I Upon arrival, I checked in at the office and I was greeted by the director Modesti Cavanaugh. I completed a walk-through of the facility. At the time of the visit, there were a total of 80 children present. The children were observed playing outside, enjoying a ride in the bye-bye buggy, participating in circle time, and preparing for lunch and nap/rest time. LICENSE STATUS Currently this center operates with a Notice of Compliance GS110 license issues October 11, 2019. INSPECTIONS I reviewed required inspections including fire drills, emergency drills, sanitation, fire and playground inspections. MONITORING During today’s visit, a partial assessment was conducted, monitoring all space occupied by the children. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. I reviewed two staff files. The following violation was documented: Violation Number Comment Rule 847 Parent's medication authorization did not include required information. Two children that had emergency medication did not have the correct permission to administer forms. 10A NCAC 09 .0803(4)(6-9) Please mail or email your letter of compliance by October 21, 2025. The letter must address the violation and describe how the violations were corrected. Any violations documented during visits to your program may have an impact on the total compliance history score and cause your score to drop below the mandated level. **Prior to today’s visit, the compliance history score for the center was 98%. Any violations cited during today's visit may affect this percentage. TECHNICAL ASSISTANCE: **During the walk-through. I reviewed emergency medications and required paperwork. The medication permission for two children was not completed for the correct medication. CONSULTATION >>I noticed the children’s blankets stored on the children cots. Please tuck the blankets under the cot sheet. So, the blankets do not touch the other child’s cot. ABCMS We discussed at length how to set-up the ABCMS portal. I explained that you will need to complete the webinar and link all your staff. I left instructions and I sent an email explaining how to find the MOODLE training. QRIS This facility is a Notice of Compliance GS 110 and does not participate in the star rating system. REMINDER CLEAN WATER/WATER TESTING Rule, 10A NCAC 41C .1001 through .1007 and 15A NCAC 18A .2816 require all child care facilities (homes and centers) to test for lead in drinking, water, identify lead-based paint hazard, and mitigate or restrict access to identified hazards. The program is free and the first step is to watch one of the pre-enrollment webinars, which are available at www.cleanwaterforUSkids.org/carolina or paste https://bit.ly/3CK-webinar into your web browser. Please check your lead water testing date. Per sanitation this must be completed every three (3) years. Upon arrival I checked in at the main office. I completed a walk-through with the director Modesti Cavanaugh. At the time of the visit, there were a total of 62 children present. The children were observed playing outside, lining up to use the bathroom, and the infants were enjoying tummy time, napping and feeding. LICENSE STATUS Currently this center operates with Notice of compliance issued November 29, 2005. I reviewed the NC Secretary of State's website and observed that the owner is listed as current/active. INSPECTIONS I reviewed all required inspections including fire drills, emergency drills, sanitation, fire and outdoor inspections. The fire inspection was conducted by did not pass due to the school sprinkler system. A work order has been submitted. MONITORING During today’s visit a full assessment was conducted, monitoring all space occupied by the children, including the outdoor play environment. I reviewed a selection of staff and children files. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. A checklist was used to note the requirements I monitored today. The following violations were observed and documented: Violation Number Comment Rule 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. One child''s file was missing their health assessment. GS110-91(1) 1322 A written statement from each child's parent giving standing permission which may be valid for up to twelve months for participation in off premise activities that occur on a regular basis was not available. This was missing from one child's file. .1005(b)(4) 1908 A child's file did not have a statement with parent signature acknowledging receipt and explanation of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy and/or the acknowledgement did not have all the required information. This was missing from one child's file. .0608(b)(1-6) Please mail or email your letter of compliance by April 1, 2024. The letter must address each violation and describe how the violations were corrected. Please remember that each child must have a medical/health assessment. **One child’s file (E.M.) was missing a signed statement that they received The Shaken Baby and Abusive Head Trauma policy and a signed statement that they give permission go outside the fenced area. You contacted the family via email while I was present during today’s visit. **We also discussed plastic bags and that they need to be five foot or higher or locked up, so children can not reach the bags. **We discussed the fire inspection and once all the items are corrected, please send me the report. REMINDERS Please check your lead water testing date. Per sanitation this must be completed every three (3) years.
