The FCCH operates with a 3 STAR rated license. You are licensed to care for 8 children ages 0-12 and of these 8 children, only 5 children can be preschool aged. Today's visit was conducted with Cynthia Kornegay, owner/operator. Today, I completed a walk-through of the FCCH. Upon arrival, there were 4 children present ranging in ages 1– 4 years old. The children were observed having circle time and then transitioned to playing in centers, having lunch and going down for nap. Records: Provider and program records were monitored during today's visit. The attendance records were not completed for yesterday and today (2/13/2025 and 2/14/2025). Ms. Kornegay updated the records during my visit today and we discussed the importance of keeping all required record keeping up to date at all times. I also monitored the required posted documents for compliance. CPR, First Aid and SIDS were all current and on file. Ms. Kornegay has a current CBC but she was having trouble printing her most recent qualifying letter. I informed her she can all the CBC Unit at 919-814-6401 for assistance. Ms. Kornegay stated there are no other household members living in the home with her. All children’s records were monitored today and observed to be in compliance. Ms. Kornegay completed her Health and Safety trainings on 10/12/2024. Ms. Kornegay has completed 9.5 of her 10 required annual training hours. She stated she is planning to complete two trainings for the Food Program that will take her over her 10 required annual training hours for this year. Emergency telephone numbers were readily accessible and emergency procedures were posted. A written daily schedule was posted. The daily schedule reflected outside time. Fire drills, EPR drills and outdoor playground inspections were monitored and I did observe that fire drills were not conducted monthly as required. The last fire drill was conducted in December 2024. I also found Ms. Kornegay’s EPR plan to be expired. Please remember that fire drills must be conducted monthly and EPR plans need to be reviewed annually. The FCCH operates on city water so no bacteriological testing is required. Child Care Space: The main area that the children use is a converted bedroom on the side of the home. The children also use the living room which is also located on the front side of the home. During my walkthrough of the provider’s home today, I observed the kitchen and laundry room accessible to the children. There were a variety of cleaning products and laundry detergent in both areas that should also not be accessible to children. For outside play, the children use the back yard as well as the front yard and driveway. The shed off the back of the house was unlocked today. The hot water heater is stored there along with gardening materials. The door must remain locked at all times during child care hours. I also observed an open bag of planting soil accessible to the children in the backyard as well. Ms. Kornegay is aware that all hazardous items such as planting soil must remain locked away and out of the reach of the children at all times. The smoke detector and fire extinguisher were observed to be in working order during my visit today. Transportation requirements were monitored today. Ms. Kornegay did not have an updated insurance card for me to monitor. I informed Ms. Kornegay she will not be approved for transportation until I have a copy of her updated insurance card. Ms. Kornegay drives a Chrysler Van with license plate KAD 6978. I reminded Ms. Kornegay to make sure all of the emergency information on her child care children is up to date at all times especially when transporting her child care children. I suggested she also keep emergency information on herself with the information she has on her child care children. *****Violations****** Five violations of child care requirements regarding program records/files and maintaining a safe indoor and outdoor environment were observed during my visit today. A corrective action letter stating how the violations were corrected is due to me no later than February 28, 2025, and can be mailed to e-mailed to me to the address at the bottom of this report. Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. The kitchen and laundry room areas were accessible to the children today. Hazardous items such as cleaning supplies, dish detergent, bleach, and laundry detergent were accessible to the children. .1719 (a)(7) 921 Operator did not maintain accurate daily attendance records including documentation of arrival and departure for all children in care, including the operator's own preschool children. Attendance was not completed for the past two days, February 13, 2025 and February 14, 2025. .1721(e)(6) 1853 The operator did not conduct a monthly fire drill. The last fire drill was documented as conducted in December 2024.Fire drills must be conducted and documented on a monthly basis. .1719(a)(15) & .1721( e)(2) 1886 Required locked storage was not secure with combination locks, electronic, or magnetic devices, key, or equivalent locking devices. The outside storage shed was found to be unlocked today. The water heater and outdoor supplies are housed in this area. This outside storage area must remain locked during child care hours. .1719 (a) (7) 2056 Documentation of the operator's Emergency Preparedness and Response plan was not completed and/or maintained on the template provided by the Division of Emergency Management. The provider's EPR plan was found to be expired during today's visit. This document must be updated on an annual basis. 10A NCAC 09 .1714(c) & .1721(e)(1) The above violations must be corrected immediately, and a letter must be sent to me no later than 2-28-25 stating how the item was corrected and a plan to maintain compliance with the requirements. The following were discussed with you during today's visit: * Intentional planning of the environment ensures a safe environment has been created which prevents and reduces injuries to young children. Items that require locked storage were found to be accessible to child care children during today’s visit. Find a time either the last thing in the evening or the first thing in the morning, confirm all areas such as the kitchen, laundry room and bedrooms are all either locked or inaccessible to the children by using baby gates if necessary. It is extremely important to take the time necessary to confirm all child care space is safe at all times before children arrive for care each day. *The outdoor learning environment offers a sense of freedom for children. Children are able to play freely with peers, expand their imagination beyond restraints of indoor activities, release energy and explore their sense of touch, smell, taste and their sense of motion. Since children are exploring it is very important to provide a safe environment, free of debris. Each morning you should check all areas the children will have access to so you can confirm they are free of all hazards. * Motor vehicle crashes are the leading cause of death in children two to fourteen years of age in the United States. It is necessary for the safety of children to require that the caregiver/teacher comply with requirements governing the transportation of children in care, in the absence of the parent/guardian. Each child transported must have emergency contact information on file along with a photograph of each child. This is helpful in identifying children in the event of an emergency/accident. *Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. *Technical Assistance was provided with the following suggestions that may be beneficial: *A suggestion was made to save the most recent child care requirements in Chapter 9 effective November 1, 2024, to your desktop computer for easy access. Section 1700 is all of the Family Child Care Home rules. The FCCH operates with a 3 STAR rated license. You are licensed to care for 8 children ages 0-12 and of these 8 children, only 5 children can be preschool aged. Today's visit was conducted with Cynthia Kornegay, owner/operator. Today, I completed a walk-through of the FCCH. Upon arrival, there were 4 children present ranging in ages 2 – 4 years old. I did observe that arrival and departure times for the children were not current and on file. Ms. Kornegay stated she uses her Ring doorbell as her arrival and departure times and records them. I discussed that this is not a consistent method of documenting arrival and departure times of the children and reminded her to go back to using the clipboard that used to sit at the door. Ms. Kornegay stated she is researching to see if there is a new system perhaps using a barcode that she can have parents use as they drop off and pick up the children. I also monitored the required posted documents for compliance. CPR, First Aid and SIDS were all current and on file. A current CBC Qualifying letter was on file for Ms. Kornegay. Ms. Kornegay stated there are no other household members. I reminded Ms. Kornegay that her Criminal Background check will need to be renewed prior to October 7, 2024. Please remember that you can get this done up to 6 months prior to expiration. Ms. Kornegay inquired as to if mobile units are approved for having fingerprints completed. I advised Ms. Kornegay to check with the Criminal Record Check Unit to see if the particular company you are looking at is an approved provider. Procedures for those mobile units tend to require additional steps than just having fingerprints taken at the location on Hammond Road. All children’s records were monitored today and observed to be in compliance. Ms. Kornegay is now caring for an Infant on a drop in basis. Sleeping baby checks were monitored and observed to be in compliance. Ms. Kornegay completed her Health and Safety trainings 10-29-19 and we discussed today that you are due for renewal by 10-29-24. There were no completion certificates on file for fulfillment of your required ongoing training. As we discussed, Ms. Kornegay stated she has taken trainings but could not locate her certificates.
