Name of Operation: LITTLE SUNFLOWERS PLAYHOUSE Facility ID: 47000316 Consultant: ALECIA PASCHAL Operation Type: Family CC Home Case Number: Visit Date: 9/22/2025 Number Present: 4 Completed Date: 9/22/2025 Age: From 2 To 3 Total Minutes: 240 Time In: 10:00 AM Time Out: 02:00 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Annual Comp w/Rated Lic Assess Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable child care requirements for an annual compliance with a rated license assessment visit. Tonia Peterkin, operator, assisted me with the visit. This program currently operates with a three-star license, issued November 06, 2022. License restrictions include first and second shift, maximum of five preschool children at any time, fireplace/woodstove not used during operating hours, and children in care on ground level only. The program’s compliance history was one hundred percent (100%) as of September 22, 2025, and was reviewed with the operator during today’s visit. Your last Annual Compliance visit was on February 7, 2025. A walk-through of the facility was completed today; all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Lunch was baked chicken, rice, sliced bread, green beans, apples and milk. Staff/child ratio, group size, supervision, use of licensed space, space capacity, license restrictions, and the outdoor environment were monitored. The license and emergency care plan were posted. Fire drills, medication administration, and storage of hazardous products were also monitored. Children's files were reviewed, the Verification of Required Information, the FCCH Annual Compliance Checklist and the FCCH Statement of Responsibility were completed. The following violation was observed during today’s visit: Violation Number Comment Rule 1847 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. T. Peterkin did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. G.S. 110-90.2 & .2703(r) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before October 6, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. You completed the Family and Community Engagement Standards Worksheet, and you chose two additional options to add to your current plan. You are using an approved curriculum and formative assessment. You completed your CQI and PD plan during today’s visit. I will process your application for a Five Star Rated license when I receive your letter of correction. At the completion of the visit, this visit summary and the FCCH enrollment worksheet, The Verification of Required Information, the FCCH Annual Compliance Checklist and the FCCH Statement of Responsibility completed and signed.
