A full assessment of child care requirements was conducted. Your program currently operates with a Three-star license. The facility was monitored for approved curriculum. Today’s visit was conducted with you, Justin Casper, Center Director and Jackie Cook, Assistant Director. I monitored classrooms in use for compliance with staff/child ratios, supervision, capacity, group size and staff interaction/discipline. Children throughout the facility were returning from outside, completing handwashing and toileting routines, preparing for and eating lunch, participating in group time, and interacting with each other. Children also transitioned to rest time. Staff assisted children by leading group time, sitting with children during lunch, talking with children about recent activities that included the solar system, and interactions. Staff rubbed children backs while the children rested. In the classroom for younger children, staff completed diapering, feeding, and monitored sleeping infants. They documented the infants’ sleep position on the sleep charts. Staff sat on the floor while talking to and interacting with the infants and toddlers. Children were able to have floor play time and moved around the area freely. The facility was monitored for health and safety requirements both indoors and outdoors. One violation regarding health and safety was documented. Program requirements, equipment, and furnishings were monitored. All inspections and drills were current. A sampling of children’s and existing staff records, and all new staff records were monitored and information documented on the children’s records form and the staff and training worksheet. One violation regarding children’s records was documented. A checklist was also used to monitor compliance and one additional violation regarding nutrition was documented. The last annual compliance visit was conducted on 5/7/2025. The owner/operator is listed as current/active with the NC Secretary of State. Three vans used at different times to transport children were monitored. Violation Number Comment Rule 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. Stored in drawers under the counter in some classrooms serving children ages one and two years of age, some of the following items were found: plastic gloves, foam magic erasers, foam pretend eggs, plastic grocery bag, and plastic storage bags. One classroom serving children aged two was a container of foam rings. .0604(q) 1311 Emergency medical care information was not on file in the center on the child's first day of attendance and/or was not updated as changes occurred or at least annually for each child. Four children's file did not have annually updated emergency medical care information. .0802(c) 1792 Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. One staff member was drinking a soda in the presence of children. .0901(i) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. Children this age explore with their mouths, and the items pose a choking and suffocation hazard. Today staff immediately started working to remove those items. The Styrofoam play rings were removed. Continue to remove all the items. Emergency Information – Today you explained that you have already sent parents the information to update the children’s emergency information. Nutrition – Today you explained that the staff member does not normally drink soda. I explained that the staff must mimic good eating habits in front of children. ADDITIONAL INFORMATION: CPR/First Aid – Today we discussed that all staff would need to have actual copies of their CPR/FA cards in their files. Some staff had cards in their individual email. I reviewed the Clean Water for Carolina Kids site, https://www.cleanwaterforuskids.org/en/carolina/, and observed your facility has completed the requirements for lead in water testing. You are awaiting Survey review by RTI for lead in paint and asbestos. You may want to reach out to them to see when this will be completed. Today’s visit was conducted with you, Justin Casper, Center Director and Jackie Cook, another administrative staff member. You visited each child care area with me. I monitored classrooms in use for compliance with staff/child ratios, supervision, capacity, group size and staff interaction/discipline. Children throughout the facility were completing art activities related to Mother’s Day, listening to books being read about Mother’s Day, playing in activity areas, playing outside, participating in picture day, participating in group time, reading independently, completing routine tasks, and preparing for and eating lunch. Infants were being fed, having floor play time, and playing in an indoor gross motor area. Children later napped or had quiet activities. Staff assisted children with routines such as handwashing, toileting, meal time, and transitions. Staff met children’s developmental needs. The facility was monitored for health and safety requirements both indoors and outdoors. Program requirements, along with equipment and furnishings, were monitored. All inspections and drills were current. A sampling of children’s and staff’s records, including eight new staff records were monitored. One violation regarding children’s records was documented. Your program currently operates with a Three-Star license. You accompanied me as I walked through the facility today. I observed in specific classrooms for an extended time. Throughout the child care I observed children’s art work related to current and past activity plans. Children used chalk, paint, construction paper, crayons, and other items to create artwork that was displayed throughout the classrooms. Staff and children talked about what to expect when they went outside. They discussed rules while children put on their coats independently and staff offered help when needed. Children throughout the child care played in activity areas, participated in group time, had outdoor play, ate lunch, napped, or had quiet activity time, and completed transitions and routine care. Staff assisted children with activities, routines, allowed children to lead conversations, and met children’s developmental needs. In the room for infants, children listened to a book, music, and the teacher singing. They moved on the floor freely and gravitated towards the staff. Staff sat with the infants while having floor play time, conducted routine care activities, and met the children’s developmental needs. When an infant needed assistance, the staff immediately comforted the children. Supervision, group size, permit capacity, staff/child ratios and space capacity complied. Eight new staff’s files were monitored. Other staff’s qualifying letter and special training complied. One violation related to first aid certifications was cited. Children’s records were not monitored. Program records complied. Fire and sanitation inspections were current. Emergency medical care plan was accessible. ADDITIONAL REQUIREMENTS: General health requirements complied. Medications were stored properly. The facility continues to meet requirements for adequate and approved space. Permission to administer emergency medications must be updated every six months by the parent. We discussed the procedure to update emergency medication authorizations. Today the staff immediately contacted the parent. CONSULTATION: Today we discussed concerns regarding staff access to trainings in Spanish. I explained I would reach out to the DCDEE staff who approve training to get information. We also discussed the helpline that is available for staff who may want to access resources for managing classrooms. Staff may contact 1-888-600-1685 option one, to speak with someone directly. Today we discussed the types of foam or plastics that children under age three may not have access to. ABCMS (new staff): In G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility, within five business days of employment. The process of notifying the Division has changed and is now captured in Automated Background Check Management System (ABCMS). This change has been in effect since February 2024. You will need to link you and your staff. Center directors/owners can now access the ABCMS, the DCDEE’s criminal background check system. This access will allow providers to: -See the real-time background check status of staff members. -Run a printable report of the staff roster to assist with compliance visits. -See new background check applicants and add to staff roster. Access to the ABCMS requires the successful completion of a Moodle course consisting of a short video followed by a test. The course is titled ABCMS Child Care Provider Portal Training - under the Early Childhood Professional Development Tab, then under the Criminal Background (CBC) tab. A staff member failed to attend to a sixteen-month-old child in a nurturing manner causing the child to sustain a significant bruise to the child's arm. G.S. 110-91(10) 904 Child was handled roughly. A staff member bit a sixteen-month-old child on the arm. .1803(a)(1) 906 Discipline was delegated to another child. A staff member encouraged a child to bite another child as a form of discipline. .1803(a)(3) 1810 There was a substantiation of child maltreatment. On February 13, 2024, the DCDEE determined child maltreatment occurred when a staff member bit a sixteen-month-old child and caused a significant bruise. Upon arrival, I was greeted by the directors, Justin Caspar and Jacqulene Cook. Mr. Caspar accompanied me on my walk-through of the facility and Ms. Cook assisted me with the paperwork. I completed a walk-through of the facility. At the time of my visit there were a total of 181 children present. The children were observed engaged in circle time, eating lunch and going down for nap. I observed three infant rooms today. The staff in each class were observed feeding the children, engaged in floor activities with them and putting the infants down for nap. LICENSE STATUS Currently this center operates with a Three Star license issued November 20, 2020. INSPECTIONS I reviewed all required inspections including fire drills, emergency drills, sanitation, fire, and outdoor inspections. The last fire inspection was completed on June 29, 2022. The facility director has already requested to have a new inspection conducted. MONITORING During today’s visit, a full assessment was conducted, monitoring all space occupied by the children. A selection of staff files were reviewed. A selection of children’s files was reviewed. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. A checklist was used to note the requirements I monitored today. The checklist will be provided at the conclusion of the visit. The following violations were cited during today's visit. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection was completed on June 29, 2022. 10A NCAC 09 .0304(a) 533 Breast milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. Two bottles were observed with no lids. Two bottles were observed with the wrong date on them. Two bottles were observed with no date on them. 15A NCAC 18A .2804(d) 871 Center staff did not comply with the safe sleep policy. Safe sleep charts were observed to have a "T" for tummy documented when infants were first placed in their crib for nap time. 10A NCAC 09 .0606(a) 1882 Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. Thirteen diaper creams were not accompanied by the required medication authorization form. .0803(6)(a-i); .0803(7)(a-g); .0803(8)(a-d) COMPLIANCE HISTORY According to NC General Statute all Child Care programs must maintain a compliance history of at least seventy-five percent (75%) for the past 18 months or during the length of time the facility has operated, whichever is less. Any violations documented during visits to your program may have an impact on the total compliance history score and cause your score to drop below the mandated level. **Prior to today’s visit, the compliance history score for the center was 96%. Any violations cited during today's visit may affect this percentage. TECHNICAL ASSISTANCE *To maintain compliance with labeling bottles and diaper cream permission forms, you can send a reminder email to parents reminding them of the rules and policies regarding bottles and diaper creams and how their assistance is imperative to maintain compliance. Also review the rules with your staff and assist them in developing a schedule to monitor all medications and the required paperwork needed to maintain compliance (i.e. review all medication forms on a weekly basis). *Have all staff review the Safe Sleep policy and sign off that they understand the policy and procedures that are required to be met in order to maintain a safe environment and remain in compliance. CONSULTATION **Use your upcoming Staff Development Day to conduct a rules review and have all staff sign off that they participated and understand the rules that must be followed to maintain compliance at the facility. Important areas for review, Safe Sleep Policy, staying within arms reach of children during snack and mealtimes, maintaining compliance with required classroom paperwork (i.e. medication forms and required documentation, dates on lesson plans, names on water bottles, names and dates on baby bottles, etc.) **As summer is coming to a close, use this opportunity to clean up all binders and update required emergency information on staff and children. Clean and organize each van. Make sure all documents (i.e. vehicle registration) are located in the same place on each van. **DCDEE in collaboration with other agencies have started a challenging behavior hotline.
