    Name of Operation: DILLARD DRIVE ELEMENTARY Facility ID: 92003038 Consultant: ANTIONETTE WILLIAMS Operation Type: Center Case Number: Visit Date: 1/17/2025 Number Present: 16 Completed Date: 1/17/2025 Age: From 4 To 5 Total Minutes: 60 Time In: 10:30 AM Time Out: 11:30 AM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable child care requirements for an annual compliance visit, including compliance with requirements located in Child Care Rule Section .3000 in all spaces where children participating in the NC Pre-K program are cared for. Your program currently operates with a Five-star license. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Today’s visit was conducted with you, Amy Jordan, Lead Teacher. Ms. Revanol, a Representative from Wake County Pubic School central office, was also present. I monitored the one classroom in use for compliance with supervision, and staff interaction/discipline. The NC Pre-K requirements in section .3000 of the childcare rules were monitored for compliance. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. Children were playing outside when I arrived. Children transitioned inside to lunch time and group time afterwards. Proper hand washing, toileting, and nutrition practices were observed. The facility was monitored for health and safety requirements both indoors and outdoors. Program requirements, equipment, and furnishings were also observed. Compliance with staff records was documented by you. All new staff and a sampling of existing staff records and children’s records were reviewed today. One violation regarding children records was documented. Staff records were reviewed. A sampling of children’s records was reviewed and information documented on the children’s records form. A checklist was used to monitor compliance today. The last annual compliance visit was conducted 3/23/23. The sanitation and fire inspections were current. NC PRE-K REQUIREMENTS: NC Pre-K requirements were monitored which also included developmental screening, family engagement, children records, ongoing assessments, and parent/teacher conferences. Violation Number Comment Rule 1768 The health assessment did not include a hearing screening. The health assessment for one child did not include a hearing screening. .3005 (a)(4) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before 1/31/2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted.
