Pamela O’Brien, principal, assisted me with today’s visit. The allegations are as follows: 1. There is a concern that the posted staff/child ratio is not being followed. 2. There is a concern of inadequate supervision. Staff/child ratio, group size, supervision, use of licensed space, space capacity, license restrictions, and the indoor environment were monitored. The license and emergency care plan were posted. Fire drills, medication administration, and storage of hazardous products were also monitored. There have been no new staff since my last visit. Additionally, I observed the indoor and outdoor space used by the children. I observed the teachers properly supervising and interacting and with the children while they returned to the classroom and began washing their hands. I interviewed the administrator and the lead teacher regarding supervision and the required staff: child ratio of children in the classroom. The administrator and the teachers described their supervision strategies in the classroom. The allegation that there was inadequate supervision was not substantiated. The administrator stated that they have additional staff who can assist teachers so staff: child ratios are always met. She reported that the drop off procedure to assure staff: child ratio is that parents in the car line drop off their children to her or another adult with a qualifying CBC letter and they are walked down to their classroom. All teaching staff are scheduled to be in their classroom at 7:00 am through 2:30 pm. The children begin arriving at 7:15 am. The substitute scheduled to be in the classroom did not come today. The assistant principal, D. Smith, went to the classroom to meet the required staff: child ratio until another substitute arrived. She stated that there were 11 (eleven) children in the classroom when she arrived. The teacher, N. Perez, stated that she was alone with 13 (thirteen) children from 7:25 am-7:45am. The required staff: child ratio is 1:10 in a NC Pre-K classroom. The allegation that the posted staff: child ratio is not being followed was substantiated. The following violation was documented. Violation Number Comment Rule 1775 NC Pre-K program staff/child ratios and group sizes were not met. The teacher in group one was alone with more than 10 children for approximately 20 minutes until a qualified staff arrived to meet the required staff/child ratios. This was corrected after 20 minutes. .3009 Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Due to the nature of violation documented, a follow-up visit will be completed. Technical Assistance: On staff: child ratio: Today, we discussed that assuring a substitute is available who holds all applicable requirements can be challenging. You have partnered with Annette Fernandez, NC Pre-K Coordinator, to ensure you always have additional staff available. You plan on adding to the procedures to assure that children are always in the required staff: child ratios. The lead teacher, in each classroom, assisted me with the visit. This program currently operates with a five-star license, issued May 9, 2023. License restrictions include daytime care only, no children under three years old, meets enhanced ratios minus one; meets enhanced ratios and meets enhanced space. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-nine percent (99%) as of November 4, 2024. The license was posted, and the restrictions were in compliance. The last annual compliance visit was completed on November 21, 2023. The last sanitation report was conducted March 25, 2024, with 0 demerits and received a superior status. The last fire inspection was conducted on September 12, 2024, and was approved for daytime care only. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor and outdoor environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in routine free play, transitions, and personal care routines. The teachers were interacting and meeting the developmental needs for each of the children. The Annual Compliance Monitoring Checklist, Children’s Records Worksheet and the Staff and Training Worksheet was completed today. Rated license requirements were reviewed to ensure compliance was maintained with current program and education components as well as the quality point. Lunch was chicken tenders, peaches, broccoli, mashed potatoes and milk. The following violations were observed: Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. There were two gallon sized hand lotion/soap and one gallon sized glass cleaner observed under the sink area in the bathroom, several Lysol wipe containers stored in an unlocked bin on the floor of the bathroom area. All items were corrected during visit. .2820(b) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). P. O'Brian did not complete and submit required forms to complete a criminal background check. Her qualification letter expired on August 14, 2024.
