A full assessment of child care requirements was conducted. Your program currently operates with a Five-star license. Today’s visit was conducted with you, Alexandera Roberts, Center Director. You visited each child care area with me. I monitored classrooms in use for compliance with staff/child ratios, supervision, capacity, group size and staff interaction/discipline. Children throughout the facility were participated in small group activities. Half the classroom played outdoors while the other half stayed inside. Later some groups joined the other children outdoors. Children also ate snack, completed handwashing, and toileting activities. I observed proper handwashing, diapering, and toileting routines. Staff assisted the children when needed during routine care activities. Staff also sat on the floor and interacted with children, sat with children for activities that required additional supervision, and talked with them about their activities. The facility was monitored for health and safety requirements both indoors and outdoors. Program requirements, equipment, and furnishings were monitored. One violation regarding program records was documented. All inspections and drills were current. A sampling of children’s and existing staff’s records, and all new staff records was monitored and information documented on the children’s records form and the staff and training worksheet. Information regarding compliance was documented on an Annual Compliance Monitoring Checklist. The last annual compliance visit was conducted 5/21/2025. The owner/operator is listed as current/active with the NC Secretary of State. The facility does not transport children. Violation Number Comment Rule 853 Incident logs were not completed and maintained as required. All incidents including those that did not require medical assistance were not documented on the incident log. .0802(g)(1-6) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before May 19, 2026, I must receive a letter of compliance explaining how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. Be sure to include your facility’s name and identification number. If sufficient information is not received by the due date, a follow-up visit may be conducted. There are staff who are on leave that will be added when they return. You also explained one staff has been locked out of NCID. You are awaiting a visit for the lead in paint and asbestos testing from RTI. The visit summary was reviewed and left with you today. A full assessment of child care requirements was conducted. Your program currently operates with a Five-Star license. Today’s visit was conducted with you, Brandi Nobles, Center Director and Anita Peele, Assistant Director. You visited each child care area with me. I monitored classrooms in use for compliance with staff/child ratios, supervision, capacity, group size and staff interaction/discipline. Children throughout the facility were playing outside, playing in activity areas, participating in group time, listening to a book being read, and having music and movement. In some classrooms children participated in smaller group activities outdoors while the other group played indoors. At some point, the groups would switch when one returned indoors and the other went outdoors. Infants had floor play time, manipulated toys, moved around the indoor space, and interacted with staff. Proper handwashing and diapering procedures were observed. Staff assisted children with using materials, talked with them about the next activities, assisted with transitions, and helped children with interactions when needed. Staff met children’s developmental needs. The facility was monitored for health and safety requirements both indoors and outdoors. Program requirements, along with equipment and furnishings, were monitored. All inspections and drills were current. A sampling of children’s and staff records, including new staff records, were monitored. A checklist was also used to monitor compliance. A violation regarding medication and one regarding medical action plan was cited. The last annual compliance visit was conducted 6/18/2024. The owner/operator is listed as current/active with the NC Secretary of State. The facility does not provide transportation. Violation Number Comment Rule 1835 The medical action plan was not updated on an annual basis or when changes to the plan were made by the child's parent or health care professional. The medical action plan was completed April 2024 and has not been updated. .0801(b) 1882 Medication authorization, giving the caregiver standing authorization did not meet the specifications in rule. Emergency medication authorization for allergy and seizure medication expired April 2025 and March 2025 and was not updated every six months. .0803(6)(a-i); .0803(7)(a-g); .0803(8)(a-d) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before 6/4/2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 TECHNICAL ASSISTANCE: Having authorization to administer emergency medication is key to ensure the correct instructions are followed for the correct medication. Authorization to administer emergency medications must be updated every 6 months by the parent. Today you stated you did not realize the authorization was not current and you would have the parents update the forms. Having a medical action plan in place when caring for children with health issues is vital in ensuring their health and safety. These health issues could include allergies, asthma, or other chronic conditions that require specialized health services. The maintenance of these plans and the ability to implement such plans for the treatment of reactions are essential. It is important to remember that a medical action plan must be attached to the child’s application for enrollment, completed by the child's parent or a health care professional and must be updated on an annual basis. CONSULTATION/REMINDERS: Lead Free Classrooms: It appears you have completed the lead in water testing requirements. You have started the lead in paint and asbestos requirements.
