Ms. Wilson was on the premises and I reviewed this visit summary documentation with her at the end of today’s visit. Your program currently operates, with a four-star license issued December 8, 2022, earning two points program, seven points education and one quality point by meeting an education option. Restrictions included: first/second shift, fireplace/woodstove not used during operating hours and meets enhanced ratios. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-one percent (91%). The last annual compliance visit was conducted on October 30, 2026. The sanitation inspection was completed on November 24, 2025, with a “Superior” classification. The last fire inspection was conducted on January 7, 2026, and your facility was approved for daytime care only. The Staff and Training Worksheet was completed for one new staff member. The NC Secretary of State website was reviewed on November 9, 2023, and MaDears Developmental Day Care Center was listed as current active. I visited the indoor and outdoor space today. Children were observed in free play activities. Children were being attended to in a nurturing and appropriate manner and keeping with their developmental needs. Staff/child ratio, group size, supervision, use of licensed space, space capacity, license restrictions, and the outdoor environment were monitored. The license and emergency care plan were posted. Fire drills, medication administration, and storage of hazardous products were also monitored. Lunch was observed and consisted of chicken nuggets, corn, mixed fruit bread and milk. The following violations were observed during today’s visit: Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. K. Murchinson's First Aid certificate expired on 4/5/26. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. K. Kathleen Stephenson, owner, was on the premises and I reviewed this visit summary documentation with her assistant director, LaSonya DeBose, at the end of today’s visit. Your program currently operates, with a four-star license issued December 8, 2022, earning two points program, seven points education and one quality point by meeting an education option. Restrictions included: first/second shift, fireplace/woodstove not used during operating hours and meets enhanced ratios. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-seven percent (97%) as of today, November 9, 2023. The last annual compliance visit was conducted on November 16, 2022. The sanitation inspection was completed on July 11, 2023, with a “Superior” classification. The last fire inspection was conducted on February 9, 2023, and your facility was approved for daytime care only. The Staff and Training Worksheet, The Children's Records Worksheet, and the Annual Compliance Monitoring Checklist were completed during the visit. The NC Secretary of State website was reviewed on November 9, 2023, and MaDears Developmental Day Care Center was listed as current active. You visited each indoor and outdoor space with me. Children were observed in free play activities. Children under twelve months old received care according to individual needs including diapering and bottle feeding. Proper hand washing techniques and diaper changing procedures were observed. Lunch was observed and consisted of tomato sauce with ground beef, pasta, green peas, mixed fruit and milk. The following violation(s) was(were) observed during today’s visit: Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. The dome climber and metal car and the basketball goal had exposed rust. G.S. 110-91(6); .0601(b) 1898 Staff did not complete the health and safety training within one year of employment. You LaSonya DeBose, Administrative, was not on the premises. However, Kathleen Stephenson, Owner, was on the premises. Your program currently operates, with a four-star license issued December 8, 2022, earning two points program, seven points education and one quality point by meeting an education option. Restrictions included: first/second shift, fireplace/woodstove not used during operating hours and meets enhanced ratios. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-eight percent as of today, July 24, 2023. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, routine free play, transitions and personal care routines. The caregivers were acting and meeting the developmental needs for each of the children. The last fire inspection was dated February 9, 2023. The last sanitation report was dated July 11, 2023, with a superior classification and five demerits. Lunch was served and consisted of chicken with noodles, corn, mixed fruit, biscuits and milk. A violation was observed and corrected during today’s visit. Violation Number Comment Rule 887 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of one month. In space #2, there was one sleeping infant in the crib, with no daily sleeping chart completed on the child. .0606(g) No compliance letter shall be required. Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Your compliance history score before today’s visit was 98%. Technical Assistance was provided on the importance of sleeping charts. Basic knowledge on safe infant sleep practices and environments is that it reduce the risk of Sudden Infant Death Syndrome (SIDS) and other sleep-related infant deaths. SIDS is the death of an infant younger than 1 year of age that is unexplained after a thorough scene investigation, autopsy, and review of the clinical history. Documentation of sleeping charts shows that the caregiver are visually checking sleeping infants every fifteen minutes. Also, it shows the pattern of the sleeping infants, and it helps caregivers to see that the infant is sleeping peacefully and comfortable, and breathing properly. This is not only a child care rule but best practice for all caregivers to perform as they provide care in the infant room, and it is part of your safe sleep policy. I conducted a walk-through of the facility unaccompanied. During the visit, I spoke with LaSonya Debose, Administrator, and one additional staff member. Limited monitoring of child care requirements occurred during today’s visit. The following violations were observed and/or confirmed during today’s visit. Violation Number Comment Rule 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. A staff member failed to care for children in a nurturing or appropriate manner when the staff member, on multiple occasions, used a stick to "tap" the children to get their attention. G.S. 110-91(10) All violations documented above must be corrected immediately. A written, dated, and signed letter of compliance must be submitted to me within one week, by July 17, 2023, describing how and when the violations were corrected and how compliance will be maintained in the future.
