    Name of Operation: CRE8V CHILDREN CHILD CARE Facility ID: 92003718 Consultant: KIMBERLY WIMBERLY Operation Type: Family CC Home Case Number: Visit Date: 7/2/2026 Number Present: 2 Completed Date: 7/2/2026 Age: From 5 To 7 Total Minutes: 130 Time In: 09:20 AM Time Out: 11:30 AM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Annual Comp w/Rated Lic Assess Announced/Unannounced: Announced The purpose of today’s announced visit was to monitor child care requirements during an annual compliance visit with a rated license assessment. Todays visit was conducted with Latishia Hodge, owner/operator. License Status: The FCCH currently operates with a 3 STAR rated license. The license and all required documents were observed posted and all restrictions were observed being maintained. All general licensing requirements were being maintained. Upon arrival today, there was one preschool age child and one school ager observed present. The children were playing with various toys in the child care area. Ms. Hodge had purchased some new board games and was playing with the children. Ms. Hodge was observed integrating colors and numbers into the conversations as she played the games with them. All hazardous items were observed locked up or placed out of reach of the children. The license and all required documents were observed posted. Supervision was observed being maintained. Provider and Program Records: CPR and First Aid were observed current and on file. A current criminal background qualification letter was on file for Ms. Hodge as well as her daughter who resides in the home as verified in the ABCMS database. Fire Drills, EPR Drills and Outdoor Playground Inspections were current and on file. The last fire drill was conducted on 6-18-26 and the last EPR drill was conducted on 5-18-26. Don’t forget that another EPR drill must be conducted by 7-31-26 in order to remain in compliance. On-going training was monitored during my visit today and I observed that the required training Recognizing & Responding to Child Maltreatment was expired. MS. Hodge stated she would complete the renewal training today and send a copy of the completion certificate. The FCCH operates on city water so no additional bacteriological testing is required. Negative lead water testing results were on file. Negative results for lead paint and asbestos were also current and on file. Ms. Hodge stated that she has decided not to provide care for infants anymore and we discussed that your license will be updated to reflect ages served are age one to age twelve. Ms. Hodge has a total of eight (8) dogs which reside in the home but are kept inaccessible to the children in the lower part of the home. Vaccination records were observed current and on file for all dogs. I continue to encourage you to have discussions with your insurance company to make sure that there are no restrictions to providing care for children in a home where eight dogs reside. Due to the breeds of the dogs, I also suggest that you continue to keep the dogs inaccessible to the children. Larger dogs require strong management and controlled environments to avoid any potential amplified risks in a child care setting. There is a fenced area in the back yard which was originally set up as an outside playground but due to recent easement encroachments, the City of Raleigh has taken over a portion of the back yard and Ms. Hodge has designated that area for the dogs and the children now play in the front yard. One violation of child care requirements regarding special training requirements was observed during the visit today and must be corrected. Proof of completion of the training must be completed and sent to me no later than July 16, 2026 and can be sent to the address at the end of this report. Violation Number Comment Rule 2019 Operator did not complete Recognizing and Responding to Suspicions of Child Maltreatment training within the required time frame. Ms. Hodge did not have a current Recognizing & Responding to Child Maltreatment completion certificate on file. .1703(a)(5) QRIS rated license paperwork was submitted to me via e-mail on 4-1-26 and was reviewed with Ms. Hodge during my visit today. Ms. Hodge has applied for a 5 STAR rated license through Pathway #2, Classroom & Instructional Quality. The results of the assessment are below. Family & Community Engagement Standards: Ms. Hodge meets all the minimum required standards as stated in Child Care Rule 10A NCAC 09 .3220 as well as at minimum three (3) categories of engagement as observed by consultant and discussed today. Ms. Hodge has a QRIS notebook which has documentation that she actually participates in all engagement standards. FCCH CQI Plan Ms. Hodge submitted her CQI plan on 4-1-26 which indicated her professional goal for the upcoming year is to rebrand and align with a nature and animal focused early learning model. Approved Curriculum – •Ms. Hodge has purchased and chosen to use the Funny Daffer curriculum. Formative Assessments: •Ms. Hodge has chosen to use the formative assessments which are a part of the Funny Daffer curriculum. These assessments will be shared with parents twice annually. Please be reminded that these observations must include evidence of the child’s activities. Coaching/Training Options: •Ms. Hodge will be participating in the additional of five (5) on-going training hours in addition to applicable requirements in Child Care Rule .1703(f). Education – •Ms. Hodge completed the NCECC in 2012 and has twenty (20) additional hours in early education as verified in WORKS database. Ms. Hodge has been a licensed FCCH since 2011. Enrollment: •Ms. Hodge will not enroll no more than four children under the age of 24 months to meet GS 110-91(7)(b)(1.) Based on the above information, it appears you qualify for a 5 STAR rated license. Congratulations! Paperwork will be submitted to DCDEE to be processed and a new license will be issued.
