The facilities’ last annual compliance visit was conducted on May 14th, 2024. A checklist was used to note the requirements I monitored today. The center currently operates with a 3- star license, operating at 30sq. ft. inside, & enhanced ratios. My visit was conducted with you, Britney Fisher, Administrative Assistant, and Tyneita Blount, owner and center staff. The children were observed participating in napping and tummy time in the infant room, circle time, and the beginning of lunch. Lunch was in compliance with USDA guidelines for children in childcare. During the walkthrough to obtain enrollment and conduct observations. I monitored required posted documents (license, EMC, sanitation, no smoking, first aid signage, program records) interactions between staff and children, discipline, supervision, indoor and outdoor environment, group sizes, space capacity, program requirements, toys, materials, equipment and furnishings. VERIFICATION OF RECORDS: *Last fire inspection= 4/16/2025 and approved for daytime care only *Sanitation inspection= 10/8/2025- 4 demerits superior rating. *Fire Drill- 9/25/2025 *Playground inspection- 10/15/2024 *Lockdown- 9/17/2025 * EPR plan last updated 5/14/2025 • The following violation was cited during today’s visit and requires a letter of corrective action. Violation Number Comment Rule 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The ABCMS criminal record portal requirement was not completed. My visit was conducted with you, Tyneita Blount, owner and center staff Kimberly Gilliam, Director arrived at the conclusion of my visit. The children were observed participating in structured tabletop activities as documented on the activity schedule, outdoor play, lunch and going down for nap at the conclusion of the visit. Lunch was in compliance with USDA guidelines for children in childcare. During the walkthrough to obtain enrollment and conduct our observations. We monitored required posted documents (license, EMC, sanitation, no smoking, first aid signage) interactions between staff and children, discipline, supervision, indoor and outdoor environment, group sizes, space capacity, program requirements, toys, materials, equipment and furnishings. VERIFICATION OF RECORDS: *Last fire inspection= 4/17/2024 and approved for daytime care only- during today’s visit a phone call was made to your newly assigned fire inspector following your completion of your online appointment request. Violation Number Comment Rule 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. Three staff had health questionnaire's that were completed in 2023 and not updated prior to its expiration. .0701(a) 1792 Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. During the walkthrough in the space for enrolled two year old's we observed a cup of hot tea with a cover and Fritos that were accessible to the enrolled children. .0901(i) 1831 At least one child care provider, who has completed ITS-SIDS training was not present in the infant room, while children were in care. A new employee assigned to work in the infant room had not completed training yet and was working alone in the classroom, not working along side a staff person with ITS-SIDS training until it is obtained. .01102 (f) *****TECHNICAL ASSISTANCE/REMINDERS: ******* Violations of childcare requirements were cited during your visit and a letter of corrective action is required by November 24th, 2024. A letter of corrective action with your facility name, ID #, item # how you corrected the violation and how you will keep in compliance going forward. • Activity schedules and outside time should be followed and utilize the weather chart to determine if modifications need to be made to the times outside. Cold weather is coming. • Health questionnaires should be completed prior to the date it was completed the year before. • Orientation forms should not be signed and dated until all orientation topics are completed and all sections of the form are signed off first. • CBC Provider Portal Technical Assistance & Notification to the Division of New Hires or Residents Technical Assistance Guidance As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. This facility currently operates with a Three (3) Star License that was issued on 4-6-23. The last sanitation inspection was completed on 10-5-23 with an “Superior” classification. The last approved fire inspection was conducted on 4-17-24. The NC Secretary of State website was reviewed today and this facility’s corporation’s name, Early Stages Child Care Center, LLC was listed as “Current-Active.” Today’s visit was conducted with you, Kimberly Gilliam, center director. I monitored the four (4) classrooms in use for compliance with staff/child ratios, supervision, and staff interaction/discipline. The program was also monitored for health and safety requirements both indoors and outdoors using the annual compliance monitoring checklist. Children completed daily activities with adequate supervision. The children were observed interacting in activity centers, singing and dancing to music, at the table completing an art activity drawing and coloring the habitat of their favorite animal, and sitting at the table while the caregiver read books to the children. Appropriate handwashing and diapering routines were also observed. The program requirements, along with equipment and furnishings were observed. Compliance with staff records was documented by you. New staff records, along with children’s records were reviewed today. Medication observed on the premises was monitored. This facility does not provide transportation. The following violations of child care requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance was provided. Whether a child needs medication to manage asthma, allergies, seizures, diabetic care, or other conditions, you want to make sure that the medication is given safely and correctly. No medication can be left at the center if the permission slip has not been properly completed and signed by the parent. In addition, documentation of administration of the medication is just as important. Clear and concise administration of medication documentation is critical to providing children with quality care, ensuring accurate and timely administration, along with mitigating risks, all help with the child’s treatment and to maintain the continuum of the child’s care. As an administrator you must allow time to review all necessary documentation to ensure accuracy. It was suggested to set an alarm weekly to conduct a review of medications and medication permission slips. • A suggestion was made to save the most recent child care requirements in Chapter 9 effective January 2024, to your desktop computer for easy access. • Center directors can now access the Automated Background Check Management System (ABCMS), the DCDEE’s criminal background check system. This access will allow providers to: • See the real-time background check status of staff members. • Run a printable report of the staff roster to assist with compliance visits. • See new background check applicants and add to staff roster. Access to the ABCMS requires the successful completion of a Moodle course consisting of a short video followed by a test. The course is titled ABCMS Child Care Provider Portal Training - under the Early Childhood Professional Development Tab, then under the Criminal Background (CBC) tab. Today’s visit was conducted with you, Ms. Tyneita Blount. This facility currently operates with a Three Star License that was issued on 4-6-23. Upon arrival to the facility, I was greeted by you, and the complaint allegations regarding staff/child ratios and staff files were shared with you. In regard to the allegation concerning staff/child ratios, it was alleged that there are five (5) one year olds and an eight (8) month old in care with one (1) caregiver. You stated that staff/child ratios are maintained at all times. You added that caregivers are aware of the ratios and that if for some reason, there is a caregiver absent, children may be combined to ensure that the staff/child ratios and group sizes are met. Caregivers present were interviewed and indicated that staff/child ratios are maintained. Caregivers corroborated that children are moved around when needed to ensure that ratios are maintained as required. During today’s visit attendance records were observed and staff/child ratios were observed in compliance in all classrooms. Based on my observations and conversation with staff, the allegation regarding staff/child ratios is unsubstantiated. Regarding the allegation concerning staff records, it was alleged that only one caregiver has a file. When the allegation was shared with you, you stated that all staff have an employee file, along with a medical file. You provided eight (8) files for the currently employed staff. Based on my observations and conversation with staff, the allegation regarding staff records is unsubstantiated. The following violation of child care requirements was observed during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance was provided during today’s visit. Violation Number Comment Rule 124 The center did not maintain records as required in rule, and/or were not made available to the Division for review. A staff file record was available for each staff member, however all of the required paperwork for each staff file was not available. Staff development plans, health questionnaire, orientation, maltreatment training, CPR & First Aid training, shaken baby syndrome acknowledgement, etc. was paperwork missing from some staff files. G.S. 110-91(9); .0304(g); .2318 The above violation must be corrected immediately and a letter must be sent to me no later than 8-22-23 stating how the item was corrected and a plan to maintain compliance with the requirements. If I do not receive your letter by 8-22-23 and/or if you do not address the violation a return visit to your program may be made to monitor for compliance of the violation. Additional violations may be documented if the situation has not been corrected or addressed. The following were discussed with you during today’s visit: • Today time was spent discussing each staff file form and how to fill it out. Please use the staff file checklist provided today and reorganize your files asap. • Continue to use your Child Care Health Consultant, Maria Nelson, RN as a resource for health and safety practices and reach out to Child Care Services Association’s Infant/Toddler Specialist as a resource. • We discussed that new Sanitation rules went into effect on 7-1-23. Please familiarize yourself with these rules because your sanitation inspection will be due sometime this month. https://ehs.dph.ncdhhs.gov/hhccehb/cehu/ • The Healthy Social Behaviors (HSB) Project supports teachers to promote healthy social-emotional development and reduce the expulsion rate among young children in licensed child care centers across North Carolina. Specialists are available with early childhood education backgrounds who are passionate about empowering teachers to develop learning environments and teaching practices that promote prosocial skills in young children. A flyer with the challenging behaviors hotline information was provided today. • Criminal Record Qualifying Letters are now valid for 5 years and there is a new portal for Criminal Background Checks.
