Today’s visit was completed on my own, the director and teachers were busy with the children and holiday activities. I visited two classrooms during today’s visit. The classrooms were monitored for staff/child ratios, group sizes, capacity, supervision, interactions, discipline, materials, activities, postings and health and safety. Children were observed having free play and using the restroom and washing hands. Teachers were observed interacting with the children as they moved around to different activities. In classroom one there was one two-year-old child and eleven three-year-old children. The ratios were posted on the wall, and the center is following minimum staff/child ratios. The ratios for children that are two years old is 1:10. You may never have more than 10 children if a two year old child is present. Two children that were three years old were moved to classroom 2 with the 4- and 5-year-old children so the ratio was in compliance in classroom 1. A violation was cited regarding staff/child ratios, and the violation was corrected during the visit. A follow-up visit will need to be conducted to confirm staff/child ratios are in compliance. Outdoor inspections: Not completed since 9/29/25. Outdoor inspections must be completed monthly. Create a way to remind yourself to complete the outdoor inspections, either by documenting on a wall calendar, phone or place a clip board by the door to the outdoor play area. A violation was cited since outdoor play area inspections were not completed since 9/29/25. Fire drills: The last fire drill was completed 11/24/25. A fire drill is planned for today. Attendance/arrival and departure times: Current Incident log: Incident reports were found in children’s files. The incident reports should be with an incident log together. Place the log in a notebook and place the incident reports behind the log in the order they occurred. Medication: No medications. Transportation: Children had permission to transport for specific field trips to the library. QRIS: We were unable to discuss this information during today’s visit due to holiday activities with the children and their parents. Staff files: Five staff files were reviewed today. Four of the five files were missing orientation documentation. Staff qualifications and training requirements are essential for adults who interact with and teach children. These requirements protect children in childcare facilities by ensuring these facilities provide a physically safe and healthy environment where the developmental needs of children are being met and where children are cared for by qualified staff. Each staff person shall receive 16 hours of orientation within the first 6 weeks of employment. Six of the hours are required to be completed within the first two weeks of employment. When documenting orientation, please ensure that all required information is entered onto the form. You can conduct orientation training as a group with new staff instead of one on one. Ensure you and applicable staff members reflect the date, hours, and signature on the orientation training form. We referenced adding any additional information to the bottom of the orientation form that is reviewed with the employees. One employee did not have current CPR and first aid training and has been employed since 9/20/25. All five employees did not complete the recognizing and responding to child maltreatment training that was due within 90 days of employment. All seven files were missing the parent sign off for receipt of the Operational policies, parent participation plan and permission to be outside the fenced area. One child was missing the prevention of shaken baby syndrome policy sign off. One child was missing the receipt of the NC Summary of the Law. I have a parent sign off page I can email to you to use if you would like to have all of the information on one page. (this was provided to you previously) I noticed the discipline policy did not include the child’s date of enrollment on the form that was modified for the center. Some of the children’s files had the DCDEE discipline policy on file which includes the date of enrollment. I did not cite a violation for not having the date of enrollment on the discipline policy. Go back through all children’s records to confirm that all children have the same discipline policy on file and it includes the date of enrollment. It is important to create procedures to maintain organized children’s records and to confirm all children have all required information on file. I recommend that all parents complete all required paperwork PRIOR to enrollment and parents sign off on forms received. Use the children’s file checklist on the DCDEE website and place the information in the same order as it is listed on the checklist. This makes it easier to maintain and monitor. Violation Number Comment Rule 114 A summary of the NC Child Care Law was not given to a parent of every child enrolled in the center. A copy of the NC Summary of the law was in the child's file but was not shared with the parent on or before the first day of attendance. Two three year old children were moved to classroom 2. GS 110-91(7);.0713(a-d) 428 A current activity plan was not posted for each group of children for reference. Classrooms one and two did not have an activity plan posted. GS 110-91(12); .0508(a) 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. Four employees did not have orientation documentation on file. Repeat violation cited 10/14/2025 .1101(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. First aid training was not completed within 60 days of employment. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One employee did not have CPR training within 60 days of employment. .1102(d) 1203 Operational policies were not discussed with parents on or before the child's first day and/or they were not notified in writing of all changes. All seven files were missing receipt for permission to be outside the fenced area. .1005(b)(4) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. Four employees did not complete the required training within 90 days of employment. QRIS: We were unable to discuss this information during today’s visit due to holiday activities with the children and their parents. Staff files: Five staff files were reviewed today. Four of the five files were missing orientation documentation. Staff qualifications and training requirements are essential for adults who interact with and teach children. These requirements protect children in childcare facilities by ensuring these facilities provide a physically safe and healthy environment where the developmental needs of children are being met and where children are cared for by qualified staff. Each staff person shall receive 16 hours of orientation within the first 6 weeks of employment. Six of the hours are required to be completed within the first two weeks of employment. When documenting orientation, please ensure that all required information is entered onto the form. You can conduct orientation training as a group with new staff instead of one on one. Ensure you and applicable staff members reflect the date, hours, and signature on the orientation training form. We referenced adding any additional information to the bottom of the orientation form that is reviewed with the employees. One employee did not have current CPR and first aid training and has been employed since 9/20/25. All five employees did not complete the recognizing and responding to child maltreatment training that was due within 90 days of employment. The trainings are self paced and online. https://www.swcdcinc.org/ Children’s files: I reviewed seven children’s records during today’s visit. All seven files were missing the parent sign off for receipt of the Operational policies, parent participation plan and permission to be outside the fenced area. One child was missing the prevention of shaken baby syndrome policy sign off. One child was missing the receipt of the NC Summary of the Law. I have a parent sign off page I can email to you to use if you would like to have all of the information on one page. (this was provided to you previously) I noticed the discipline policy did not include the child’s date of enrollment on the form that was modified for the center. Some of the children’s files had the DCDEE discipline policy on file which includes the date of enrollment. I did not cite a violation for not having the date of enrollment on the discipline policy. Go back through all children’s records to confirm that all children have the same discipline policy on file and it includes the date of enrollment. It is important to create procedures to maintain organized children’s records and to confirm all children have all required information on file. I recommend that all parents complete all required paperwork PRIOR to enrollment and parents sign off on forms received. Use the children’s file checklist on the DCDEE website and place the information in the same order as it is listed on the checklist. This makes it easier to maintain and monitor. Violation Number Comment Rule 114 A summary of the NC Child Care Law was not given to a parent of every child enrolled in the center. A copy of the NC Summary of the law was in the child's file but was not shared with the parent on or before the first day of attendance. GS 110-102 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. In classroom 1, there were 12 children ages 2-3 years old. Two three year old children were moved to classroom 2. GS 110-91(7);.0713(a-d) 428 A current activity plan was not posted for each group of children for reference. Classrooms one and two did not have an activity plan posted. GS 110-91(12); .0508(a) 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. Four employees did not have orientation documentation on file. Repeat violation cited 10/14/2025 .1101(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. First aid training was not completed within 60 days of employment. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One employee did not have CPR training within 60 days of employment. .1102(d) 1203 Operational policies were not discussed with parents on or before the child's first day and/or they were not notified in writing of all changes. All seven children's files were missing the parent receipt of the operational policies. 10A NCAC 09 .0514(b) 1207 Parent participation plan was not discussed with parents on or before the child's first day of attendance and/or a copy was not given to them or posted in the center. All seven files were missing receipt from the parent that the parent participation plan was reviewed with them. 10A NCAC 09 .0515(a) 1322 A written statement from each child's parent giving standing permission which may be valid for up to twelve months for participation in off premise activities that occur on a regular basis was not available. All seven files were missing receipt for permission to be outside the fenced area. .1005(b)(4) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. Four employees did not complete the required training within 90 days of employment. One other employee has two days to complete the required training. .1102(g) The violations cited during today’s visit must be corrected immediately. Email me a corrective action letter with supporting documentation to show the date the violation was corrected, how the violation was corrected and how you plan to prevent repeat violations in the future. I should receive the letter within 2 weeks of today’s visit, 1/5/25. I allowed three more days since the center will be closed all next week.
