    Name of Operation: A Safe Place Child Enrichment Center Inc. #2 Facility ID: 92003954 Consultant: ELIZABETH LESTER Operation Type: Center Case Number: Visit Date: 4/16/2025 Number Present: 28 Completed Date: 4/16/2025 Age: From 0 To 5 Total Minutes: 150 Time In: 09:30 AM Time Out: 12:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor compliance during a Routine Unannounced visit. Upon arrival I was greeted by the director Ms. Simone Green. I completed a walk-through of the center with the director. Michele Remington, Licensing Supervisor, accompanied me during today’s visit. At the time of the visit, there were a total of 2children present. The children were observed transitioning to outside, playing with playdoh, playing outside, engaging in water play on the playground, and the infants were enjoying tummy time and one infant was finishing up breakfast. LICENSE STATUS Currently this center operates with five star license issued April 10, 2024. I reviewed the NC Secretary of State's website and observed that the owner is listed as current/active. INSPECTIONS I reviewed required inspections including fire drills, emergency drills, playground, sanitation and fire inspections. All inspections were found to be in compliance. MONITORING During today’s visit, a partial assessment was conducted, monitoring all space occupied by the children. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. I reviewed two new employee files for the required information. The following violations were documented: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The center received a failed fire inspection on 2/6/2025. 10A NCAC 09 .0304(a) 540 An individual written feeding plan was not provided by child's parent or health care provider or was not followed and posted. (omit posting for centers located in a residence) The infant feeding schedules were not posted for each child. The feeding schedules were in a notebook. 10A NCAC 09 .0902(a) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In the toddler room there was an unlocked closet with paint cans and other chemicals. The lock was on the door but not clasped and the key was in the lock. .2820(b) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One new employee did not complete this training within 90 days of employment. The staff member was hired in November 2024. .1102(g) Please mail or email your letter of compliance by April 26, 2025. The letter must address the violation and describe how the violations were corrected. Any violations documented during visits to your program may have an impact on the total compliance history score and cause your score to drop below the mandated level. **Prior to today’s visit, the compliance history score for the center was 100%. Any violations cited during today's visit may affect this percentage. TECHNICAL ASSISTANCE/CONSULTATION: ***We discussed the center’s fire inspection. I was able to speak with the owner, Kim Shaw, and it was explained that the inspection was failed due to renovation permits. Your center must have an annual passed fire inspection. The owner stated that she is working with the city of Raleigh to correct failed items. **All the children in the classroom for infants did not have their feeding plans posted or updated as the children’s needs change. **All chemicals or hazardous materials must be locked. In the classroom for toddlers there was a door with a lock, but the key was in the lock. Inside the closet there was paint cans and other chemicals. There was also a lower cabinet that was locked with inappropriate lock. You moved the Clorox wipes to the locked closet and removed the key and locked the closet door. **One new employee did not complete Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. The staff member was hired in November 2024. Please remember that each new employee must complete Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. WATER LEAD and ASBESTOS TESTING >>It is required that your center is free from lead and asbestos. You must complete a webinar and provide information on your center through https://www.cleanwaterforuskids.org/en/carolina/ and RTI. They will determine if you need further testing and will provide guidance. AGENCY UPDATES Hold harmless has been extended until the new QRIS (STAR rating) rules are implemented (SB 425). Providers in Cohort 1 are not required to continue their with a rated license assessment unless you want to, and providers in Cohort 2 do not need to start their preparation year assessment unless you want to. Prep year scales and assessment year scales are optional and up to your discretion at this time. Message from NCRLAP https://ncrlap.org/ 2024: “NC continues to use the Revised editions of each scale for official assessments. The Revised editions have spiral bindings on the side and not the top, so make sure to have the correct version when thinking about your next assessment. Beginning on February 1, 2025 the Third editions of the ECERS, ITERS, and FCCERS will be used for official assessments; these scales all have a spiral binding on the top. More information about these versions will be posted in the fall of 2024 to help programs get to know the new measures and think about future assessments”. ABCMS PORTAL Provider Access to ABCMS is Live! North Carolina Child Care Providers can obtain access to the Provider Access to ABCMS after completing the Moodle training and completing the form afterward. Once providers are logged in, you can access codes to allow your staff and applicants to create a connecting application to link themselves to your facility. **DCDEE in collaboration with other agencies have started a challenging behavior helpline. To Speak to a Behavior Support Advisor for advice and resources specific to the challenging behaviors in your classroom please call 1-888-600-1685 and choose Option 1.
