Applicable childcare requirements and the inspection was discussed with Owner, Devona Dancey. Currently this facility operates with a 4 star license effective 9/30/25. During the visit I monitored the indoor and outdoor spaces used for child care. I observed children during routine activities. Age appropriate materials and equipment was accessible to all children. The facility was monitored for compliance using the annual compliance monitoring checklist. I also monitored, permit restrictions, posting requirements, training requirements, use of approved space, program records, and nutrition. 5 children's records were reviewed and staff records were reviewed and the verification form completed. The last fire drill was completed on 1/5/26. The last lockdown drill was completed 1/5/26. Outdoor inspections were observed to be completed monthly. This operation does not care for infants at this time so applicable rules were not reviewed. Prior to caring for infants, Ms. Dancey must obtain training in ITS-SIDS. This operation does not have any children that have any emergency medications at this time. Transportation is not provided by this facility. This operation was observed to have completed enrollment in asbestos and lead paint testing. One cat previously listed as being in the home is no longer present. One cat present in the home was observed to have expired vaccinations. This cat does not come into contact with the children present. I observed 1 violation during this inspection. Violation Number Comment Rule 907 Written record of up-to-date pet vaccinations as required by NC law and local ordinances, including rabies vaccination for dogs and cats, was not available. The records available for review for one cat in the home were observed to be expired. .1719(b)(1) The violation documented above must be corrected immediately. A signed and dated letter of compliance must be mailed by January 27, 2026 stating that all violations have been corrected. If you cannot meet the requirements by January 27, 2026 you must contact me with a proposed timeline of the corrections. In some cases the timeline may be extended. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited during today's visit may negatively impact your compliance score. TECHNICAL ASSISTANCE The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. No cost connecting applications can be submitted by employees to get them listed under your facility. ABCMS guides were emailed during inspection. A temporary license was issued effective January 21, 2025 to July 21, 2025. Children may not be in care on the second level of the home. Upon arrival, the children were observed changing activities. The children enjoyed playing with playdough and working on Mr. Potato head. The provider was Interacting with the children as they played. The two activities merged together and the children created Mr. Potato heads with playdough and used the eyes, feet, lips, ears and hats to create their own. They had a really good time. The provider assisted children as they cleaned up after their activities. She helped them sort the playdough tools and place other items in containers. She assisted the children with problem solving. Postings: Observed in compliance. No changes. Children files were reviewed again to verify that medicals and immunizations were on file. All files contained the required information. Written plan of care: UPDATE: This form is not required if you do not complete routine tasks while children are in care. The form is on file for the 5 preschool age children enrolled. Reminders: Direct supervision is required when the children are using the sensory table. The small craft balls are considered choking hazards for children under 3 years old. Medications: No children with medications. Transportation: Not provided. Aquatic activities: Not provided. Attendance: Records were current and complete. DCDEE requires at least one year of attendance records to be available for review. If you provide care for children that have vouchers, keep the attendance records for 3 years so they can be reviewed if needed. Emergency drills: The last fire drill was completed April 4, 2025. A fire drill should be completed before May 4, 2025. May 4, 2025, falls on a Sunday. A shelter in place drill was conducted April 16, 2025. Outdoor play area: No concerns observed during the visit. Outdoor inspections: Current and completed as required. FCCERS 3-month self-study: You started working on the self-study. I will review the self-study documentation during the last monitoring visit to finalize the rated license assessment. Education was verified in WORKS. You have an AAS degree in Early Childhood Development and have 21 years’ experience as a licensed family childcare home provider. Quality point: Pending You completed the rated license application during today’s visit. The rated license will be processed 30 days before the temporary license expires. Let me know when you receive the license by mail. Activity plan/materials: The activity plan was current but the topic was not. The plan was dated for April/May but the topic was WINTER. Update the activity plan so it matches the activities that you provide. The age range on the license was changed to show ages 1-12 years old. The provider does not plan to offer care for infants under age one. The Recognizing and Responding to Child Maltreatment training March 13, 2025. You had the certificate on file. Violation Number Comment Rule 930 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years from the date of issuance). The qualifying letter expired February 12, 2025. G.S. 110-90.2 & .2703(n) & (o) The violation cited today should be corrected immediately. Mail or email me a letter stating how the violation was corrected, the date the violation was corrected and how you plan to prevent repeat violations in the future.
