This facility currently operates with a Five (5) Star License that was issued on 8-27-19. Today’s visit was conducted with you, Rebecca Krantz, center director. During today’s visit, I discussed the information regarding the self report of a lack of supervision with you. Based on information obtained, the following was determined. You stated that the lack of supervision was caused by a staff member not adequately adhering to the Beth Meyer Preschool Child Supervision Policy. The staff member failed to complete the action in the policy that states “when children leave the room or playground, the last adult in any room must thoroughly check the entire room and bathroom for students.” Based on my conversation with you, the self-report related to supervision is substantiated. See violation below. Brief observations during today’s visit did not yield any further supervision concerns as children were adequately supervised. One classroom was observed coming inside from the playground. The caregiver counted the children as they lined up against the fence. The children walked to their classroom with one caregiver in front of the children and one caregiver at the end of the line of children. Once the children reached their classroom door, the children were counted, and the door was held for them to walk inside the classroom as the children were counted once again. Limited monitoring of child care requirements occurred during today’s visit. The following violation of child care requirements was cited and must be corrected immediately. Technical assistance on how to maintain compliance was provided. Violation Number Comment Rule 303 Children were not adequately supervised at all times. On 9-4-25 a child was left on the smaller playground for less than five minutes, when the classroom for two year olds transitioned inside from the smaller playground. .1801(a)(1-5) The above violation must be corrected immediately and a corrective action letter must be sent to me no later than 10-2-25 stating how the violation has been corrected and a plan to maintain compliance with the requirements. The following were discussed with you during today’s visit: • Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. • Technical assistance on how to maintain compliance with supervision was provided. Staff must supervise children to ensure their safety and prevent accidents, as young children are active, impulsive, and lack the awareness to protect themselves. Effective supervision supports children's development by allowing providers to monitor their progress, intervene in social interactions, and foster a nurturing environment. Furthermore, maintaining proper supervision is a legal and ethical responsibility to the families who entrust their children to the care of the facility. You stated that you take this lack in supervision incident very seriously and want to assure me that immediate action was taken to rectify the situation and prevent any such incidents from occurring in the future. You indicated that as a result of this incident, the staff member that left the child unsupervised elected to resign from Beth Meyer Preschool. You indicated that you have reviewed your Child Supervision Policy with all staff and have had them re-sign acknowledgement of review and receipt of this policy. You added that staff will also be retrained on outdoor safety and transitions during the next teacher workday on October 20, 2025. • Quality Rating and Improvement System (QRIS), is North Carolina’s new star rating system for early childhood education facilities. QRIS is a systemic approach to assess, improve, and communicate the level of quality in child care. It encourages programs to engage in continuous quality improvement and families may use the star rating to make informed decisions when choosing a child care facility. Today it was explained that you can review the new QRIS requirements, which includes three pathways to choose for the rated license process. You stated that you were interested in being assessed through pathway 2 which seems to best align with your facility. The last approved fire inspection was conducted on 3-3-25. The NC Secretary of State website was reviewed today and this facility’s corporation’s name, Beth Meyer Preschool, Inc. was listed as “Current-Active.” Today’s visit was conducted with you, Rebecca Krantz, center director. I monitored the six (6) classrooms in use for compliance with staff/child ratios, supervision, and staff interaction/discipline. The program was also monitored for health and safety requirements both indoors and outdoors using the annual compliance monitoring checklist. Children completed daily activities with adequate supervision and engagement from the caregivers. The children were observed outside on the playgrounds, interacting in activity centers, eating food brought from home, and completing toileting and handwashing routines. The program requirements, along with equipment and furnishings were observed. Compliance with staff records was documented by you. New staff records, along with children’s records were reviewed today. Medications and permission to administer medications were observed on the premises and monitored. This facility does not provide transportation. The following violation of child care requirements was cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance was provided. Staff orientation is crucial because it helps share information about care routines, improve customer service, ensure quality service, train staff, foster a safe and nurturing environment, and helps build the best childcare program. You must ensure that each new staff member who is expected to have contact with children receives a minimum of 16 clock hours of on-site orientation within the first six weeks of employment. As part of this orientation, each staff member shall complete six clock hours of orientation within the first two weeks of employment. • Post the updated version of the Summary of the NC Child Care Law dated February 2025. • Please finish connecting all of your staff in the ABCMS Portal. • Today you explained that the playground for older preschoolers was going to be renovated with a new stationary play structure and poured rubber installed this summer. This facility currently operates with a Five (5) Star License that was issued on 8-27-19. The last sanitation inspection was completed on 11-13-23 with an “Superior” classification. The last approved fire inspection was conducted on 3-1-24. The NC Secretary of State website was reviewed today and this facility’s corporation’s name, Beth Meyer Preschool, Inc. was listed as “Current-Active.” Today’s visit was conducted with you, Crystal Bass, assistant director. I monitored the six (6) classrooms in use for compliance with staff/child ratios, supervision, and staff interaction/discipline. The program was also monitored for health and safety requirements both indoors and outdoors using the annual compliance monitoring checklist. Children completed daily activities with adequate supervision. The theme of the week is Bugs and the children engaged in bug art activities. One class used dot markers to decorate butterflies, and another class talked about and explored bugs habitat by making a “bug hotel.” Another class decorated butterflies by using pipettes filled with various colors of watercolor paint. In circle time the children talked about today being the last week of school. The children sang bug songs and took group photos. Appropriate handwashing and diapering routines were also observed. The program requirements, along with equipment and furnishings were observed. Compliance with staff records was documented by you. New staff records, along with children’s records were reviewed today. Medications and permission to administer medications were observed and monitored on the premises today. This facility does not provide transportation. The following violation of child care requirements was cited during today’s visit and must be corrected immediately. Technical assistance on how to maintain compliance was provided. Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months.Two staff members did not have a staff medical report on file. 10A NCAC 09 .0701(a) The above violation must be corrected immediately and a corrective action letter must be sent to me no later than 6-18-24 stating how the violation was corrected and a plan to maintain compliance with the requirements. The following were discussed with you during today’s visit: • Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. • Technical Assistance was provided with the following suggestion that may be beneficial: • We discussed record keeping is one of the most time-consuming tasks associated with administering a child care program. It is an important part of licensure. As an administrator you must allow time to review all necessary documentation to ensure accuracy. Set a reminder to review staff files often to ensure the program is maintaining accurate licensing records. Organization and completeness of records is important to ensure easy access to information in emergency situations. We discussed making sure that you develop a record keeping system for staff and children’s records that promotes organization and efficiency. Administrative staff must allow time to review all necessary documentation prior to employment of staff to ensure the applicant brings in the three required items (staff medical report, TB test results and criminal record qualifying letter) prior to employment. Using a checklist to check and date receipt of documents will help you track employment documents. A staff file checklist is available on the DCDEE website under the Provider tab and provider documents links. Please refer to the Staff and Training Worksheets to continue to keep all staff members current with their upcoming due dates for CPR/First Aid training, child maltreatment training, and health and safety training requirements • A suggestion was made to save the most recent child care requirements in Chapter 9 effective January 2024, to your desktop computer for easy access. • We discussed that play is an important part of a child’s development. Playing outside in the fresh air can be fun and adventurous. With careful planning and monitoring, play environments can be safe for children. There have been numerous reports of snakes on the playground in outdoor environments. Remember child care rule 10A NCAC 09 .0604 reads that once a day, prior to initial use, the indoor and outdoor premises shall be checked for debris, vandalism, and broken equipment, in addition look for hazards/snakes. • The NC Child Care Health and Safety Resource Center is a valuable resource where you can find skills-based training and technical assistance from your Child Care Health Consultant on specific medications or health conditions, as well as the new poster on Stand-Up Changing Procedures. Sign up for their quarterly newsletter and find updated resources at www.healthychildcare.unc.edu or 800-367-2229, choose option 1, then 2. • The Healthy Social Behaviors (HSB) Project supports teachers to promote healthy social-emotional development and reduce the expulsion rate among young children in licensed child care centers across North Carolina. Specialists are available with early childhood education backgrounds who are passionate about empowering teachers to develop learning environments and teaching practices that promote prosocial skills in young children.
