caring for children with health issues is vital in ensuring their health and safety. The maintenance of these plans and the ability to implement such plans for the treatment of reactions are essential. It is important to remember that a medical action plan must be attached to the child’s application for enrollment, completed by the child's parent or a health care professional and must be updated on an annual basis. Today you explained the one child whose action plan was older than twelve months was an oversight by the parent. We discussed the parent can complete and sign an updated action plan. I explained a form may be downloaded from the provider documents portal on DCDEE’s website. You stated you will have the parent complete the form. You also explained the Child Care Health Consultant will visit next week to review the medications to ensure the correct information is on file. CONSULTATION: Today we discussed I will need to obtain additional information regarding when a copy of a child’s prescription box with label is used instead of the actual box the medication came in. Today I observed a copy of a box and label for an Epi-Pen. We will need to determine how to ensure the copy of the box and label matches the medication. Lead Free Classrooms: It appears you have completed the lead-free water, lead in paint, and asbestos testing. The visit summary was reviewed and left with you today.
