Name of Operation: CROSBY HEAD START Facility ID: 92001438 Consultant: ELIZABETH LESTER Operation Type: Center Case Number: 1025-254L Visit Date: 10/24/2025 Number Present: 43 Completed Date: 10/24/2025 Age: From 3 To 5 Total Minutes: 50 Time In: 09:55 AM Time Out: 10:45 AM Time In: Time Out: List to Use: Center Type Of Visit: Self Report Announced/Unannounced: Unannounced The purpose of today’s visit was to obtain information related to a self-report received by the Division on 10/17/2025. During today’s visit, the director Abagail Moore, assisted me with the visit. During the visit, I discussed the information with Ms. Moore. Based on information obtained, the following was determined. On 10/16/2025, a parent was dropping off their child and witnessed a teacher redirecting a child in an aggressive manner, by grabbing the child by the arm on two occasions. Prior to the visit, Ms. Moore reported the incident to me via email, and I advised her to follow up with the DCDEE intake reporting line to ensure it was received by our agency. The director stated the parents were informed of the incident and signed the incident report. The caregiver was terminated immediately due to the policies/expectations the operation has. Limited monitoring of child care requirements occurred during today’s visit. The following violation was confirmed during today’s visit: Violation Number Comment Rule 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs.a parent was dropping off their child and witnessed a teacher redirecting a child in an aggressive manner, by grabbing the child by the arm on two occasions. G.S. 110-91(10) You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited during today's visit may negatively impact your compliance score. RESPONSE In response to this incident, the director held a staff meeting, met with both families and placed the Educational and disabilities Specialist in the classroom during transitions for extra support. I reviewed the staff meeting minutes. The meeting was on 10/23/2025. They reviewed their appropriate conduct policy, their standards of conduct, their SpeakUp policy and their active supervision policy. The staff signed that they reviewed and understand the policies and procedures. Name of Operation: CROSBY HEAD START Facility ID: 92001438 Consultant: ELIZABETH LESTER Operation Type: Center Case Number: Visit Date: 10/14/2025 Number Present: 63 Completed Date: 10/14/2025 Age: From 3 To 5 Total Minutes: 110 Time In: 10:50 AM Time Out: 12:40 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor compliance during an annual compliance visit. Upon arrival I parked in the parking lot and was greeted by the security guard as he escorted me to the building. I entered the building and was greeted by the director Ms. Abigail Moore. I completed a walk-through of the center with the director. At the time of the visit, there were a total of 68 children present. The children were observed playing in centers, engaging in circle time, dancing, listening to a book and walking down the hall to the bathroom. LICENSE STATUS Currently this center operates with five star license issued June 26, 2019. They are license for three (3) to five (5) year olds. I reviewed the NC Secretary of State's website and observed that the owner is listed as current/active. This facility is a head start and NCpreK program and has four open classrooms. Three (3) classrooms are NCPreK and one is head start. INSPECTIONS I reviewed all required inspections including fire drills, emergency drills, sanitation, fire and outdoor inspections. Inspections were found to be in compliance. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. I reviewed a selection staff and children files. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. A checklist was used to note the requirements I monitored today. ANNUAL IN-SERVICE TRAININGS I discussed the center annual in-service training calendar and this center’s annual in-service calendar dates are August 1 through July 31st. One violation was cited but this violation was corrected during the visit: Violation Number Comment Rule 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. In two classrooms there was medication in an unlocked cabinet with emergency medications. 15A NCAC 18A .2820(d) COMPLIANCE HISTORY According to NC General Statute all Child Care programs must maintain a compliance history of at least seventy-five percent (75%) for the past 18 months or during the length of time the facility has operated, whichever is less. Any violations documented during visits to your program may have an impact on the total compliance history score and cause your score to drop below the mandated level. **Prior to today’s visit, the compliance history score for the center was 100%. Any violations cited during today's visit may affect this percentage. TECHNICAL ASSISTANCE **In two classroom there were non-prescription allergy medications with emergency medications such as epi-pens. All non-emergency medications must be locked the rule 10A NCAC 09 .1719 (5) states: “medications including prescription and non-prescription items shall be stored in a locked cabinet or other locked container. Designated emergency medications shall be stored out of reach of children at least five feet high, but are not required to be in locked storage. For purposes of this Rule, designated emergency medications are those that are used or needed for the immediate recovery from a life-threatening event and include Glucagon, epinephrine auto-injector, diazepam rectal installation and albuterol” QRIS We discussed the new QRIS pathways. You are currently NAEYC accredited, and a Head Start program and qualifies for pathway 3.
