The Notice of Compliance restrictions include daytime care only. Age range 1-5 years; capacity 53. The last annual compliance visit was conducted on May 9, 2025. The last annual fire inspection was dated November 4, 2025. The last sanitation inspectionwas dated November 21, 2025, with a superior classification. I visited the indoor and the outdoor area. The children were observed playing freely in center areas and on the playground. The Staff and Training Worksheet for new staff was completed today. Staff/child ratio, group size, supervision, use of licensed space, space capacity, license restrictions, and the outdoor environment were monitored. The Notice of Compliance and emergency care plan were posted. Fire drills, medication administration, and storage of hazardous products were also monitored. The last annual fire inspection was dated October 11, 2024. The last annual sanitation was dated April 1, 2025, with a superior classification. The program’s compliance history was ninety-five (95%) percent as of May 7, 2025. The NC Secretary of State website was reviewed on May 7, 2025, and Rockfish Christian Academy was listed as current- active. I visited the indoor and the outdoor area. The children were observed playing in center areas. The Children's Records Worksheet, and the Annual Compliance Monitoring Checklist were completed during the visit. The Staff and Training Worksheet was completed today. Staff/child ratio, group size, supervision, use of licensed space, space capacity, license restrictions, and the outdoor environment were monitored. The Notice of Compliance and emergency care plan were posted. Fire drills, medication administration, and storage of hazardous products were also monitored. The following violations were observed during today’s visit. Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. The last annual compliance visit was conducted June 13, 2023. The last annual fire inspection was dated February 28, 2023. The last annual sanitation was dated May 16, 2023, with an approved classification. Melani Huck, Administrator, was present and on the premises during today’s visit. This program currently operates under GS 110-106. The Notice of Compliance was issued September 25, 2013. The Notice of Compliance restrictions include daytime care only. Age range 1-5 years; capacity 53. Staff/child ratio, group size, supervision, use of licensed space, space capacity, license restrictions, and the outdoor environment were monitored. The license and emergency care plan were posted. Fire drills, medication administration, and storage of hazardous products were also monitored. When I arrived, I rang the doorbell. No one answered so I knocked on the door. Melanie Huck, administrator, and lead teacher, left her classroom to answer the door. The children remained in the classroom without supervision for approximately one minute. I introduced myself and explained the purpose of my visit. I visited all the indoor and outdoor spaces. The children were observed playing in centers, singing, and being attended to in a nurturing and appropriate manner. Lunch was observed and grilled cheese sandwiches, minestrone soup, oyster crackers, mixed fruit and milk were served. The following violations were observed during today’s visit: Violation Number Comment Rule 303 Children were not adequately supervised at all times. Four children were left in the classroom for approximately 1 minute while the teacher left the room to answer the front door. .1801(a)(1-5) 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. One outlet in the classroom was not covered. 10A NCAC 09 .0604(c) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance).
