License restrictions include daytime care only, meets enhanced space and children under 2 ½ years old in rooms with direct exits only. The last annual compliance visit was conducted on May 14, 2025. The sanitation inspection was completed on November 18, 2025, with a “Superior” classification. The last fire inspection was conducted on July 15, 2025, and your facility was approved for daytime care only. The program’s compliance history was one hundred percent (100%) as of today. Staff/child ratio, group size, supervision, use of licensed space, space capacity, license restrictions, and the outdoor environment were monitored. The license and emergency care plan were posted. Fire drills, medication administration, and storage of hazardous products were also monitored. I visited each indoor and outdoor space. Children were observed in free play activities and teacher directed activities. Proper hand washing techniques were observed. Children were observed during morning activities. Lunch consisted of carrots, raspberries, chicken and cheese on whole wheat bread and milk. The following violation was observed during today’s visit: Violation Number Comment Rule 1419 All commercially manufactured equipment and furnishings were not assembled and installed according to procedures specified by the manufacturer and/or the manufacturer's instructions were not kept on file or electronically accessible, if available. A mattresss located in space one was ripped and foam was exposed. .0601(b) 1898 Staff did not complete the health and safety training within one year of employment. .1102(a) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history. The violation observed was corrected prior to my visit so a letter of compliance was not required. QRIS Discussion: Today we reviewed the new QRIS Pathways, and you plan on pursuing the Classroom and Instructional Pathway. We reviewed the child care center requirements applicable to pathway 2. We reviewed the Family and Community Standards form. Keep documentation on how and when you meet the options. We also reviewed approved curriculums and formative assessments, the Facility and Continuous Quality Improvement plan and how to implement them. We reviewed the educational requirements for a center. You are currently updating your staff in WORKS. Make sure that the staff have been assessed for the position they are working in. Please submit your application for your star rated license and an updated staff and training worksheet no later than September 15, 2026.
