    Name of Operation: THE SUNSHINE HOUSE #148 Facility ID: 47000183 Consultant: ALECIA PASCHAL Operation Type: Center Case Number: Visit Date: 12/3/2025 Number Present: 34 Completed Date: 12/3/2025 Age: From 0 To 4 Total Minutes: 210 Time In: 10:30 AM Time Out: 02:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable child care requirements for an annual compliance visit. Brenda Monroe, director, was on the premises and I reviewed the attached visit summary documentation with her at the end of today’s visit. Your program currently operates with a three-star license, issued August 30, 2022, earning two (2) points in program, five (5) points in education and one (1) quality point in the programmic option. License restrictions include daytime care only and meets enhanced space. The last annual compliance visit was conducted on December 10, 2024. The sanitation inspection was completed on March 6, 2025, with a “Superior” classification. The last fire inspection was conducted on January 30, 2025, and your facility was approved for daytime care only. The Secretary of State website was checked on December 3, 2025, and The Sunshine House, INC was currently active. The center's compliance history was one hundred percent (100%) on December 3, 2025, and it was reviewed with the operator. The Staff and Training Worksheet, The Children's Records Worksheet, and the Annual Compliance Monitoring Checklist were completed during the visit. You visited each indoor and outdoor space with me. Children were observed in free play activities and teacher directed activities. Proper hand washing techniques were observed. Lunch consisted of sliced turkey, tortillas, mixed fruit, diced carrots and milk. The following violations were observed during today’s visit: Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. The composite structure on the preschool playground contained multiple areas that were cracked and peeling on the platforms and stairs. Bolts were rusted. G.S. 110-91(6); .0601(b) 1867 The depth of the loose surfacing was not based on critical height of the equipment. The sand used as resilient surfacing was less than 6 inches under the swings and the composite structure. .0605(k)(1-4) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before December 17, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. . Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. You were provided with a copy of the quality initiatives so you can review it. I provided you with a copy of the Facility CQI Plan, and I provided you with a copy of the Individual CQI and PD Plan. All staff should complete this annually. We discussed curriculum and formative assessments and NCFELD. We reviewed the educational requirements for a center. You plan on updating all staff in WORKS.
