The facilities last annual compliance visit was conducted on October 16th, 2024. A checklist was used to note the requirements I monitored today. The center currently operates with a 5-star license, operating with enhanced ratios. My visit was conducted with you, owner/director, Farrah Dickerson and the center staff. The children were observed participating in tummy time, feeding and personal care and napping in the infant room and free play, center play in the other classrooms. or outdoor play. Lunch served today followed USDA guidelines for children in childcare. During the walkthrough to obtain enrollment and conduct my observations I monitored for required posted documents (permit/ restrictions, sanitation grade, EMC plan, first aid/CPR poster, safe sleep poster), interactions between staff and children, discipline, supervision, group sizes, space capacity, program requirements, equipment, furnishings . VERIFICATION OF RECORDS: *Last fire inspection= 9/19/2025- The fire inspection was due in July 2025 and was not requested and completed. During today’s visit I contacted City of Raleigh fire inspector Grady who arrived during today’s visit and conducted the center’s visit. *Sanitation inspection=5/28/2025 with 7 demerits and a superior rating *On September 19th, 2025, I reviewed the NC Secretary of State’s website and observed Bridge Academy of Wake Forest, LLC owner of the facility Bridge Academy) listed as current-active. Facility now uses the curriculum Funny Daffer a DCDEE approved curriculum *The following item of non-compliance was observed today. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The facilities yearly fire inspection was due in July 2025 and was not requested. The fire inspector was contacted by phone today and an inspection was conducted. 10A NCAC 09 .0304(a) 318 Children between 12 and 24 months of age were grouped with children 3 years of age or older. Children ages one through three were being cared for together in the early preschool classroom. 10A NCAC 09 .0713(a)(6) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One staff person's training expired and is due to attend training to renew. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff person's training expired and is due to attend training to renew. .1102(d) 1065 Child care providers scheduled to work in the infant room, did not complete ITS-SIDS training within two months of employment or did not complete the training every three years. Child care administrators did not complete the ITS-SIDS training within 90 days of employment and every three years thereafter. The administrators ITS-SIDS has expired and is due to be renewed on 9/23/2025. .1102(f) Violation was cited during today’s visit. A letter of correction is required and to be submitted by October 3rd, 2025, listing the item number and the violation cited and explaining how this was put in compliance and how it will be kept in compliance going forward. *****TECHNICAL ASSISTANCE/REMINDERS: ******* Group sizes: Depending on the ratios the facility is required to meet; the group sizes will vary. Group sizes are in place so teachers can maintain supervision, and to ensure the safety of the children. You should be following the ratio based on the youngest child in the group throughout the day. This includes the first and last hour of the operating day. ITS-SIDS: All staff working with infants must complete the Infant/Toddler Safe Sleep and SIDS Risk Reduction Training developed by the NC Healthy Start Foundation and administered by the North Carolina Child Care Health and Safety Resource Center for the Division of Child Development and Early Education for caregivers of children ages 12 months and younger. Although the training shall be completed within two months of an individual assuming responsibilities in the infant room and the administrator shall complete ITS-SIDS training within 90 days of employment; at all times, one child care provider who has completed ITS-SIDS training shall be present in the infant room while children are in care. A certificate of each staff member's completion of this course shall be maintained in the staff member's file in the center. CPR/First Aid: a. Must be approved agency b. Face-to-face only; no on-line or blended courses c. Counts towards Health & Safety Training d. Renew every two years before the expiration date on the certification card (does not count towards on-going training hours) e. Violation Number Comment Rule 843 A drug or medicine was administered after its expiration date. Review of an enrolled child's epi-pen found that it had expired in August 2024. 10A NCAC 09 .0803(1)(d) 9995 A violation was found for which there is no item number. Per sanitation rule: 15A NCAC 18A .2806 FOOD STORAGE AND PROTECTION (a) In child care centers, food shall be stored in approved, clean, tightly covered, storage containers once the original package is opened. Approved containers include resealable bags and other containers made of plastic or glass. Reusable containers that come in direct contact with food must be easy to clean, in good repair and intended for food storage I observed a box of rice, a box of mashed potatoes and a goldfish crackers that were previously opened and taped shut. Violation was cited during today’s visit. A letter of correction is required and to be submitted by October 28th, 2024, listing the item number and the violation cited and explaining how this was put in compliance and how it will be kept in compliance going forward. *****TECHNICAL ASSISTANCE/REMINDERS: ******* Medical Action Plans - Growing up healthy is every child's right and every parent's dream for their children. Significant advances in improving the overall health and development of children with chronic illnesses have been made. Having a medical action plan in place when caring for children with health issues is vital in ensuring their health and safety. These health issues could include allergies, asthma, or other chronic conditions that require specialized health services. The maintenance of these plans and the ability to implement such plans for the treatment of reactions are essential. It is important to remember that a medical action plan must be attached to the child’s application for enrollment, completed by the child's parent or a health care professional and must be updated on an annual basis. Permission to administer emergency medications must be updated every 6 months by the parent. EPI-PENS and all prescribed or OTC medications: All medications whether over the counter or prescribed should be checked periodically or documented somewhere that teachers see and can keep track of expiration dates so products can be replaced prior to expiration by parents.
