    Name of Operation: BRENTWOOD CHILD CARE - THE FOUNDATION OF KNOWLEDGE Facility ID: 92003234 Consultant: ELIZABETH LESTER Operation Type: Center Case Number: 0823-224L Visit Date: 8/21/2023 Number Present: 61 Completed Date: 8/21/2023 Age: From 0 To 10 Total Minutes: 140 Time In: 09:50 AM Time Out: 12:10 PM Time In: Time Out: List to Use: Center Type Of Visit: Complaint Visit Announced/Unannounced: Unannounced The purpose of today’s visit is to investigate allegations of non-compliance with the NC Child Care Requirements. A report was received by the agency on August 17, 2023. Upon arrival, the assistant director Shante Loftin, greeted me and assisted me throughout my visit. Office manager, Khalil Rossi arrived later in the visit and also assisted me. ALLEGATION There is a concern of inadequate supervision in the classroom for two-year-old children. ALLEGATION OBSERVATIONS AND DISCUSSIONS -I discussed the allegation with the Ms. Loftin and she was not present at the time of the incident, however, she had some knowledge and provided me with the incident report. I discussed the incident with Mr. Rossi and he stated that he spoke with the child’s mother the evening of August 10, 2023, the date of the incident. I reviewed email communication discussing the injury. He also took picture of the injury. Mr. Rossi stated that he spoke to the teachers that were with the child on 8/10/23. Each teacher stated that they were not aware of the injury and they did not see the child get hurt while in their care. -I reviewed the pictures, the incident report, email communication, a follow-up letter sent to the child’s mother. I observed an injury on the child’s face from the pictures that were taken on the afternoon of the incident, the report stated that “no one saw” the injury. The email stated that the child’s mother spoke with Mr. Rossi about the injury. The follow-up letter stated that they did not recall anything happening that day and he has spoken to the staff about “being more vigilant and aware of all the students’ well-being”. -I interviewed two teachers. The lead teacher stated that she does a daily check, and she did not see the injury the morning of 8/10/23. She has a notebook where she makes notes of any concerns she might have. If she has a concern, she notifies the assistant director. Then she will communicate with the families if needed. I reviewed the notebook and there was not an entry about this incident. -I interviewed the co-teacher and she also stated that she was not aware of and did not witness any incident that caused an injury. She stated that she did not see the injury. -Lastly, I reviewed another incident report dated 8/14/23 and it clearly explained the injury and how it happened. The staff immediately notify the parent and explained what happened. RESOLUTION Based on my discussions with staff, review of documentation and pictures. This allegation has been determined to be SUBSTANTIATED. The following violation was observed or documented. Violation Number Comment Rule 303 Children were not adequately supervised at all times. There was an unexplained injury in the classroom for two-year olds. .1801(a)(1-5) The above violation must be corrected immediately. Adequate supervision shall mean that: (4) staff must provide supervision appropriate to the individual age, needs, and capabilities of each child **We discussed steps taken to ensure this will not happen again. He stated that they are having a staff meeting on September 1, 2023. He said that they are going over supervision, daily health checks and re-checks, and classroom management.
