This facility currently operates with a Five (5) Star License that was issued on 6-20-25. The last sanitation inspection was completed on 7-18-25 with an “Superior” classification. The last approved fire inspection was conducted on 8-28-25. The NC Secretary of State website was reviewed today and this facility’s corporation’s name, SEG Inc. was listed as “Current-Active.” Today’s visit was conducted with you, Cindy Brusca, the center director. I monitored the five (5) classrooms in use for compliance with staff/child ratios, supervision, and staff interaction/discipline. The program was also monitored for health and safety requirements indoors using the annual compliance monitoring checklist. The renovated playground space was observed today. You have installed approved turf, added space for soccer lessons, four (4) new pieces of stationary equipment, and a giant 4-in-a-Row Connect Game. You also showed me the children’s oasis outside of space #1. You have created a tranquil garden where the children have planted pansies, tulips, lemon balm, violas, and will soon start to plant herbs and vegetables. There are 10 planters in their garden and birds frequent the beautiful blue bird feeder/house and there is a suncatcher reflecting light. The children can sit and relax or have snacks at the three (3) handicap picnic tables in the garden. Today children were observed completed daily activities with adequate supervision and engagement from the caregivers. The children were observed at the table with manipulatives, in circle time where the caregiver was reading a book to the children, engaged in activity centers, and the children were observed completing toileting and handwashing routines. The program requirements, along with equipment and furnishings were observed. Compliance with staff records was documented by you. New staff records, along with children’s records were reviewed today. Medication observed on the premises were also monitored. This facility does not provide transportation. The following violation of child care requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance was provided. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The activity plan posted in space #5 was dated March 9 to March 13, 2026. GS 110-91(12); .0508(a) The following were discussed with you during today’s visit: • It was recommended to post the updated November 2025 version of the Summary of the NC Law & Rules for Child Care Centers that was emailed to you today. • Quality Rating and Improvement System (QRIS), is North Carolina’s new star rating system for early childhood education facilities. QRIS is a systemic approach to assess, improve, and communicate the level of quality in child care. Today the exit interview was being completed with COGNIA and they indicated a recommendation of full COGNIA accreditation with no monitoring. You have intentions on applying for Pathway 3: Accreditation Pathway once you receive the COGNIA accreditation, which should occur within the next six (6) to (8) eight weeks. • It is strongly recommended that staff complete the NC Foundations for Early Learning and Development training. The purpose of the North Carolina Foundations for Early Learning and Development is to establish a framework that supports the healthy development and school readiness of children from birth to age 5. This framework is designed to guide early childhood caregivers in creating an environment that fosters learning and development. This 5-hour course provides a clear set of expectations and goals that caregivers can use to plan and implement their teaching practices for supporting children in their learning and development. The NC Foundations for Early Learning and Development training is offered online and is self-paced/on demand by Southwestern Child Development Commission for $30. The following violations of child care requirements were cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance was provided. Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. In space #4 there was no permission to administer Benadryl to a child. 10A NCAC 09 .0803(1)(a & b) 1834 Application did not have a medical action plan attached for any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services. The Seizure Action Plan in space #1 was dated 8-7-23. .0801(b) The above violations must be corrected immediately and a letter must be sent to me no later than 4-21-25 stating how the items were corrected and a plan to maintain compliance with the requirements. Technical Assistance was provided with the following suggestions that may be beneficial: Medication Authorization: Any medication on the premises must be labeled with the child’s name or a prescription label, be accompanied by a permission slip, filled out completely with the parent/guardian’s signature and date. Set a reminder to review/update medications in each classroom. You must remember the “five rights” of medication use: the right child, the right medication, the right time, the right dose, and the right route—all of which are generally regarded as a standard for safe medication practices. You and the caregivers must know what medication the parent/guardian authorized to be administered and how the parent/guardian wants medication given. If documentation is required after the medication is administered, then documentation with the required information must be completed and kept on file for six months. Having clear, detailed procedures for obtaining administered medication protects children from mistakes that can affect their health. It was suggested to reiterate in your monthly newsletter to parents that medication must accompany a signed permission slip and it was suggested all staff complete training on administering medications. Set reminders often to check medications for the required medical action plans. • Remember to keep on file, all manufacture instructions from your new playground installation and from your poured rubber surfacing. • Emergency medications were observed locked, please refrain from locking these medications to ensure ease of access. These medications do not have to be locked, just maintained 5 feet off the floor. • You inquired about access to the ABCMS Portal to connect your staff today. As we looked at the ABCMS Provider portal, your NCID did not allow you access. After inquiring with a representative from the Criminal Record Check Unit, they indicated that you would need to set up a new individual NCID username name and password. The caregivers were nurturing and attentive as they assisted the children with the daily routines. The program requirements, along with equipment and furnishings were observed. The main purpose of the physical exam is to maintain the health and safety of all children enrolled at the facility. • Please send to me your completed staff and training worksheets tomorrow. • Hold harmless has been extended until the new QRIS (Star Rated License System) rules are implemented (SB 425). This facility currently operates with a Three (3) Star License that was issued on 2-14-23. The last sanitation inspection was completed on 3-5-24 with an “Superior” classification. The last approved fire inspection was conducted on 9-3-24. The NC Secretary of State website was reviewed today and this facility’s corporation’s name, SEG, Incorporated was listed as “Current-Active.” Today’s visit was conducted with you, Kelsey Desrosiers, office administrator. I monitored the five (5) classrooms in use for compliance with staff/child ratios, supervision, and staff interaction/discipline. The program was also monitored for health and safety requirements both indoors and outdoors using the annual compliance monitoring checklist. Children completed daily activities with adequate supervision and engagement from the caregivers. The children were observed in circle time where the caregiver was reading the children a book, another group of children were learning about the letter “G” and making “grape art“ in the shape of the letter G, another group of children were celebrating “hat day” and the children paraded around showing off their hats. Other activities occurred, as one group of children had a volcano experiment, and another group measured with string the distance from one place to another on a map. The caregivers were nurturing and attentive as they assisted the children with the daily routines. The program requirements, along with equipment and furnishings were observed.
