Your program currently operates with a Two-Star license. Today’s visit was conducted with you, Ms. Cheyenne, new Center Director. Staff interaction/discipline, staff-child ratios, maximum group sizes, and supervision were verified in compliance. The facility uses the multi-age staff/child ratio. Upon arrival, children were outside having water play time. They used water buckets, splash pad with hose, and other water play toys. You transitioned the children indoors, assisted them with changing clothes, and getting dry. As children finished, they chose a book and read independently on the carpeted area. The other staff member started reading a book to the children. Once the children wanted to do other activities, they left the group and chose an activity they were interested in. Children played in activity areas including completing a floor puzzle and painting using red, white, and blue. A staff member assisted the child with completing the floor puzzle and other children when needed. The facility was monitored for health and safety requirements, program requirements, equipment, and furnishings. One existing and the one new staff member’s records were monitored. Children records were reviewed today, and information was documented on the children’s records form. One violation related to children’s records was documented. A checklist was also used to monitor compliance today. The last annual compliance visit was conducted on 7/17/25. Sanitation and fire inspections are current. The facility’s owner is listed as active/current with the NCSOS. Violation Number Comment Rule 1311 Emergency medical care information was not on file in the center on the child's first day of attendance and/or was not updated as changes occurred or at least annually for each child. A full assessment of child care requirements was conducted. Your program currently operates with a Two-Star license. Today’s visit was conducted with you, Cheyenne Hathaway, Acting Center Director. You accompanied me as I walked through the facility. I monitored the child care space for compliance with staff/child ratios, supervision, capacity, group size and staff interaction/discipline. There were three children present today. Children were playing with materials in various activity areas. A staff member walked around while interacting with children. They also participated in an art activity. The staff met each child’s developmental needs. The facility was monitored for health and safety requirements both indoors and outdoors. Program requirements, equipment, and furnishings were monitored. All inspections and drills were current. A sampling of children’s and staff records, including one new staff member, was monitored. A checklist was also used to monitor compliance. The owner/operator is listed as current/active with the NC Secretary of State. Violation Number Comment Rule 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. Documentation of visually checking on sleeping infants was only available for 7/11/2025 to 7/15/2025. .0606(g) 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. One staff person hired in March 2025 did not receive sixteen hours of orientation. .1101(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff member employed in March 2025 completed First Aid training from an unapproved training organization. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. Observation of infants while they are sleeping are critical to reduce the risk of Sudden Infant Death Syndrome in child care. Documentation of monitoring sleeping infants’ sleep position must be maintained for at least thirty days. You explained you have not completed the documentation prior to 7/11/2025. Staff Records - Record keeping is one of the most time-consuming tasks associated with administering a child care program. It is an important part of licensure. Review staff files often to ensure the program maintains accurate licensing records. Organization and completeness of records is important to ensure easy access to information in emergency situations. Using a checklist to check and date receipt of documents will help you track employment documents. A staff file checklist is available on the DCDEE website under the Provider tab and provider documents links. Please refer to the Staff and Training Worksheets to continue to keep all staff members current with their upcoming due dates for CPR/First Aid training, child maltreatment training, and health and safety training requirements. CONSULTATION: Menus – Today we discussed your menu and how it is organized. You may access a meal planning tool in the provider document portal on DCDEE website. The tool is very helpful. Cots – Today we discussed alternatives for storing cots including a nonporous barrier. HOW TO LINK QUALIFYING LETTERS IN THE ABCMS SYSTEM: Today we discussed you need to link you and any household member(s) immediately to your facility in the ABCMS. I reviewed the portal and observed you have not completed this process. This is a child care requirement that was effective in 2024. Center directors can now access the Automated Background Check Management System (ABCMS), the DCDEE’s criminal background check system. Access to the ABCMS requires the successful completion of a Moodle course consisting of a short video followed by a test. The course is titled ABCMS Child Care Provider Portal Training - under the Early Childhood Professional Development Tab, then under the Criminal Background (CBC) tab. A webinar will be scheduled later this month to share the implementation plan and will also provide resources that can assist you with understanding the new rules. Today's visit was completed with Kelly Hunter and on my own at times. Kelly assisted the lead teacher with the children during transitions. One teacher was present with three children ages 1-2. The children were observed having free play, exploring materials, having snack and preparing to go outside to play and playing outside. No infants are currently enrolled. The room was being rearranged when I arrived. The temporary license, NC Summary of the law, safe arrival and departure procedures, emergency medical care plan, first aid reference chart, daily schedule, activity plan and staff/child ratios were posted. The lead teacher stated that none of the children have allergies. I verified this based on documentation on the children's applications when files were reviewed. Two children's records were reviewed today. One of the files had a wellness check print out but not an actual medical completed by a doctor. One other child did not have a medical completed at all. Immunizations are totally separate from the medical. Both are required within 30 days of enrollment. It may be helpful to request the medical and immunizations on the first day so it is on file early. Attendance was current, fire drills were current and outdoor inspections were current. One staff file was reviewed during today's visit. The file contained the required information. Reminder to have the TB test and medical on file before an employee starts working. The health questionnaire was dated after the employee started working, make sure it is on file the first day of employment. Be sure that all employees have 16 hours of orientation. Each employee should have 6 hours of orientation during the fist 2 weeks of employment and the remaining 10 hours in the next 4 weeks. This allows time to be thorough so employees have time to process and learn procedures and information. A shade structure was added to the outdoor play area. Materials were stored under the shade structure. No concerns with the outdoor play area during today's visit. Materials were available for children to use while playing outside. Violation Number Comment Rule 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. Two children did not have medical on file within 30 days of enrollment. GS110-91(1) The violation cited today should be corrected immediately. Mail or email me a corrective action letter stating how the violation was corrected, when the violation was corrected and how you plan to prevent repeat violations in the future.
