Name of Operation: ASBURY PRESCHOOL Facility ID: 9255315 Consultant: MICHELE HIRSCH Operation Type: Center Case Number: Visit Date: 1/15/2026 Number Present: 65 Completed Date: 1/15/2026 Age: From 2 To 5 Total Minutes: 120 Time In: 10:06 AM Time Out: 12:06 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor compliance during your annual compliance visit. The facilities last annual compliance visit was January 24th, 2025. A checklist was used to note the requirements I monitored today. The center currently operates with a 5-star license, operating at 30sq. ft. inside, & enhanced ratios and holds a NAEYC accreditation. My visit was conducted with you, Stephanie Tarrats, Director, and center staff. The children were observed participating in teacher directed, free play and center play and circle time activities indoors. I also observed teachers and children bundled up for the cold windy weather during outdoor play. During the walkthrough to obtain enrollment and conduct my observations I monitored for required posted documents (license, EMC, sanitation, no smoking sign), interactions between staff and children, discipline, supervision, group sizes, space capacity, program requirements (fire drill, playground inspection, lockdown or shelter in place, equipment/toys and materials, furnishings, and medication requirements. VERIFICATION OF RECORDS: *Last fire inspection= 12/2/2025 and approved for daytime care only *Sanitation inspection= 11/10/2025 with 9 demerits and a superior rating *On January 24th,2025 I reviewed the NC Secretary of State’s website and observed Asbury United Methodist Church owner of the facility (Asbury Preschool) listed as current-active. * Compliance history as of 1/15/2026- 98% * EPR Plan last updated- 9/10/2025 * Fire Drill- 12/17/2025 @12:20pm * Lockdown- 12/15/2025 @9:45am * Playground inspection- 1/5/2026 *ABCMS- complete; we learned during today’s visit when there is something related to ABCMS and payments overdue that the portal is not accessible like we are used to and this financial situation needs to be immediately rectified to view your roster. *One violation was cited during today’s visit Violation Number Comment Rule 1320 Children's records that include an application for enrollment, medical and immunization records, and permission to seek emergency medical care was not on file for each child. During children's file review it was found that one child only JN only had an application and no additional paperwork on file for review. GS 110-91(1);.0302(d)(2); .0304(g) Technical Assistance: A violation of childcare requirements was cited today related to children’s files. It appears you have an electronic process to send and receive application packets from parents for their enrolled children, in order not to overlook a file like what occurred today a good idea would be to utilize your outlook calendar and document the name or initials of the incoming children and double check yourself to make sure everything is received and filed in a timely manner. Set reminders when things are missing. A violation was cited requires a letter of corrective action. Please list your facility name, ID, item # for the violation, how you came into compliance with the violation and how you will keep in compliance going forward. If you have any questions or concerns, please feel free to contact me.
