This facility currently operates with a Four Star License that was issued on 9-20-19. Today’s visit was conducted with you, Tonya Kangkolo, owner/provider. Upon arrival to the residence, I was greeted by you. The children were observed in the living room, (next to the kitchen) the room used for child care. The children were in circle time where you talked about the nocturnal dormouse, sang songs and you read the children a book. The children prepared for naptime and slept on cots and in pack-n-plays. You were nurturing and communicative as you met the children’s needs. I monitored the room next to the kitchen that is used for child care. Compliance with capacity, supervision, discipline, attendance, program requirements, storage of hazardous substances, CPR & First Aid, permit restrictions, the required postings, and staff record documentation was monitored as a partial assessment of applicable child care requirements. A partial assessment of child care requirements was monitored. The following violation was cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance was provided. Violation Number Comment Rule 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. A health assessment was not on file for one child who had been enrolled for more than 30 days. GS 110-91(1); 10A NCAC 09.1721(a)(1) The above violation must be corrected immediately and a letter must be sent to me no later than 12-16-25 stating how the item was corrected and a plan to maintain compliance with the requirements. Maintaining the health assessment of file can contribute to the overall health and wellbeing of the children in your care, ensuring that the children receive the necessary support and care to thrive. It was suggested to have parents submit the health assessment and immunizations on the first day of enrollment or prior to enrollment to ensure you don’t have to monitor for the 30 day requirement of submitting the medical records. • Invoices for the Annual License Fee were emailed to facilities on November 3, 2025. Payments were due by December 3, 2025. All payments must be submitted through our website using a Mastercard or Visa debit/credit card or via e-check. Please be aware that failure to pay the license fee may result in the revocation of your child care license. Unpaid license fees will also incur a 10% late fee and interest as established by NCGS § 147-86.23. • Your files are maintained in a crate in 3-ring binders very neatly, however it still took some time to monitor for the required paperwork. Natural and man-made disasters include but are not limited to the following: fire, tornado, flood, power failures, chemical spills, bomb threats, earthquakes, blizzards, nuclear disasters, or a dangerous person(s) in the vicinity. As trained staff you are required to review the EPR Plan annually and update the plan as needed. Additional caregivers who are present must be informed of the EPR Plan and its location. You stated that you could not access your EPR Plan. The issue may be your NCID password needs to be updated. If you do not log into your EPR Plan within twelve (12) months, you are locked out of the account. It is recommended that you log into the Risk Management Portal to access your EPR Plan at least every six (6) months to prevent being locked out. Contact the NCID Help Desk https://it.nc.gov/support/ncid/reset-password-unlock-account or call the service desk at 800-722-3946 if you can’t access your EPR Plan after changing your password. Also, it is strongly suggested to set a reminder on your calendar and phone to update/log into your EPR Plan every six (6) months. Closed Doors: Closed doors are unsafe for sleeping children because you cannot readily see what is happening in the room, potentially missing signs of distress or potential hazards and in case of an emergency like a fire, a closed door can delay your ability to quickly reach the children and evacuate them. Leave doors open and frequently check on sleeping children. You are aware that if the children are under twelve (12) months old, then sleep charts must be completed every fifteen (15) minutes documenting the child’s sleep position. • It took a long time to review paperwork today. Please take time to reorganize your paperwork using the children’s file checklist, provider file checklist and program file checklist that were provided for you today. • Please link you and your household member asap. Center directors/family child care owners can now access the Automated Background Check Management System (ABCMS), the DCDEE’s criminal background check system. This access will allow providers to: • See the real-time background check status of household members. • Run a printable report of the roster to assist with compliance visits. Access to the ABCMS requires the successful completion of a Moodle course consisting of a short video followed by a test. The course is titled ABCMS Child Care Provider Portal Training - under the Early Childhood Professional Development Tab, then under the Criminal Background (CBC) tab. Today the “Creating Connecting Applications in the Automated Background Check Management System (ABCMS)” flyer with instructions was emailed to you. Invoices will be emailed to the facility address on file by 11-30-24. Online payments will be due by December 31, 2024. Reporting for 2024-2025 has been extended due to Hurricane Helene. Children have a grace period from requirements until 11-30-24, and reports will be accepted until 1-15-25.
