Katie Lewis, DPI off-site administrator, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Lewis, Kendall Grigg, principal, and Genevieve Meyer, assistant principal. Limited monitoring of child care requirements occurred during today’s visit. Violation Number Comment Rule 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. Staff members in the classroom for four and five-year-old children pinched children and sent children inside the bathroom to address challenging behaviors. This facility operates with a Four Star license issued on 8-27-18. The last annual compliance visit was conducted on 10-3-23. The last fire inspection was completed on 10-16-23. The last sanitation inspection was completed on 6-11-24 with a “Superior” classification. Today’s visit was conducted with you, Ms. Chandra Farmer, Lead Teacher. This facility has one (1) NC Pre-K classroom occupying space #204. The NC Pre-K program operates for a minimum of 7.0 hours a day from 8:45am – 3:30pm and attendance documenting time of arrival and departure is taken daily. USDA approved meals are served and menus are planned at least one week in advance. The center uses the approved Creative Curriculum Teaching Strategies Gold (TSG) as an early childhood teaching resource. The NC Pre-K classroom abides by a 1:9 staff/child ratio. NC Pre-K paperwork was reviewed during today’s visit. Three (3) children’s files were randomly selected verifying health assessments and/or developmental screenings were on file. The DCDEE NC Pre-K Program Monitoring Tool is used as a guide to ensure compliance with NC Pre-K requirements. Today the children were in the cafeteria where they were eating and conversating with you and the instructional assistant/teacher assistant. When the children returned to the classroom the children were ready for naptime. They rested on cots with linens as they listened to soft music. Toileting and handwashing routines were completed after naptime. The children were then observed reading books on the carpet and having music and movement on the carpet. The animation of the Penguin Song and Bear Hunt songs were displayed on the Smart Board and the children moved to the instructions and sang the songs. The children were observed choosing manipulatives to engage with at the tables. The facility was monitored for staff/child ratios, supervision, discipline, and health and safety requirements indoors. There was heavy rain during the visit and the outdoor area was not monitored. The program requirements, along with equipment and furnishings were also observed. Compliance with staff records was documented by you and your new staff records, along with three (3) children’s records were reviewed today. This facility is not approved for transportation. There is one child with food allergies and the permission to administer the medication was observed. The medication is maintained in the main office with the school nurse. A full assessment of applicable child care requirements was conducted today. The following violation of child care requirements was cited and must be corrected immediately. Technical assistance on how to maintain requirements was provided. Violation Number Comment Rule 1834 Application did not have a medical action plan attached for any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services. An allergy action plan was not on file for one child with allergies. .0801(b) The above violation must be corrected immediately and a letter must be sent to me no later than 10-3-24 stating how the item was corrected and a plan to maintain compliance with the requirements. The following were discussed with you during today’s visit: • Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. • Technical Assistance was provided with the following suggestions that may be beneficial: Allergy Action Plan: An allergy action plan is required because it helps ensure that all staff are prepared to respond to an allergic reaction. An allergy action plan provides clear instructions on what to do in an emergency, such as when and how to administer medication. Parents should provide the completed allergy action plan along with the prescription labeled medication and the completed permission to administer the medication. Remember that the permission to administer the medication is only good for six months. The allergy action plan is good for one year from the date that the parent signed the plan. It was suggested that you monitor compliance with medications to ensure all of the required paperwork is current and on file. • A suggestion was made to add a photo of the child with the allergy and list their allergy prominently. An alternate allergy list with space for a photo was provided today. • It was strongly recommended to keep emergency medication for asthma and allergies in the classroom with the child. Children with these conditions can, at times, have life-threatening symptoms. They may need these medications that could save their lives. Staff must give these medications quickly when a child has these symptoms. Staff will want emergency medications nearby instead of in the school office. • Today we discussed the Nutrition Opt Out form. This form is only used when the parent/guardian is providing all the food and beverages that the child will consume during each school day. • Hold harmless has been extended until the new QRIS (Star Rated License System) rules are implemented (SB 425). Providers in Cohort 1 are not required to continue with a rated license assessment unless preferring to, and providers in Cohort 2 do not need to start their preparation year assessment unless preferring to move forward with an assessment. Prep year scales and assessment year scales are optional and up to your discretion at this time. • You were reminded that your new instructional assistant needs to complete CPR/First Aid and Maltreatment training within ninety (90) days of employment. • The NC Child Care Health and Safety Resource Center is a valuable resource where you can find skills-based training and technical assistance from your Child Care Health Consultant on specific medications or health conditions, as well as the new poster on Stand-Up Changing Procedures. Center directors/owners can now access the Automated Background Check Management System (ABCMS), the DCDEE’s criminal background check system. This access will allow providers to: • See the real-time background check status of staff members. • Run a printable report of the staff roster to assist with compliance visits. • See new background check applicants and add to staff roster. Access to the ABCMS requires the successful completion of a Moodle course consisting of a short video followed by a test. The course is titled ABCMS Child Care Provider Portal Training - under the Early Childhood Professional Development Tab, then under the Criminal Background (CBC) tab. • During your previous visit you stated that you wanted to wait until your next visit to be evaluated for the reassessment of stars. During today’s visit you stated that you were ready to have your full rated license assessment completed since having your Environment Rating Scales competed in April 2024. With your new instructional assistant on board, their education standards need to be evaluated so your education points can be calculated accurately. A follow-up visit will be conducted so all components of the star rated license can be assessed and points awarded for a new star rated license. • Please continue to visit DCDEE’s website for child care updates at https://ncchildcare.ncdhhs.gov/.
